This initiative responds to the increasing demand for guidance on safely reducing or discontinuing psychiatric medications, providing comprehensive support for clinicians and patients alike. Drawing on insights from contributors to The Lancet Psychiatry, the handbook offers step-by-step instructions for tapering antidepressants, benzodiazepines, gabapentinoids, and z-drugs, along with troubleshooting strategies.

"Our main concerns are that the use of what they call an asynchronous supply, where the prescriber neither sees nor talks to the patient at the time they issue the prescription, does lay itself open to perhaps people getting hold of medication that they shouldn't," the Head of Policy, PDA said. The Pharmacists' Defence Association (PDA) engages in a critical conversation on BBC Radio 4's Woman's Hour, shedding light on the online prescribing of weight loss drugs and advocating for safer practices. The PDA's spokesperson, Head of Policy Alima Batchelor, emphasised the importance of adhering to professional guidelines to ensure patient safety and mitigate regulatory concerns. ""There are guidelines and advice about verification within the DiCE guidance, but it doesn't go quite far enough for us. The Pharmacist's Defence Association (PDA) has raised concerns over the adoption of Digital Clinical Excellence (DiCE) guidelines for online prescribing of GLP-1 receptor agonists for weight management in adults. This follows a clash with expert recommendations commissioned by the General Pharmaceutical Council (GPhC) as the PDA "believes the final guidance conflicts with the expert opinion underpins the GPhC's prosecution strategy towards pharmacist prescribers under investigation." In a recent statement, the PDA highlighted the discrepancy between DiCE's guidance and the expert report employed by the GPhC to prosecute pharmacist prescribers using a questionnaire-based consultation model. This conflict has prompted the PDA to withhold its endorsement of the DiCE guidance as it differs from an expert report commissioned by the GPhC. The PDA's reservations stem from the inherent risks associated with questionnaire-based prescribing.

Recent NHS data sheds light on the evolving landscape of fit note certification, indicating a gradual expansion of roles beyond traditional medical practitioners to pharmacists and nurses. This uptake in numbers coincides with the Department of Work and Pensions's (DWP) call for evidence in relation to fit note reform which will remain open till 8 July 2024. According to figures published by NHS Digital, during the nine-month period from April 2023 to December 2023, pharmacists certified a significant number of fit notes. Approximately 1.4 per cent of fit notes of the 8 million were issued by pharmacists while the rest were issued by the GP surgeries reflecting the impact following legislative amendments in July 2022, which extended certification authority to pharmacists, nurses, occupational therapists, and physiotherapists. In June 2022, the Pharmacists' Defence Association (PDA) advised pharmacists not to sign fit notes where the required therapeutic, diagnostic, or examination skills lie outside their scope of competence. The association remarked that healthcare professionals needed mandatory training under the Health Education England before they are fit to issue fitness notes to the public.

The Company Chemists' Association (CCA) has urged NHS England to expand the Pharmacy First service by increasing treatment options, adding more conditions, and integrating Independent Prescribing into the service. This would further promote pharmacists as antimicrobial stewards while enhancing patient access to urgent care, it said. Managing patient demand for antibiotics remains a challenge for healthcare professionals, including pharmacists. However, a new report by CCA confirms that pharmacists are acting as responsible providers of antibiotics, in line with Antimicrobial Resistance (AMR) guidelines. The service allows pharmacies to provide advice and treatment, including prescription-only medication, for seven common conditions: earache, uncomplicated urinary tract infections (UTIs) in women, sore throat, sinusitis, infected insect bites, impetigo, and shingles. As the assisted dying bill moves through Parliament, pharmacy professionals are encouraged to take part in the public debate. The UK chief pharmaceutical officers (CPhOs) - Andrew Evans (Wales), Alison Strath (Scotland), Cathy Harrison (Northern Ireland), David Webb (England) - have issued advice for pharmacists and pharmacy technicians wishing to give their opinions on this important topic. "In our view it is entirely reasonable for any pharmacists and pharmacy technicians to give their opinions, and that the public would expect that," they said in a statement released along with Elizabeth Fidler, senior professional advisor for pharmacy technician practice at NHS England, and Richard Cattell, deputy chief pharmaceutical officer for England. However, pharmacy professionals are advised to avoid identifying individuals they have cared for unless with their consent and directly engaging patients in the debate. The guideline also emphasised that pharmacists and pharmacy technicians should express their personal views, and not speak for the professions. The CPhOs noted that the final decision on assisted dying must be made by society as a whole, through the parliamentary.

In a surprising move that has divided the pharmacy profession, the General Pharmaceutical Council (GPhC) has sanctioned hundreds of pharmacies to allow patients to self-select certain Pharmacy (P) medicines, a decision met with strong opposition from the Royal Pharmaceutical Society (RPS) as well as from the Pharmacy Defense Association (PDA). In a statement, RPS shared that at a board meeting held on 19th June, they expressed "disappointment" over what they described as a lack of prior consultation and comprehensive communication from the GPhC regarding the implications of this policy shift. They argue that allowing patients to self-select certain P medicines could potentially jeopardize established professional guidelines and compromise the role of pharmacists in ensuring safe medication use. "Enabled by the General Pharmaceutical Council, we understand that hundreds of pharmacies, from large multiples to small independents, have been approved by the regulator to enable patients to self-select certain P medicines," the statement said.

Following the publication of a new study by the British Medical Journal (BMJ) that estimates the economic impact and prevelance of hazardous NSAID prescribing in England, the Royal Pharmaceutical Society (RPS) has underscored the pressing need for enhanced safety measures in medication management. The study, led by RPS Fellow Rachel Elliott, reveals significant health and financial costs associated with the high-risk prescribing of oral non-steroidal anti-inflammatory drugs (NSAIDs) among vulnerable populations. James Davies, Director for England at the RPS, emphasised the critical findings of the research. "Improving the safety of NSAID prescribing requires a multi-pronged approach, that educates and supports prescribers on the risk, as well as uses the skills of pharmacists in the primary care team to help identify higher risk patients and prescribing scenarios," Davies remarked. The study highlights that NSAIDs, while effective in managing pain and inflammation, are associated with serious adverse events, including gastrointestinal bleeding, renal dysfunction, and cardiovascular issues.

The National Institute for Health and Care Excellence (NICE) has made a new recommendation in its final guidance this week that could benefit almost 30,000 adults suffering from moderate to severe symptoms of uterine fibroids. The Department of Health and Social Care's executive non-departmental public body, NICE, has recommended linzagolix, also known as Yselty by Theramex to treat uterine fibroids. Linzagolix is recommended for use as a longer-term treatment (more than 6 months) and is not for people who need short-term treatment, for example, before planned uterine fibroid surgery. According to Jonathan Benger, NICE's Chief Medical Officer and Deputy Chief Executive, expressed his approval of the new recommendation, stating, " What makes Linzagolix particularly noteworthy is its flexibility; it can be taken either with or without hormone therapy. "This means that clinicians can now offer a more personalised approach to care, tailoring treatment to the specific needs and circumstances of each patient. "For many women, this will represent a significant improvement in managing this often challenging and distressing condition."

Victoza (liraglutide) 6mg/mL pre-filled injection pens, previously used in the treatment of type 2 diabetes mellitus (T2DM), have been out of stock in the UK since August 2023. Novo Nordisk has confirmed to The Pharmaceutical Journal that the product has now been discontinued as part of their broader strategy to consolidate their portfolio. The Danish pharmaceutical company said that they are in close dialogue with the Department of Health and Social Care (DHSC), which is expected to release guidance outlining timelines, alternative treatments, and recommended next steps for healthcare professionals. However, the company clarified that they have no plans to discontinue Saxenda (liraglutide) pre-filled injection pens, though this product has also faced supply restrictions in the UK since shortages began. The DHSC initially announced a national shortage of glucagon-like peptide-1 receptor agonists (GLP-1 RAs), including Victoza, in June 2023. At that time, prescribers were advised to limit use of these drugs for their licensed indications and to avoid initiating new GLP-1 RA therapies for T2DM patients during the shortage.

It emphasises that pharmacy teams providing such services need to adhere to the standard process of clinical assessment and care provision they have been trained to take as healthcare professionals. Professor Kamila Hawthorne, chair of the Royal College of GPs, has cautioned that while weight loss drugs hold significant potential to enhance many patients' lives, they are not without risks. She also expressed concern that expanding the roll out of tirzepatide at the scale proposed by the National Institute for Health and Care Excellence (NICE) would have "significant practical and resource implications for the NHS and primary care." The final draft guidance from NICE, published on 5 December, suggests that around 220,000 people living with obesity will be offered the weekly injection, along with a reduced-calorie diet and increased physical activity, over the next three years. Marketed under the brand name Mounjaro by Eli Lilly, tirzepatide is recommended for people with a body mass index (BMI) of more than 35 and at least one weight-related illness. Accepting the request from NHS England to roll out the medication over 12 years, the NICE guidance proposes to prioritise those with the highest clinical need.
