The Pharmaceutical Services Negotiating Committee (PSNC) has developed a webtool to help community pharmacy contractors estimate the impact on their payments under the new discount deduction system. The calculator aims to illustrate the changes to discount deduction that a pharmacy could experience during the transition to the new arrangements from October 2022 to January 2024, when the new system will be fully in place. PSNC's calculator requires contractors to input information about their individual pharmacy dispensing mix (i.e. split of reimbursement by appliances, brands and generics) to provide an estimate for the level of impact the new discount deduction system will have. "The impact on individual pharmacies cannot be estimated without using dispensing mix data, meaning that any estimates which do not take this into account will not be reliable," said PSNC. For years many contractors have been frustrated by how discount is applied to their accounts and have often raised concerns about the blanket nature of its application to PSNC.

The Company Chemists Association (CCA)'s 'Prospectus for community pharmacy' has revealed that community pharmacies can release over 42 million appointments from general practice every year. In its prospect, the association calculated that community pharmacies could reduce hospital readmissions by 65,000 and administer an additional 10m routine vaccines annually. The prospectus sets out bold ideas and proposals regarding the future of community pharmacy. The association said: "Community pharmacies already work collaboratively with the NHS to ensure that patients can access care easily and safely. Whilst the sector has evolved considerably in recent years, the CCA proposes that pharmacies could do even more to directly tackle key problems for patients." CCA is concerned that without immediate action pharmacy closures will become increasingly common. "Fewer pharmacies will considerably diminish access to vital medicines and services, with the greatest impact on those in deprived communities."

Data crunched by an online pharmacy delivery company paints a concerning picture for the pharmacy sector in England. Gophr's 'Prescription For Pressure' initiative reveals a challenging time for the sector and is supported by separate analysis by the BBC showing that the number of pharmacies in England is at its lowest since 2010. In addition, new powers being introduced for pharmacists to prescribe common prescription drugs and perform routine tests create an even greater workload for already overstretched pharmacists. Gophr's data reveals that: * Each pharmacy in England serves an average of 6,078 people. * In 2022, pharmacists dispensed 1.043 billion prescriptions, 26.3 million more than 2021. * A single pharmacy in England dispensed 248 prescriptions a day on average in 2022. Based on the most up-to-date statistics from the Office for National Statistics, NHS England and Statista, Gophr's calculations show that pharmacists have around 116 seconds to dispense a prescription, which is less time than the 180 seconds it takes to make a Quarter Pounder at McDonald's.

The Pharmacists' Defence Association (PDA) has made the its 'Education revision programme' accessible for new registrations who are preparing for the GPhC/PSNI November 2022 assessment. The programme offers a wide range of practice calculations questions, some mock exams, practical tips on revision, and an opportunity to chat to other PDA members who are preparing to sit the assessment. NHS England has commissioned the Royal Pharmaceutical Society (RPS) to develop guidance that helps community and hospital pharmacy teams across Britain to reduce the impact of pharmacy services, pharmaceutical care and medicines on the environment. The RPS said the Greener Pharmacy Guidance will enable pharmacies to self-assess their impact against the standards, benchmark and improve through evidence-based activities and actions. "I'm delighted our strong commitment to helping pharmacy reduce its environmental impact can now be taken to the next level through developing guidance and accreditation for pharmacy teams," RPS president Professor Claire Anderson said. "Medicines account for 25 per cent of carbon emissions within the NHS and this initiative underscores our commitment to promoting sustainable healthcare and supporting the NHS's goal of achieving 'net zero' emissions by 2040." Peter Morgan, medicines assistant director at NHS England, commented: "Pharmacy staff are involved in the purchasing and dispensing of almost every medicine used in the NHS and the new Greener Pharmacy Guidance and Self-accreditation scheme will provide support for pharmacy professionals by outlining clear actions to deliver more environmentally sustainable pharmacy practices." The RPS said the guidance and digital self-assessment toolkit will integrate with carbon calculator tools to help pharmacy teams to measure their carbon footprint, action plan to reduce use of carbon and improve sustainability.

The Pharmaceutical Services Negotiating Committee (PSNC)'s director of pharmacy funding Mike Dent and Jack Cresswell, its funding strategy manager, will be hosting a webinar about changes to the discount deduction system on Wednesday 12th October at 7.30pm. The discount deduction scale has been a point of contention for community pharmacy contractors for many years, and PSNC has been pushing to remedy this. "The changes will lead to a levelling out for all pharmacies, with the impact on individual contractors varied due to their unique dispensing mix. We have been developing a series of resources about the changes, including an explanatory briefing, FAQs and an impact calculator. Our latest resource comes in the form of a webinar that aims to explain the new arrangements and the transition period in more detail," said PSNC.

Community pharmacy contractors eligible under the revised Pharmacy Access Scheme (PhAS) will receive their first of the new monthly payments with the January reconciliation payment due on April 1, 2022. Details of the revised version of the PhAS that commenced from January 1, was published in August 2021, with contractors receiving letters from the NHS Business Services Authority (NHSBSA) indicating their eligibility in the Autumn, PSNC said in an update. Payments under the scheme are dependent on registration on NHSBSA's Manage Your Service (MYS) portal to provide the Community Pharmacist Consultation Service(CPCS). Meanwhile, the review application window is now open for contractors, which would enable the NHS England and NHS Improvement (NHSE&I) to correct inaccuracies related to a pharmacy's distance criteria/calculation.

The National Pharmacy Association (NPA) has been in dialogue with the senior management of NHS Property Services to bring down the rent for community pharmacies based in NHS health centres. Association said: "Community pharmacies based in NHS health centres are struggling to make ends meet due to expensive rental rates." It is calling for pharmacies to have the ability to renegotiate rental rates part way through a contract. Gareth Jones, Director of Corporate Affairs at the NPA, said landlords need to take into account that pharmacy spending power is shrinking and the previous formula used to set rental rates is no longer fit for purpose. "It no longer makes sense to calculate based on historical assumptions about how many prescriptions the co-located GP will generate for the pharmacy," he said.

Community pharmacy workforce is in a fragile condition and faced a shortfall of 3,000 pharmacists in England over a period of five years, a recent research conducted by the Company Chemists' Association (CCA) has revealed. It noted that over the last five years, the net growth of community pharmacists in England a year stood at 733. This is much lower than the required average growth of 1,025, based on data reported by the Health Education England workforce survey from 2021. Acknowledging the discrepancies in the data calculation, the paper published by the association has used "the best available data" to highlight recent changes to the community pharmacy workforce in England. The report noted that despite an increase in the number of pharmacists, many contractors reported an increase in pharmacist vacancies. They also reported a significant increase in locum hourly pay rates, indicating demand exceeds supply.

In 2023, almost 14,000 people in England are estimated to have died due to excessive waits in emergency departments, averaging more than 260 deaths per week, according to new figures from the Royal College of Emergency Medicine (RCEM). The RCEM estimate is based on a large study of NHS patients published in the Emergency Medicine Journal in 2021, which found one excess death for every 72 patients who spent eight to 12 hours in an A&E department. Following a Freedom of Information audit of NHS trusts, the college has found that 65 per cent of people waiting 12 hours or more in A&E were patients waiting for a hospital bed. NHS data for England revealed that over 1.5 million patients waited 12 hours or more in major emergency departments last year. Based on the RCEM calculation, this indicates that over a million of those patients were waiting for a bed.
