WEDNESDAY, April 22, 2015 (HealthDay News) -- In a step toward personalized vaccines against cancer, scientists report they have developed an immune-system therapy that knocks out several types of aggressive tumors in mice. German researchers said the findings, reported April 22 in the journal Nature, could lead to a "blueprint" for developing tailored vaccines for a range of cancers. Such vaccines would be designed for individual patients, based on the specific genetic mutations in their tumors.

Artificially boosting the body's immune response against cancer is the most exciting advance in the treatment of tumors in the past couple of years. But as the jam-packed sessions at a recent scientific conference in New York City made clear, a lot of questions remain to be answered before anyone can declare victory in the war on cancer. Among them: What is the best way to kick the immune system into action? Will immunotherapy work for all sorts of people with all kinds of cancer or just for a lucky few? Is there a way to make the treatments less dangerous or expensive?

This directed case study follows the story of "Pauline," a 20-year-old college student who has just received results from a personal genetic testing kit she purchased online. The report shows a negative result for variants of the BRCA 1 and 2 genes, which are associated with a greater risk for breast cancer. Although Pauline has a family history of breast cancer, she concludes that she no longer needs to be concerned, or does she? As students work through the questions in this case study, they review the role of genes and how they code for proteins as well as the effects of proteins on health, especially on cellular growth regulation and cancer. They also learn about the process of genetic testing and consider the ramifications of positive and negative tests for diseases or health conditions, especially with respect to breast cancer. The case is designed for non-science majors in a scientific methods course and could also be used in an introductory biology course. The questions in the case could be adapted for an upper level genetics class.

Vanderbilt-Ingram Cancer Center has become the first cancer center in the Southeast and one of the first in the nation to offer adult cancer patients routine "genotyping" of their tumors at the DNA level.

In Milan, two German Shepherds trained for this particular task have certainly proven themselves accurate. The dogs were trained to detect specific chemicals in urine samples. In it, Dr. Charles Sawyers explains the difference between proto-oncogenes, oncogenes, and tumor suppressor genes, and how mutations in these genes drive cancer development. "Cancer genes" can affect several cellular processes that he groups into three categories: cell growth and survival (i.e., genes involved in the cell cycle, cell fate (i.e., genes involved in cell differentiation), and genome maintenance (i.e, genes involved in DNA repair.)

Reporting on a small preliminary study, Johns Hopkins researchers say a simple blood test based on detection of tiny epigenetic alterations may reveal the earliest signs of pancreatic cancer, a disease that is nearly always fatal because it isn't usually discovered until it has spread to other parts of the body.

Common tests used in cancer diagnostics

One of these days, there could well be a simple blood test that can help diagnose and track cancers. We aren't there yet, but a burst of research in this area shows we are getting a lot closer.

At the annual meeting of the American Association for Cancer Research in Washington DC this week, Wolchok and other researchers will report on their search for immunotherapy markers - ways to predict a patient's response to an immunotherapy or to show whether a given treatment is working. The work is hampered by the complexity of the immune system, but early results are converging on one point: that patients' own immune responses to cancer are crucial in determining outcomes.
