    The purpose of this Funding Opportunity Announcement (FOA) is to seek research applications that pilot test approaches to improve diabetes and obesity prevention and/or treatment in healthcare settings where individuals receive their medical care. Research applications should be developed to pilot test practical and sustainable strategies to improve processes of care and health outcomes for individuals with or at risk of diabetes and/or obesity. The research should also focus on approaches with the potential to be broadly disseminated outside the specific setting where it is being tested. The goal is that, if the pilot study shows promising results, the data from the R34 will be used to support a full-scale trial focused on improving routine healthcare practice and informing healthcare policy for the prevention or management of diabetes and obesity. Therefore, interventions must be integrated into the existing healthcare structure and/or processes. The healthcare setting may not be used solely as a venue for recruitment.

Through the annual research program, the association awards grants of up to $115,000 over three years in support of creative and innovative projects that hold significant promise for advancing the prevention, cure, or treatment of diabetes. Priority is given to high-risk projects with the potential to generate high-impact results; projects that extend existing knowledge into a new area or application; projects that utilize novel technological or cross-disciplinary approaches; and/or projects that may not be sufficiently developed for traditional funding sources (e.g., NIH R01). Applicant must hold a PhD, MD, PharmD, DO, or DPM degree (or, for other health professionals, the equivalent doctoral-level health- or science-related degree) and possess the necessary skills and training to carry out the proposed work.     Once established, type 1 diabetes (T1D) requires daily insulin administration and episodes of hyperglycemia and hypoglycemia are common. As a result, patients with T1D without adequate control may suffer devastating acute complications such as life-threatening hypoglycemia and ketoacidosis and long term complications including accelerated cardiac and peripheral vascular diseases, nephropathy, retinopathy, neuropathy, amputation and premature death. Studies such as the Diabetes Control and Complications Trial (DCCT) and the UK Prospective Diabetes Study (UKPDS) have shown that long term microvascular and macrovascular complications can be reduced through tight blood glucose control. However, the increased risk of hypoglycemia that is associated with tight control and the limitations of current glucose control technologies make tight control difficult to achieve. Thus, there is a need for tools to help more people with T1D to safely achieve and sustain tight blood glucose control and reduce burden of care.

    Despite the availability of increasingly effective treatment modalities, including insulin analogues, continuous glucose monitors (CGMs), and continuous subcutaneous insulin infusion (CSII) devices, a substantial proportion of patients with type 1 diabetes (T1D) cannot achieve adequate glycemic control and avoid acute complications such as hypoglycemia.  NIDDK has long supported this area of research, especially through small business projects, and this support has contributed substantially to the development of new devices that are increasingly used in clinical practice.  Nonetheless, the approved devices and current technologies still have significant limitations, and it is important to put renewed emphasis on the creation of the next generation of devices that will further the goals of relieving patients of the burden of diabetes self-management and achieving daily euglycemia to prevent acute and chronic complications. However, given the pace of technical progress in this field and the substantial room for improvement in reliability, accuracy, and patient burden of current devices, NIDDK considers it important to continue supporting research in this field. This FOA is intended to support cutting edge research conducted by small business leading to the development of innovative technologies that may advance progress toward an integrated, long term, wearable/implantable, glucose regulated open and closed loop insulin/pancreatic hormone delivery systems.  This announcement has two main purposes: a) promote technical innovation and b) pre-clinical testing of single or combined components of open and closed loop systems

    This Funding Opportunity Announcement (FOA) invites an application from the current awardee institution of the Accelerating Medicines Partnership in Type 2 Diabetes Knowledge Portal (AMP T2D KP). The AMP T2D program is a collaborative partnership between the NIH, pharmaceutical companies, and nonprofit organizations to develop new models for identifying and validating promising biological targets to serve as biomarkers and/or for drug discovery (www.nih.gov/research-training/accelerating-medicines-partnership-amp/type-2-diabetes). This is a one-time FOA to provide funds to the AMP T2D KP to continue the development of the web portal and underlying, curated knowledge base; as well as to become the flagship resource encompassing the myriad types of data and information that provides an understanding of the biological heterogeneity in patients with T2D and its complications. In its next five-year period, the AMP T2D KP will be expected to expand the KPs data sets and traits, import additional data types, deploy additional analytical and visualization tools, and increase its utility to a more diverse user base. To support the conduct of these studies, the AMP T2D KP will coordinate, support, and work with companion Functional Genomic Project awardees (RFA-DK-19-012) to form a Consortium. The AMP T2D KP will also oversee an opportunity pool of funds to solicit collaborative projects to accomplish the objectives of the Consortium.
