Paramedicine expands to rural communities in B.C. - Infomart - 0 views

The services provided may include checking blood pressure, assisting with diabetic care, helping to identify fall hazards, medication assessment, post-injury or illness evaluation, and assisting with respiratory conditions.

They will also be able to teach skills such as CPR at community clinics.

"Even if there is not a lot of communication with words you can see the beautiful smiles on everyone's face. Many of the children come to know a lot of the seniors and when they pass by our window they'll be excited waving and saying 'Hi ,'" Yang said.
  • Youville occupational therapist Sheralyn Manning said the children's visits are a big part of the seniors'day. Besides planned events, such as doing crafts together, every so often the children will visit when the weather is bad and they are not able to play outdoors. Manning pointed out the friendship between Clark and Han has been particularly touching to watch and Clark has a recent craft project Han gave her prominently displayed in her room. When most people think of nursing homes the image that comes to mind is a stand-alone building offering residential care only for the aged.
  • It's a place seldom visited unless you are a family member, friend or volunteer. But these days more homes are trying to build bridges to the wider community. Of B. C.'s 460 government and private nursing homes, only a handful have daycares or doctor's offices on site, said Daniel Fontaine, CEO of the B. C. Care Providers Association, which represents 60 per cent of the privately-operated homes. But none are attached to a facility that offers a large variety of community services. One of the best Canadian examples of a nursing home that achieves just that, said Fontaine, is Niverville Heritage Centre, near Winnipeg. It is home to 116 seniors but is also a gathering place for major community events.
  • The centre hosts 100 weddings each year. As well, about 50,000 visitors drop in at the centre annually to access their doctor's office, dentist and pharmacist or visit the full-service restaurant and pub. "We found seniors don't want to be retired to a quiet part of the community and left to live out their lives. They want to live in an active community and retreat back to their suite when they want that peace and quiet ," said Niverville Heritage Centre's CEO Steven Neufeld.
  • Before the centre opened in 2007, he said, members from the non-profit board that operates the centre visited traditional nursing homes and discovered that the lounges that were built for seniors were seldom used. "I remember going to one place where there was a screened-in porch that was packed. The seniors were all there wanting to watch the soccer game of the school next door ," he said. Having services like doctors'offices, dentists, a daycare, a full-service restaurant, and hair styling shop on site fulfil the centre's mission of being an "inter-generational meeting place which fosters personal and community well-being." Fontaine said it's worth noting that Niverville was able to "pull all of this together in a community with a population of less than 5,000 people." He hopes more B. C. nursing homes follow Niverville's lead.
  • Elim Village in Surrey, which offers all levels of residential senior care on its 25-acre site, is on that track. There are 250 independent living units, 109 assisted living units and 193 traditional nursing home beds. The village also has a 500-seat auditorium, located in the centre of the village, that hosts weddings and is available for rent for other public events. Elim Village also rents out space in one of its 10 buildings to a school, which allows inter-generational programs to take place easily between students and seniors. Another "continuing care hub " at Menno Place, in Abbotsford, has a public restaurant called Fireside Cafe, popular with staff from nearby Abbotsford Regional Hospital and Cancer Centre. There's also a pharmacy and hairdresser on its 11-acre "campus " site but these services are available only to the 700 residents and staff. "We purposely try to involve the community as much as possible ," said Menno Place CEO Karen Baillie. "It's Niverville on a smaller scale." She said Menno Place partners with high schools and church groups and hundreds of volunteers visit regularly. "Seniors are often challenged with isolation and fight depression. That's why we have different programs to encourage them to socialize ," she said.
  • Research shows 44 per cent of seniors in residential care in Canada have been diagnosed with depression, and one in four seniors live with a mental health problem, such as depression or anxiety, whether they live in their own home or are in residential care. A 2014 report by the National Seniors Council found socially isolated seniors are at a higher risk for negative health behaviours including drinking, smoking, not eating well and being sedentary. The report also found social isolation is a predictor of mortality from coronary disease and stroke, and socially isolated seniors are four to five times more likely to be hospitalized.
  • Since more seniors now remain in their own homes longer those who move into care homes are often more frail and need a higher level of assistance, said Menno Place director of communications and marketing Sharon Simpson. Seniors with dementia, in particular, can be socially isolated as friends and family often find it more difficult to visit them as they decline, she said. But Simpson said an intergenerational dance program, run by ballet teacher Lee Kwidzinski, has been a wonderful opportunity for seniors with dementia to be connected to the community. The program is also offered in four other nursing homes in the Fraser Valley. "For them it's an opportunity to see children. You can see the seniors come to life, smiling and giggling at the girls'antics. It's very engaging ," she said. "Some may not be verbal but they are still able to connect. They feel their emotions and they know whether someone is good to them. They feel these girls and become vibrantly alive. It's one of the most powerful things I've ever seen."
  • Creating community connections is key as Providence Health begins its planning stage to replace some of its older nursing homes in Vancouver, said David Thompson, who is responsible for the Elder Care Program and Palliative Services. Providence Health operates five long-term-care homes for approximately 700 residents at four different sites in the city. "It's always been our vision to create a campus of care on the land ," said Thompson, of the six acres owned by Providence Health where Youville is located.
  • He said the plan is to build another facility nearby, with 320 traditional nursing home beds. One of the ways to partly fund the cost is to include facilities that could be rented out by the larger community, which would be a benefit to the seniors as well, he said. There is already child care on site, and future plans to help draw in the community include a restaurant, retail space and an art gallery. He said another idea is to partner with nearby Eric Hamber Secondary School by providing a music room for students to practise.

Eleven people attempted to take their lives in Attawapiskat two Saturdays ago, prompting the First Nation to declare a state of emergency—the fourth one since 2006. There has been more than 100 suicide attempts in the reserve since the month of September, many of which involved children. The community has been plagued by flooding and several housing crises in recent years.
  • Eighteen mental health workers were dispatched to Attawapiskat on Tuesday, including two counsellors, one crisis worker, two youth support workers, and one psychologist. While there is no set timeline, they’re not expected to leave for at least two weeks, said Health Canada assistant deputy minister Keith Conn during a teleconference last week.
  • Some of the people treated for mental health problems last week had previously been airlifted out of the community for assessment before being sent back after their examination, according to Mr. Conn. This past Tuesday, at least 13 people, including a nine-year-old child, had made plans to overdose on prescription pills as part of a suicide pact. The Nishnawbe-Aski Police Service apprehended them before sending them to the local hospital for a mental health assessment.

  • QUINTE WEST -Natalie Mehra was blunt with her assessment of the proposed cost-cutting measures facing Trenton Memorial Hospital. On a scale of one to ten, Mehra rated the severity of cuts at nine. "They are setting the ground work for the demise of the hospital. There will be no future in it," said the executive director of the Ontario Health Coalition. But the Coalition wants Quinte West and Brighton to keep fighting back, even harder than in previous years. "I am a bit worried because people get tired of fighting back. But our (the Coalition) message is that when you push back hard enough we can often win. Every community should be demanding long term stability when it comes to their hospitals. The bottom line is there should be a basket of good services available in every hospital," said Mehra.
  • The Coalition and Our TMH are planning a massive day of protest set for Friday, Nov. 13 at Trenton's Centennial Park beginning at 12- noon. Mehra said the protest will include the involvement from people from across eastern Ontario from Perth to Brockville and west to Quinte West and the Peterborough region. She also noted that hospitals in Ontario are chronically under-funded compared to other provinces. Mehra said hospitals in Welland, Fort Erie, Port Colborne, Niagara on the Lake and Niagara Falls are being gutted and face possible closure. Hospitals in those communities are part of the Niagara Health System.
  • "The first phase includes removing, diagnostics, surgical services and acute care beds, followed by replacing emergency rooms with urgent care centres," said Mehra. The final phase is closure. Mehra said amalgamated hospital systems have never worked. She used Quinte Health Care and the resulting yearly service cuts at TMH as a prime example.
  • "The current funding model has never worked. It means those hospitals face deficits every year. Virtually all hospital are under stress because the plan is to reduce the scope of services, resulting in the fact that residents will have to travel a lot further," said Mehra. The end result is that smaller hospitals inside large amalgamations are being "completely" gutted. But the local community, said Mehra, shouldn't give up hope.   • On Dec. 8, 2015, the Government of Canada announced its plan for a national inquiry into murdered and missing indigenous women and girls, in response to a specific call to action from the Truth and Reconciliation Commission.1 On Jan. 5, 2016, a pre-inquiry online survey was launched to “allow … [stakeholders an] opportunity to provide input into who should conduct the inquiry, … who should be heard as part of the inquiry process, and what issues should be considered.”2 We urge the federal government to be cognizant of the substantial knowledge, skill and advocacy of those who work in public health when deciding who should be consulted as part of this important inquiry.
  • A recent report from the Royal Canadian Mounted Police3 confirmed that rates of missing person reports and homicide are disproportionately higher among Aboriginal women and girls than in the non-Aboriginal female population. As rates of female homicide have declined in Canada overall, the rate among Aboriginal women remains unchanged from year to year. This is troubling, and the need to seek testimony from survivors, family members, loved ones of victims and law enforcement agencies in the inquiry is clear.

  • However, we should avoid diagnosing this problem merely as a failure of law enforcement. Murders represent the tip of an iceberg of problems related to endemic violence in communities. Many Aboriginal women and girls, and indeed men and boys, live each day under the threat of interpersonal violence and its myriad consequences.
  • Initial statements from the three federal ministers tasked with leading the forthcoming inquiry — the ministers of Indigenous and Northern Affairs, Justice and Status of Women — suggest that its purpose is to achieve justice, to renew trust between indigenous communities and the Canadian government and law enforcement bodies, and to start a process of healing.
  • The inquiry surely must also endeavour to lay the groundwork for a clear plan to address the broader problem of interpersonal violence, which, in turn, is rooted in several key determinants. Addressing interpersonal violence is not merely an issue of justice; it is also a public health concern.
  • Factors associated with both the experience and perpetration of interpersonal violence are manifold. They include but are not limited to mental health issues, drug and alcohol misuse, unemployment, social isolation, low income and a history of experiencing disrupted parenting and physical discipline as a child. The Truth and Reconciliation Commission’s report has highlighted that many of these factors are widespread in the Aboriginal populations of Canada.4 Many of the same factors contribute to disparities between Aboriginal and non-Aboriginal peoples in areas such as education, socioeconomic circumstances and justice.   • here is also substantial overlap with identified determinants of poor health in Aboriginal communities both in Canada and elsewhere.5,6 These are the factors associated with higher rates of youth suicide, adverse birth outcomes and tuberculosis, and poorer child health. It’s clear that a common web — woven of a legacy of colonization and cultural genocide, and a cumulative history of societal neglect, discrimination and injustice — underlies both endemic interpersonal violence and health disparities in Canada’s indigenous populations. There is no conversation to be had about one without a conversation about the other — if the aim is healing — because the root causes are the same.
  • The World Health Organization (WHO) is currently engaged in developing a global plan of action to strengthen the role of health systems in addressing interpersonal violence, particularly that involving women and girls.7 A draft report by the WHO acknowledges interpersonal violence as a strongly health-related issue that nevertheless requires a multisectoral response tailored to the specific context. Evidence from Aboriginal community models in Canada gives hope for healing.

  • The size of your wallet should not determine the quality of health care you receive. That was the message delivered to close to 1,000 protesters calling for the provincial government to free up more money for hospitals in Northern Ontario - particularly the North Bay Regional Health Centre.
  • "In North Bay, and across Northern Ontario, we are seeing the most severe cuts," said Linda Silas, president of the Canadian Federation of Nurses Unions. The rally drew supporters from across the province to protest cuts across the province. This year, the North Bay Regional Health Centre announced it is cutting almost 160 positions and closing more than 30 beds in an attempt to stave off a flood of red ink. "Here you are looking at 100 layoffs every year" if the province does not end a freeze on healthcare spending, Silas said.

  • Silas was one of a number of speakers who called on the government of Premier Kathleen Wynne to increase spending on health care in the province. North Bay, they said, is particularly hard hit because it is a P3 (public-private partnership) hospital - and because it brought three hospitals - two general and one psychiatric - under one roof. "It is time to raise the alarm," said Natalie Mehra, executive director of the Ontario Health Coalition.

Debbie McCrank from Kirkland Lake, the local co-ordinator for the Ontario Nurses Association, said the cuts are "going to impact all the North." She is responsible for the area from Kirkland Lake to North Bay, including Mattawa and West Nipissing.

  • n recent months, the Kanien'keha:ka (Mohawk) community of Kahnawake, Que., has re-opened discussion on its controversial 1984 membership law. The renewed debate has been accompanied by provocative developments: Protests outside homes, eviction notices sent to "foreign" residents, accusations of racism from the Minister of Aboriginal Affairs and a lawsuit challenging the membership law in court.
  • This fraught terrain has confused and outraged Canadians, partly because the story has lacked context and nuance in the media. While unique in many ways, Kahnawake is one First Nation among many grappling with these issues. After the failure of the 1969 White Paper on Indian policy and its assimilative prescription to eliminate Indian status and bands, the federal government has been pursuing a slightly amended policy of devolution: First Nations are asked to assume more administrative control of programs and services, from education and health care to housing and infrastructure, but with inadequate resources.

  • Corresponding to the devolution process have been revisions to the Indian Act as it relates to Indian status. From the "honorary white man" policy of the 1850s, Canadian legislation has intended to unmake Indians in the legal sense. Much of this strategy has focused on attacking women, removing their status if they marry a non-status Indian, which resulted in the disenrollment of tens of thousands of individuals who rightfully belonged within their nations. In post-Charter Canada, the Indian Act was successfully challenged and amendments in 1985 and 2011 partly ended the discrimination, precipitating a surge in the "official" First Nation population.
  • While much has been made of the "marry out, get out" provision at Kahnawake, the membership law, as well as many other First Nation membership laws, is more complex. The objective is to separate Indian status from band membership and take control of the latter from the federal government. "Membership" in this sense includes residency qualifications, electoral rules and treaty rights, eligibility guidelines on business operations and even burial plot designation. In reserve politics, or "reserve nationalism" as Audra Simpson calls it, this might be described as citizenship and even immigration policy. In a general sense, it is about who belongs.
  • There are two extremes in this conversation. The "exclusive" membership perspective views lineage as the crucial qualification to belonging. Citizens must be descended from indigenous peoples, inculcated in indigenous culture. They exclude those with weak lineage or none at all, especially if taking up land or resources. Of course, communities today have increasing numbers of both groups. About three-dozen of those who share this conservative approach to membership at Kahnawake are responsible for taking the initiative to evict non-indigenous people from the community.
  • The "inclusive" perspective advocates for a more open community not necessarily delimited by strict ancestral connection but inter-community relationships. It is the far more common practice among indigenous peoples (extending the rafters of the longhouse, the ever-expanding circle, etc.). Non-indigenous people who can make a contribution to the community while reflecting indigenous values should be welcome. In the case of Kahnawake, a handful of these inclusive proponents are appealing to the Quebec Superior Court to protect this view. In many ways, the debate revolves around claims of authenticity: a contentious notion after 150 years of Indian policies that have cultivated artificial governments and islands of reserved lands, imposed patriarchy and domesticated sovereignty.
  • Indeed, indigenous peoples would be justified in evicting white people from the little land we have left if that were the case. But this is a crude simplification.
  • Despite the opinions of pundits or politicians on the allegedly racist law at Kahnawake (or elsewhere), this is fundamentally about people passionately and earnestly working towards visions of community well-being amid very real, long-standing and external constraints.   • From fears of further privatization to first-hand hospital horror stories, an abundance of beefs concerning Sault Ste. Marie - and Ontario - health-care services was aired Thursday evening during a town hall meeting hosted by Sault and Area Health Coalition. "We can't put up with this healthcare system," Sault coalition president Margo Dale told about 75 at the Royal Canadian Legion, Branch 25. Dale said she is "sick of the rhetoric" coming from the Ontario Liberals in their explanations for cutting front-line staff and services. Her sentiments were echoed by a number of other speakers, including Natalie Mehra, Ontario Health Coalition executive director, who decried what she contends is a profound dearth of dollars being divvied out to Ontario hospitals. On top of four years of freezes to base funding, there's been nine full years in which support has not kept up to inflation.
  • "The gap gets bigger and bigger and bigger," Mehra said. "The hospital cuts have been very deep, indeed, and another year of inadequate funding for hospitals is going to mean more problems for patients, accessing care and services." In an earlier interview Thursday with The Sault Star, Mehra said Ontario, "by every reasonable measure," underfunds its hospitals and has cut services more than any other "comparable jurisdiction." "The evidence is overwhelming," she said. "It's irrefutable that the cuts have gone too far and are causing harm.   • The outpatient lab at the North Bay Regional Health Centre is under the microscope as the facility tries to staunch a flood of red ink. We are reviewing a number of different parts of the hospital operation," Dave Smits, vice-president of corporate, said Tuesday. Smits said the health centre is looking at about 40" different initiatives, including the outpatient lab. The health centre announced last week it is cutting 158 full-time equivalent jobs with another 50 possibly to be cut if the province doesn't help with one-time funding for early retirement packages. The hospital also will close 30 beds between the medical and mental health sides of the facility. The hospital has laid off 196 staff over the past three years.
  • Smits said the outpatient clinic in North Bay operates different from most communities in the province, because there is no private laboratory in the city to handle some of the tests. We are looking at it," Smits said. We've been looking at it off and on over the years. We've talked about it and we continue to go back to it. The question is, if it is different here, is it different good or different bad? Are there options in the community? Will others be able to provide the services in the community?" Smits said the lab at the hospital is heavily utilized and provides a high degree of service." He said any physician in the community can order any type of laboratory test at the hospital's outpatient lab, while other communities across the province have private labs that can pick up the majority of tests.

That means the tests or samples have to be sent out of the city, which can result in sometimes substantial delays." There isn't a patient who visits the hospital, he said, who does not have at least some contact with the lab, including testing at various clinics offered at the hospital. He expects it should be completed in the next month or so, and we will have a decision on what exactly might change in the next four to six weeks."   • Contracting Out Hospital Work to Private Clinics – Backgrounder For years CUPE has been concerned the Ontario government would transfer public hospital surgeries and diagnostic tests to private clinics. CUPE began campaigning in earnest against this possibility some years ago with a tour of the province by British Health Secretary Frank Dobson who talked about the disastrous British experience with private surgical clinics.Unfortunately, the provincial Liberal government has now moved in this direction. The door opened a few years ago with the introduction of fee for service hospital funding (sometimes called Activities Based Funding). Then in the fall of 2013 the government announced regulatory changes to facilitate this privatization, with the government finally announcing Request for Proposals for the summer of 2014.
  • Hospitals are the main focus of the government’s health care cuts. They do not see community hospitals as providing a broad range of services to the local ... [Read More]population, but instead wish to remove an untold range of services from local hospitals and transfer them to specialized private clinics. The proposal would remove the most lucrative, high volume and easiest procedures from community hospitals. The remaining community hospitals would be left with the most difficult services. If they chose to compete with the private clinics, they would have to specialize in a narrow range of services. The government’s plan is the opposite of one-stop, integrated public health care. This proposed privatization of surgeries and diagnostic tests is in addition to the aggressive attempts to remove non-acute care services from hospitals (e.g. outpatient clinics, complex continuing care, rehabilitation, long-term care, primary care, etc.). As acute care currently accounts for only about 1/3 of current hospital funding, these attacks are a grave threat to the viability of community hospitals, and in fact we are now seeing a wave of hospital shut-downs that is somewhat reminiscent of the Mike Harris era. Despite the government’s rhetoric about keeping care non-profit, services that are being cut from local hospitals now are being privatized to for-profit owned corporations. Even if the private clinics did start out as non-profit (which has not been the case so far) the whole system of private clinics could be privatized with a stroke of a pen.
  • Ontario Health Care Privatization: The push for health care privatization in Ontario picked up in 2001 when Ontario Health Minister Tony Clement announced two privatized P3 hospital projects, the Royal Ottawa and the Brampton Civic (part of William Osler Health Centre).   • MRI and CT Clinics: The PC government also tried to set up private MRI and CT clinics outside of hospitals. Community/labour campaigns however were able to stop this. A key factor was that, to increase their revenue, the private clinics were allowed to bill private patients for a certain number of hours each week (with the rest of the week dedicated to patients paid for by the public system). As the public insurance system must pay for all ‘medically necessary’ hospital services, the government was left to try to explain why any reputable clinic would allow patients to subject themselves to such tests for medically unnecessary reasons. Since this episode, private clinics have been in the news – but mostly for the wrong reasons. Private surgical and diagnostic clinics: Initially, the government let the emerging industry slip entirely free of public reporting and oversight. However, after the September 2007 death of Krista Stryland, a young mother who underwent liposuction at a Toronto cosmetic clinic, the government required the industry to face some modest oversight in 2010. Unfortunately this was not by a public authority, but through self-regulation by the doctors (even though the doctors themselves had lobbied to expand this private industry).
  • Then in the fall of 2011, following disclosure that 6,800 patients would have to be notified that faulty infection control procedures at a private clinic could have exposed them to HIV or hepatitis, the then Health Minister, Deb Matthews, declined to introduce oversight by a public authority, despite public pressure. Instead she comments, “Government can’t do everything. A professional (regulating body) like the College of Physicians and Surgeons, they take responsibility for their members....At this point I am delighted the College is taking that responsibility seriously and has found a problem that we need to fix.” Eventually the College of Physicians and Surgeons released a report on the private clinics that mentions that some 29% of the private clinics fall short in some way – but the College would not indicate which ones – or how they fell short. This caused public uproar, with the Toronto Star playing a leading role (as it would continue to do). Again, the government promised improvements. In the last two months however, the Star has followed up and revealed (after our urging) that the public reports from the College of Physicians and Surgeons fall far short. They also ran a series of often front page stories on serious quality problems at private clinics.

These have been achieved through new investments such as pay for performance incentives and policy change. They have had some important successes, but the work is incomplete.
  • Examples of the ongoing challenges that we face include substantial proportions of the population who do not have easy access to a family doctor when needed, even if they have a family doctor; little progress on improving rates of eligible patients receiving important preventive care measures such as pap smears and mammograms; continued high utilization of emergency departments and walk-in clinics compared to other countries; long waits, which remain a problem for many types of care.   • The physician does not work for you. The physician does not work for the health system. The physician is a private practitioner who bills directly to the health care system. He does not work for the CEO of the hospital or for the local health region. Therefore, your control and the levers you have with that individual are limited.
