    The Medicines and Healthcare products Regulatory Agency (MHRA) has reminded healthcare professionals prescribing aripiprazole to alert patients about its possible side effects, following a rise in the number of reports of gambling disorder associated with the drug. Patients taking aripiprazole, which is used for the treatment of schizophrenia and bipolar disorder, are at risk of developing unusual urges or cravings that they cannot resist, including behaviours such as addictive gambling, excessive eating or spending, or an abnormally high sex drive. The regulator has received 69 reports of gambling or gambling disorder suspected to be caused by aripiprazole in the last 14 years via the Yellow Card Scheme, out of which 32 were received between 1 January and 31 August this year. Alison Cave, MHRA Chief Safety Officer, said: "The number of reports for suspected gambling and other impulsive behaviours associated with aripiprazole are small in comparison to the frequency with which it is prescribed, but the consequences for any patient developing these conditions can be significant."

NICE recommends first long-acting injectable treatment for HIV-1 infection - 0 views

    In its first, the National Institute for Health and Care Excellence has recommended the use of long-acting injectable treatment for HIV-1 infection in adults, making around 13,000 people eligible for treatment with cabotegravir with rilpivirine in England. The new treatment aims to keep the number of virus particles in the blood so low that it cannot be detected or transmitted between people. Current treatment for HIV‑1 is lifelong antiretroviral tablets taken each day. Use of cabotegravir with rilpivirine is recommended when there is no evidence tosuspect viral resistance, and no previous failure of other anti-HIV-1 medicines. "Clinical trial results show that cabotegravir with rilpivirine is as effective as oralantiretrovirals at keeping the viral load low," NICE stated. Both are administered as two separate injections every two months, after an initial oral (tablet) lead-in period.

Mark Goodarzi, the director of the Endocrine Genetics Laboratory at Cedars-Sinai in Los Angeles, California, is leading an ongoing study that is following and observing people at risk for diabetes to learn whether those with lower levels of these bacteria develop the disease. "The big question we're hoping to address is: Did the microbiome differences cause diabetes, or did diabetes cause the microbiome differences?" said Goodarzi, who is the senior author of the study and principal investigator of the multicenter study called Microbiome and Insulin Longitudinal Evaluation Study (MILES).

    The outbreak of Burkholderia cepacia complex (Bcc) in the UK, which is associated with contaminated eye gels imported from India, has led to the death of one person, and many others falling ill, according to a government report. Bcc is a group of antibiotic-resistant bacteria that are widely found within the environment. Although it rarely causes infection, it can result in severe ones in individuals with compromised immune systems and those living with cystic fibrosis, the UK Health Security Agency (UKHSA) said. Between January 2023 and February 2024, there were 52 confirmed and six probable cases across the UK linked to the bacteria outbreak, as revealed in the Health Protection Report published by UKHSA. Forty-one of these cases were hospital inpatients, 38 of which were in critical-care settings. Twenty-five cases were considered to have "clinically significant infections attributable to Bcc", out of which 11 had eye infections, nine had respiratory infections and four had bacteremia (bacteria entering the bloodstream). Two individuals with cystic fibrosis (CF) were infected, and while one was treated, the other died, with the report indicating Bcc infection to have "contributed to the death."

    Britain's drug regulator, the Medicines and Healthcare products Regulatory Agency (MHRA) on Monday announced that fluoroquinolone antibiotics must only be administered when no other antibiotics are appropriate for use. It elaborated that fluoroquinolones given systemically (by mouth, injection, or inhalation) should only be prescribed "when other recommended antibiotics have failed, will not work due to resistance, or are unsafe to use in an individual patient." Further restrictions have been introduced after receiving reports from patients who have experienced long-lasting or disabling reactions following use of fluoroquinolones, the MHRA revealed. We have listened to the experience of patients regarding long-lasting and potentially irreversible adverse reactions following use of fluoroquinolone antibiotics, in some cases prescribed for mild-to-moderate infections.

The LSHTM researcher team will be working together with experts at the UK Health Security Agency (UKHSA) and the Universities of Oxford, Manchester and Nottingham on the project. Dr Rebecca Glover, assistant professor in Antimicrobial Resistance at LSHTM, who will lead the three-year project, said they will evaluate "Pharmacy First's impact on GPs and the wider NHS, pharmacy services and patients."
