A total of 68 percent of those questioned said the current Congress was the worst in their lifetime, with only 28 percent disagreeing. The negative outlook toward Congress "exists among all demographic and political subgroups. Men, women, rich, poor, young, old -- all think this year's Congress has been the worst they can remember," CNN polling director Keating Holland said. "Older Americans -- who have lived through more congresses -- hold more negative views of the 113th Congress than younger Americans. Unless the Obama administration can persuade Afghan President Hamid Karzai to sign a security pact that would permit a modest U.S. force to remain beyond 2014, the United States is almost certain to drastically scale back aid to Afghanistan. That would force aid groups to work under more precarious security conditions and compete for scantier aid dollars. It would be "a complete catastrophe" to pull the entire U.S. force from Afghanistan next year, said Andrew Wilder, who directs Afghanistan and Pakistan programs at the U.S. Institute of Peace and spent years working in the region. A deterioration in security conditions would hamper oversight of aid projects, possibly making a deeply skeptical Congress even more reluctant to fund Afghan aid. A smaller staff at the U.S. embassy in Kabul would also make it more difficult to sustain many U.S. aid programs. "My judgment is no troops, no aid, or almost no aid," James Dobbins, the U.S. envoy for Afghanistan and Pakistan, told Congress this month.     NEW YORK (Reuters Health) - One of the most common types of knee surgery performed in the U.S. is no more effective than fake surgery, at least for the first year, according to a new study. The new evidence should give doctors pause before they try to repair the meniscus, which cushions the bones of the knee, according to the Finnish doctors behind the research published in the New England Journal of Medicine. The experiment involved 146 volunteers whose knee pain appeared to be caused by wear and tear of that cushion. None of the participants had a recognized injury or osteoarthritis, both situations for which the surgery is already known to be ineffective. After 12 months, the average improvement among the people who received real surgery and those who got "sham" surgery was essentially the same, said the research team, led by Dr. Teppo Jarvinen of University of Helsinki. There was no significant improvement in knee pain after exercise and no sizable improvement in the likelihood that a patient would require subsequent knee surgery. But Dr. Craig Bennett, chief of sports medicine at the University of Maryland Medical Center, cautioned that the findings should not be over-generalized. One problem, he told Reuters Health in a telephone interview, is that such "sham" surgery is, in fact, a surgical procedure with potential benefit. People with knee pain who seem to be candidates for meniscal repair may be suffering because of debris in a swollen knee joint. "If you scope the knee (without touching the cushion), that will often help even if you don't completely address the torn meniscus issue," he said.     ISLAMABAD (Reuters) - A U.S. development worker kidnapped in Pakistan by al Qaeda two years ago appealed to President Barack Obama to intervene and help negotiate his release, in a video released on Thursday. In a 13-minute clip issued by al Qaeda's media wing and posted on several news websites, Warren Weinstein, 72, looked gaunt and tired. "I am not in good health. I have a heart condition. I suffer from acute asthma... Needless to say I've been suffering deep anxiety every part of every day," he said. "Mr President, for the majority of my adult life, for over 30 years I've served my country ... Now when I need my government it seems that I have been totally abandoned and forgotten." The video could not be independently verified. Weinstein was abducted in 2011 in the eastern city of Lahore where he worked for a U.S. consulting company.
