To achieve these objectives, the NHLBI Career Transition Award will support two phases of research: a mentored intramural phase (up to two years) and an extramural phase (three years), for a total of five years of combined support. Transition from the intramural phase of support to the extramural phase is not automatic. Approval of the transition will be based on the success of the awardee's research program during the mentored phase as determined by an NHLBI progress review, which will include an evaluation of a research plan to be carried out at the extramural institution.

    The objectives of this research are to 1. Define and quantify the health and safety issues facing public transit workers. These include, but are not limited to, behaviors that affect health and safety (sleep, exercise, diet, smoking, seat belt use, alcohol use), chronic diseases (diabetes, cardio-vascular disease, hearing loss), communicable diseases, labor environment, organizational issues (safety climate, organizational programs, policies and procedures, support at work), organization of work (shiftwork, job discretion, job demands), and access to health-promoting activities and healthcare; 2. Using existing information resources, estimate the costs associated with employee health issues; and characterize to the degree possible how these costs are distributed to individuals, transit agencies, and society; 3. Background Transitions of care are the movements of patients between providers or clinical settings which typically occur when primary care providers refer patients to specialty care, or when patients are discharged from the hospital to subsequent care settings.     The Substance Abuse and Mental Health Services Administration (SAMHSA), Center for Substance Abuse Treatment (CSAT) is accepting applications for fiscal year (FY) 2015 Cooperative Agreements for State Adolescent and Transitional Aged Youth Treatment Enhancement and Dissemination Planning [State Youth Treatment - Planning (SYT-P)]. The purpose of SYT-P is to provide funding to states/territories/tribes (hereafter known as "states") to develop a comprehensive strategic plan in order to improve treatment for adolescents (ages 12-18) and/or transitional aged youth (ages 16-25) with substance use disorders (SUD) and/or co-occurring substance use and mental disorders (hereafter known as "the population of focus"). The plan will help to assure that youth have access to evidence-based assessments and treatment models and recovery services by strengthening the existing infrastructure system. SYT-P is designed to bring together stakeholders across the systems serving adolescents and transitional aged youth to plan for a coordinated state-wide network to develop policies, expand workforce capacity, disseminate evidence-based practices (EBPs), and implement financial mechanisms and other reforms. The aim is to improve the integration and efficiency of the treatment and recovery support system serving the population of focus. SYT-P seeks to address behavioral health disparities among racial and ethnic minorities by encouraging the implementation of strategies to decrease the differences in access, service use and outcomes among the racial and ethnic minority populations served.

    The Second Chance Act of 2007 (Pub. L. 110-199) provides a comprehensive response to the increasing number of incarcerated adults and juveniles who are released from prison, jail, and juvenile residential facilities and returning to communities. There are currently over 2.2 million individuals serving time in our federal and state prisons, and millions of people cycling through tribal and local jails every year. Ninety-five percent of all people incarcerated today will eventually be released and will return to communities. The coordination of reentry of members of Native American tribes is even more complex given that they can return from federal, Bureau of Indian Affairs (BIA), state, local, and tribal facilities. The Second Chance Act helps to ensure that the transition individuals make from prison, jail, or juvenile residential facilities to the community is successful and promotes public safety. The Second Chance Act Programs are designed to help communities develop and implement comprehensive and collaborative strategies that address the challenges posed by reentry and recidivism reduction. "Reentry" is not a specific program, but rather a process that starts when an individual is initially incarcerated and ends when he or she has been successfully reintegrated in the community as a law-abiding citizen. The reentry process includes screening and assessment in a pre-release setting, the delivery of evidence-based programming in a pre-release setting, and the delivery of a variety of evidence-based programming for every program participant in a post-release setting designed to ensure that the transition from prison or jail to the community is safe and successful.     The purpose of this Funding Opportunity Announcement (FOA) is to encourage applications for Phased Innovation (R21/R33) projects to determine whether reductions in illicit drug use are associated with positive changes in health-related and other functional outcomes in individuals with substance use disorders (SUDs).  Functional outcomes include, for example, reductions in morbidity, mortality, criminal justice involvement, overall healthcare expenditures.  This FOA provides support for up to two years (R21 phase) for research planning activities and feasibility studies, followed by possible transition to expanded research support (R33 phase).  The transition to the R33 phase will be determined by NIH program evaluation of successful achievement of the milestones set for the R21 phase.
