    This online resource begins by explaining Indicator 13 and 14 which is important because many parents and students are unfamiliar with the term. The opening page also gives families an overview of agencies that specialize in special education transition services. There are several links that provide important information that can be very useful to families in this situation. In my opinion, this website is full of wonderful information and at times seems a bit overwhelming. Some of these important areas are; occupational therapy, early childhood, funding, physical therapy and parent partnerships. I would use this resource as a guide to parents and teachers who are in the process of transitioning a student/child to middle or high school. I would have a meeting with the student and parents to discuss the transition process and ask the parents to view the website to that they have an understanding about the different agencies involved to help.

    The Wisconsin Post High School Outcomes Survey (WPHSOS) assesses the outcomes of individuals with disabilities one year after they have exited high school. Former students are contacted for a telephone interview to assess areas independent living, participation in postsecondary education, employment, and high school IEP planning.

    The Monadnock Center for Successful Transitions (MCST), is a training and technical assistance center providing support to individuals (ages 14-21), especially individuals with disabilities, as they transition from high school throughout adulthood. MCST also works to improve the employment opportunities for individuals with disabilities by fostering professional connections within the community and supporting projects that encourage employment.

    The amount of information on this site in innumerable. This is such a good resource for those who are involved in transtion services. I especially enjoy the "Transition Planning" page under "Educators". This page features a variety of resources to help special educators help their students plan for transition.
