There is a need for a cultural change in clinical education, in which those involved with the professional training of healthcare professionals perceive teaching as more than the transmission of knowledge and technical skills. Process-oriented teaching practices that integrate technology as part of a carefully designed curriculum may have the potential to facilitate the development of capable healthcare graduates who are able to navigate the complexity of health systems and patient management in ways that go beyond the application of knowledge and skills.

"In conclusion, a relatively small percentage of students use multimedia (audio and video) that are provided as a duplication of lectures in the basic sciences. The distribution of frequency of access of both video and audio files was consistent across the various courses offered in the first two years of medical school. There were significant correlations in the frequencies with which individual students viewed videos of lectures from course-to-course. Finally, there was a trend for an inverse association between the frequencies with which students viewed lectures and the grades they received in the course. I heard Trojanowski (the senior author) talk about this last month at the annual meeting of the American Association for Clinical Chemistry. It's a very interesting topic with a lot of social ramifications. The statement in the absrract that among people with mild cognitive impairment, the biomarker profile had a 100% sensitivity for development of AD needs to be carefully considered. First, the metric that is of greater interest is the positive predictive value. Second, it is known that some patients who have the classic AD findings at autopsy did not have mental impairment in life. We know this from the Nun study.

Aim Because it deals with qualitative information, portfolio assessment inevitably involves some degree of subjectivity. The use of stricter assessment criteria or more structured and prescribed content would improve interrater reliability, but would obliterate the essence of portfolio assessment in terms of flexibility, personal orientation and authenticity.
