It's time for pharmacists to get their technology forecasting hats on and be selective in choosing digital technologies, commented Prof James Woudhuysen, of London South Bank University, at the National Pharmacy Association (NPA) Conference, Pharmacy Odyssey, held on Thursday (13 October). In a session on 'Healthcare in tomorrow's digital environment', hosted by Fergus Walsh, Prof Woudhuysen opined that digitisation has its merit, but it's quite tricky, and slower than we imagine. "Mechanisation has a digital component that we can look to as a way out of the gloom of the moment. And in dispensing, labelling, prescribing, and many other features of the conventional retail pharmacist, there's a lot that could be done for productivity and automation," he said. Replying to a question on 'where are we at the digital front now and where can it go', Woudhuysen replied: "It's taken 20 years for the electronic prescription. It was Tony Blair, who wanted every prescription electronic. The mishaps on repeat prescriptions and all the rest of it are still quite intense. So, we need to remember that electronics is only as clever as software and then human input into it. There're still many mistakes.

French drugmaker Sanofi will make 30 of its treatments, including insulin, available on a not-for-profit basis in 40 lower-income countries in the first step of its plan to increase access to its medicines worldwide. The treatments will be provided under the new Impact brand, part of Sanofi's global health unit launched last year, which sits outside the commercial business. The plan also includes the $25 million Impact fund, which will go towards supporting local start-up healthcare businesses and providing training on using the medicines, Sanofi said. "There's a lot of noise at the moment from different companies jumping into this space… but investing in entrepreneurship, in the ecosystem, is a new thing," said Jon Fairest, who heads the global health unit. The treatments available are on the World Health Organization's essential medicines list, and insulin in particular has been singled out by the UN agency as a life-saving medicine for diabetics that is difficult to access in many lower-income countries, where the burden of disease is growing.

If the pharmacist has the time, I ask them a few questions and once we start the conversation and they find out that I'm the general manager for a company that provides them with some of their big brand products, I often get a look of total surprise on their face, especially as I've come to visit their store and take the time to speak with them personally. I've really seen first-hand what a busy bunch they are! "I often want to know how the shop front is presented? What does the pharmacy look like from inside? Is there a big queue? What kind of advertisement the pharmacy has in place? How are our products merchandised?

The Pharmaceutical Services Negotiating Committee (PSNC) has challenged price concessions imposed by the Department of Health and Social Care (DHSC) on some of the medicines. It has raised concerns on the process for setting price concessions to senior government officials responsible for medicines supply, warning that the system is not working in the current environment from a community pharmacy contractor perspective. "When a new government takes over next week, this will be one of several urgent topics being raised by PSNC to new ministers, alongside the fuel price crisis, inflationary pressures and winter pressures on pharmacy businesses," said PSNC. The final update to August price concessions was announced yesterday taking the total concessions granted to a record 138 for August. PSNC said: "Of those, 99 were in line with PSNC requests, but prices for 39 lines were imposed by the Department of Health and Social Care (DHSC): those impositions do not match the purchase prices reported by contractors and the evidence of market prices which we passed on to DHSC. In particular, the final imposed prices of Apriprazole and Temazepam tablets has generated a lot of concern amongst contractors due to the large variation between their reimbursement prices and purchase prices during the month of August."

When I was asked by fellow independent contractors to join the Review Steering Group (RSG) back in late 2020, I said yes, because I wanted, fundamentally, to look for some ways to improve both LPCs and PSNC. Through my experiences with Greater Manchester LPC I had seen the sorts of changes that LPCs could make and the positive impact that those had on local engagement and in allowing us to do more to support contractors: I wanted to help share what we had learned with the rest of the country. I was pretty sure that the national negotiator could make some positive changes as well. It took longer than I had hoped. And it was a lot harder than I had imagined - not least because of the extent of some of the divisions within our sector - but after 15 months of hard work, I'm pleased with where we have got to, and I believe the 37 changes being proposed will help all contractors. Consensus is not a sexy word. It is not something that fires up our imaginations or that we put on our list of key work objectives for the year. It is also not something that might come naturally to us as independent contractors. But consensus is something that matters. It is the thing that allows us, as a vibrant and diverse group of businesses, to see beyond our differences and to fight together for outcomes that benefit every single one of us. Consensus is also the thing that the RSG has been relentlessly focused on throughout its work, and it's at the heart of what will give our LPCs and PSNC a stronger voice for community pharmacy in the future.

Two weeks prior to the closing of the hub and spoke consultation, issues around patient safety, costs and competition as well as practicalities such as what to do with uncollected medicines were discussed in a webinar on hub and spoke dispensing on Wednesday (May 26). The webinar was organised by Sigma Pharmaceuticals in association with the National Pharmacy Association (NPA). NPA director of corporate affairs Gareth Jones chaired the event and was joined by Sigma's Hatul Shah and Raj Haria as well as NPA vice chair Nick Kaye and NPA head of advice and support services Jasmine Shah. Kaye said: "There are still many 'unknown unknowns' with hub and spoke and the jury's still out on any potential benefits. I have lots of reservations about cost and it worries me how efficient this is for the business and the long term sustainability for the sector as a whole. Above all we need to think about the patient at the centre of all this and the potential confusion for them." Jasmine Shah felt patient safety was going to be the key in regards to whether the spoke and the hub would hold the ultimate responsibility and "who is going to take the accountability as far as patient care goes". She added: "It is most important that GPhC standards and NHS requirement are both met in identifying all the risks associated with the arrangement with hub and spoke and ensuring that (patient safety) measures are in place. Everything that needs to be looked at is by putting the patient at heart of the arrangement and seeing what is the safest way for them to receive care from both spoke and hub."

Asda Ramsgate pharmacy worker Claire Twyman saved life of a regular patient Rose Doyle by giving her a timely advice to visit A&E after spotting a blood clot on her leg. Doyle (75) spent five days in Margate Hospital, and after recovering she called in at the store to thank Twyman for her quick-thinking actions. The National Pharmacy Association (NPA) has called on the NHS England to uplift funding for pharmacy contractors to enable them to cover higher staffing costs along with a range of other cost-inflating factors. This follows latest data released by the recruitment platform Locate a Locum, which showed a huge increase in locum rates for pharmacists in 2021 and predicted the trend to continue. The report noted a 71 per cent surge in the cost of employing locum pharmacists in England, from the 2020 average to the second half of 2021. NPA chief executive Mark Lyonette said: "There is a heavy reliance on locums in community pharmacy to maintain continuity of services with the average pharmacy operating 50 hours per week. Consequently, increases in locum rates have a big effect on the cost base. "Pharmacies face a range of general cost pressures beyond locum rates, including much higher energy costs. We hear a lot about the cost of living crisis; our members are facing a cost of doing business crisis and it's every bit as real. "The underlying underfunding, significant general inflationary pressures and specific cost increases relating to the locum workforce together make a powerful and urgent case for new funding."

The National Pharmacy Association (NPA) Board has elected 14 Board members on Friday (30 March) that will govern the association for next two years. Four new Board members have been elected while five previous members left the Board after serving their terms. The period April 2023 to March 2025 will see the completion of the transition to the new Board structure. The Board will meet again on Monday (24 April) to elect the Chair and other appointments. Gareth Jones, Returning Officer for the NPA Board elections, said: "The NPA has undertaken a process of significant modernisation of organisational governance over the past years. Key elements of this process include adopting modern new Articles of Association, reforming the structure of the Board and introducing term limits. The process of electing the Board has also been changed so that half of the Board will be up for election every two years - which supports continuity and reduces the risk of a loss of organisational memory." "Recognising that the Board would already be losing a lot of organisational memory in 2023 with five members of Board standing down, the Board determined that three individuals should be co-opted onto the new Board as the process of transformation continues. In March 2025, anyone that has served 12 years or more will be required to stand down."

Various factors can be taken into account in determining reimbursement prices. The Royal Pharmaceutical Society (RPS) has urged people across Scotland to be kind to their pharmacy teams this winter through its campaign called 'Be Kind'. The pharmacies are facing lot of pressure and challenges during winter, RPS has heard from its members that abuse towards pharmacy teams, particularly those in community pharmacy, has 'unfortunately' risen. The campaign has been running throughout December, to promote 'be kind' message to the public at a time when pharmacy services are particularly stretched by winter pressures and other factors, including the recent shortages of antibiotics. "Throughout December, leading figures have provided a series of powerful videos, which all encourage the public to be kind to their pharmacy teams. Two Indian pharmaceutical companies, Cipla and Glenmark, are pulling back certain products from the American market due to manufacturing concerns, according to the US Food and Drug Administration (USFDA). A New Jersey-based subsidiary of Cipla is recalling 59,244 packs of Ipratropium Bromide and Albuterol Sulfate Inhalation Solution due to "short fill", news agency PTI reported, quoting the latest Enforcement Report issued by the US health regulator. Produced at the company's Indore SEZ plant, this medication is used to manage symptoms associated with lung diseases, such as asthma, chronic bronchitis and emphysema. In response to the concerning surge in youth vaping, the UK government in January announced its decision to ban disposable vapes as a measure to safeguard the health of children. Prime Minister Rishi Sunak stated that while the long-term impacts of vaping were uncertain, the nicotine present in these products can be highly addictive. Now, there's a stronger reason to crack down on vape sales as a new study has revealed that vaping or the use of e-cigarettes may increase the risk of developing heart failure. The study, which involved 175,000 adults in the United States, found that those who use e-cigarettes were 19 per cent more likely to develop heart failure over a four-year period. Dr Yakubu Bene-Alhasan, lead author of the study, from MedStar Health in Baltimore, underscored that an increasing number of studies are linking e-cigarettes with harmful effects, suggesting vaping "might not be as safe as previously thought." With further research, Dr Bene-Alhasan's team plans to uncover "a lot more about the potential health consequences and improve the information out to the public."
