  G-Codes are very prevalent within therapy practice. Often times it becomes easier when therapists are provided with a 'cheat sheet' in a sense. This following sheet provides how G-codes can be used in functional limitation reporting in regards to the percent impaired, limited or restricted with using severity/complexity modifiers. This sheet also provides examples on how to report these scores from current, goal, and discharge.
