    This article discusses the paradigm shift in health care towards evidence based and client centered practice to increase patient outcomes by linking payment to performance. In this type of system it is important to define OTs distinct value so that it does not get pushed out. There needs to be an increase in evidence that shows the efficacy of OT interventions. Overall, there needs to be the development of quality measures so that OTs can evaluate and document their contributions to patient outcomes.

    Home modifications aim to enhance safety and occupational performance in the home. However, given the complexity and unique meaning of the home, they can impact aspects other than function. This qualitative descriptive study aimed to explore the impact of home modifications on clients and their family's experience of home. The study found that home modifications impacted positively and negatively on five dimensions of the home environment, the personal, occupational, physical, temporal and social dimension. The outcomes of the modifications and the home modification process were influenced by three themes, workmanship, consultation or involvement in decision-making, and the societal dimension of the home environment. Conclusions: The negative outcomes and poor consultation experiences suggest the need for occupational therapists to understand their client's personal experience of home and to comprehensively follow-up and evaluate these following home modifications.

    This PDF document serves as an instructor's resource to clinical reasoning, why it is important, as well as the process to implement clinical reasoning into practice. The resource was written for nurses but can apply to occupational therapists as well. According to the authors, clinical reasoning describes the process by which occupational therapy practitioners collect cues, process information, come to an understanding concerning a patient's problem/situation, develop a plan and implement interventions, evaluate the outcomes, and lastly reflect and learn from the experience. Within healthcare, clinical reasoning is vital because those with effective clinical reasoning skills are better able to detect impending deterioration in patients and are also able to pinpoint what is impacting their ability to function at their prior level. This two skills enable therapists to provide more effective treatment and often they receive better functional outcomes from their patients. The clinical reasoning cycle outlined in this resource begins by considering the patient and their situation. The occupational therapist must then collect cues/information from the patient, their family, and the environment itself. The therapist must then process the information, identify the problems, establish therapy goals, take action through treatment intervention, and evaluate treatment outcomes. The final step includes reflecting on the process and learning from any mistakes or misconceptions in order to grow and mold into a more experienced clinician.

    This purpose of this study was to discuss the different measurement tools available for evaluating the upper limb of persons with tetrapelgia and if they are accurate. Findings show that the grasp and release test is recommended for clinical trials involving persons in the chronic stages of SCI. While muscle strength testing and electrodiagnostics can be useful measures in acute clinical trials and outcomes studies, standardized measures of hand function are difficulty because they require a sitting balance and endurance tat many people with acute tetraplegia simply do not have. Overall, the study found that there is still a lot of work that needs to be done to determine the best assessment tool for upper limb function in SCI.

    This is a great example of a future study that will consider the psychosocial aspect of individuals after a musculoskeletal injury and subsequent occupational therapy intervention. The study focuses on hand injuries and this type of injury is extremely relevant to my current clinical setting which is an outpatient hand clinic. The studies primary outcome measure is return to work vs no return to work. The secondary outcome measures focus on psychosocial questioners, work environment, demographics, and motor and sensory testing. This is a great idea for a study that would take into account the psychosocial aspect of the individuals with hand injuries and see if there is any correlation with returning to work.
