How Do We Change Practice When We Have the Evidence? | American Journal of Occupational... - 0 views

  •
This article discusses evidence based practice and how even with evidence it can be difficulty to incorporate new changes into practice. There are several steps that need to be taken in order to properly incorporate new evidence into practice, such as determining relevance to setting, creating new guidelines, and adopting these new guidelines into practice. Due to several barriers, these changes typically do not take place immediately. Some barriers to adopting new research findings into practice include: profession, legislative, administrative, practitioner, patient/patient family, and value/belief barriers.

This PDF document was part of a training course provided by Kansas Association of Homes and Services for the Aging. This particular document outlines various psychosocial needs of the elderly, and various strategies health care professionals can utilize to help provide more person-centered care. Psychosocial needs/issues can arise as a result of the many changes that can occur during older adulthood. This article outlines seven changes that frequently occur in older adulthood that can present potential issues when individuals do not adjust to the changes. For instance, family changes occur as parents who used to care for their children are now being cared for by their children. Individuals may face retirement, and no longer have a sense of who they are without a job title. Older adults may experience an awareness of their mortality as loved ones die and their health declines. Individuals may lose their spouse and companion. Fear of loss of independence may set in as persons experience a decline in health and their physical abilities. Poor adjustment to changes in income after retirement can trigger psychosocial issues. Finally, loneliness may occur as children become busy, older adults do not drive, and spouses and friends die. All of these various factors can create psychosocial needs within a person's life. This article does a great job at not only defining why psychosocial issues may be present within this population, but also provides strategies to help older adults find joy in their lives again and help address their psychosocial needs. For instance, the article focuses on building on lifelong interests and offering new activities that an individual can engage in for pleasure.

This article can help those who work with elderly individuals to identify common psychosocial needs among this population, in hopes of providing better care.

Occupational therapists' experiences of improvement work: a journey towards sustainable... - 0 views

  •
This article aims to use a qualitative descriptive study to describe occupational therapists experiences and improvements in work after adopting the Occupational Therapy Intervention Process Model (OTIPM). The OTIPM is a client-centered, top down, occupation-focused OT model of practice. The assumptions of the model are that every person is unique, has the will to engage in meaningful occupations, occupations are the primary therapeutic means and engagement in occupations in the central focus in OT. The study was conducted in an acute care hospital in Sweden starting in 2001. They were selected because the therapists wanted to improve OT services and promote evidence-practice in their hospital. The participants participated in a total of six semi-structured focus interviews in which the occupational therapists were asked to describe their experiences of the long-term improvements in work, attitudes toward improvements work and thoughts on how this improved work impacts the goal to change to a more sustainable and evidence-based OT practice in their setting. The first three interviews were in 2006 and the last three were in 2011. The interviews were analyzed and broken down into common themes among therapists. The first theme is that the therapists found their thoughts and actions were transformed on an individual and group level after adopting the OTIPM. The second theme discovered how the therapists dealt with conflicting feelings and attitudes toward change. The final theme describes a shared professional culture. In conclusion, the investigators discovered from the 10-year study and testimonies of Swedish therapists that the use of OTIPM can lead to the integration of evidence-based knowledge that has long-lasting achievements in practice.

In her Slagle Lecture, Dr. Yerxa discussed the changes that were occurring in the occupational therapy profession during the 60's to become a more professional and respected profession. Dr. Yerxa begins by breaking down the steps the profession has taken to become more professional such as: performing more organized, objectified, and accepted research; implementing workshops, seminars, and study courses outside of the annual conferences; the focus on client-centered treatment; the increased responsibility to contribute to health care fields pool of knowledge and communicate with other professionals. From these changes that occurred, occupational therapy became a more respected and recommended health care profession. This document is important because the aspects discussed throughout the article are important for us to implement into our professional careers to not only advocate for ourselves but for the occupational therapy profession as a whole.

Occupational therapists perspectives of factors influencing chronic pain management. - 0 views

Using interviews, researchers looked at how OT's viewed their clients and how they managed their pain. Both positive and negative themes emerged including: losing valued roles, having depression and receiving compensation and to knowing: being fearful, looking for a cure or not knowing what to expect, being unable to set goals and being unable to use strategies. OT's reported that client's acceptance and desire to change played a large role in management.

Evidenced-based practice - 1 views

This article focused on determining the trend of practicing Occupational therapists that utilize research to form their evidence-based practice. The article mentions how this is a vital component to a medically-based profession. To examine this question a two page survey was sent out across the country to occupational therapists across various clinical settings. The results demonstrated that the majority of practitioners have access to the relevant information required for evidenced-based practice, whether through online journals or seminars. Also, the data illustrated the higher rates of accessing information correlated positively with higher academic degrees attained. Barriers found to the implementation of such practice included cost of attending such courses as well as lack of convenience accessing online databases. Ultimately, it seems that lack of time appeared to be the greatest barrier to pursuing and attaining such information. Overall, I feel the results demonstrate the change in culture that is occurring within the profession. However, if this change is to become permanent we as the future of the profession must continue to push this ideal forward and assist the profession into the coming years.

The article begins by discussing the need for the application of ethical principles into everyday practice as the healthcare market is constantly changing. This certainly is prevalent to us as fieldwork students as Obama-Care is currently being implemented and may pose major changes to the methods of health care service delivery.
