The government has launched a consultation into radically changing the Statutory Scheme for branded medicines (known as the Statutory Scheme). The consultation comes as delicate negotiations for replacing the alternative Voluntary Scheme are underway, potentially undermining these talks, while also further damaging industry confidence in the UK as a viable place to research, launch and supply medicine. The government proposals seek to hold average revenue clawback rates under the Statutory Scheme at historic highs of between 21-27%, compared to the pre-pandemic averages of 9.4% for the Statutory Scheme (2019-2021), and 6.88% for the Voluntary Scheme (2014-2021). The accompanying cost-benefit analysis ignores any negative impact this may have on medicine supply and wrongly claims it will boost investment. The consultation comes on the heels of government data last week showing UK life sciences foreign direct investment (FDI) fell by 47% between 2021 and 2022, down by £900m year on year. This large fall in investment coincided with a rise in the main UK clawback rate under the Voluntary Scheme from 5% to 15%, and led to the UK falling from 2nd to 9th out of 18 comparator countries for life sciences FDI in 2022. The Voluntary Scheme clawback rate now sits at a record 26.5% in 2023.

The recent announcement that Royal Mail will be partnering with distance selling pharmacy (DSP) giant pharmacy2U highlights how standards of regulatory enforcement are being ignored to accommodate the DSP model. The brunt of these double standards hinges around the levelling down of temperature enforcement standards by the Medicines and Healthcare products Regulatory Agency (MHRA) which demands mapping must be audited from the point of dispensing to the patient. The MHRA has a well recognised duty to ensure medicines reach patients in a safe condition. The current anomaly appears to turn a blind eye to this step in the supply chain at the point the wholesaler releases goods to the pharmacy hub. Equally the training on delivering medicines safely and effectively direct to patients should apply fully to all hubs including DSPs. Why is it that DSPs are being treated differently to bricks and mortar pharmacies? It's essentially the same patients receiving the same medicines from the same wholesalers. A further regulatory disparity exists around how parcels must be "tracked and signed for" to be reasonably certain medicines are delivered into the hand of the intended recipient, as per existing regulations. Clearly an untracked, unsigned package cannot be guaranteed to finish in the hands of the intended recipient. There is a very real possibility that such omission could lead to community pharmacy closures which will, in turn, lead to unemployment and a reduction in the care services. At a time when integrated care systems have just gone live, the removal of vital support services leading to further inequalities is the wrong message for both providers and patients alike.

The UK's National Health Service (NHS) has partnered with P-Wave, a urinal products brand, to put cancer warnings for men on urinal mats in public places across England, as part of its new earlier diagnosis drive. Urinal mats in pubs, restaurants, shops, hotels and sports stadiums will begin to carry the message "Blood in your pee? Contact your GP practice", and men's toilets in workplaces will also be targeted, the NHS said. Health chiefs believe that this unique initiative will help men to spot the signs of cancer such as blood in pee, and catch the disease early. Following the UK government's new pay offer to NHS consultants, the Royal College of Nursing (RCN) wrote to the Health Secretary Victoria Atkins calling for fresh negotiations about nursing pay in England last week. However, the Department of Health and Social Care (DHSC) has rejected their demand, stating that there is no basis to reopen talks as the pay deal was accepted by the NHS Staff Council. In the previous pay deal, nurses were given a one-off payment between £1,655 and £3,789 for 2022/23, and a 5 per cent consolidated pay increase for the 2023/24 financial year. Nursingnotes quoted a DHSC spokesperson as saying: "We hugely value the hard work of NHS nurses and that is why we provided a 5 per cent pay rise. "We also provided two significant non-consolidated awards, which for nurses at the top of Band 5 was over £2,000, equivalent to an extra 6.1 per cent of their basic pay.
