Dental hygienists and therapists, under the proposed change, will be empowered to administer select medications, including pain relief and fluoride, without the need for dentist approval.

A PAGB report has emphasised the need to empower the public to practice self-care and improve access to over-the-counter (OTC) medicines to alleviate pressure on GPs and reduce NHS costs. Launched at the Labour Party Conference today, the PAGB's State of the Nation report explores the current state of self-care and OTC medicines. The report outlines 11 recommendations for the government to consider as part of its strategy to build an NHS fit for the future. It highlights that each year, at least 25 million GP appointments and 5 million A&E visits are used for self-treatable illnesses, which can be effectively managed through self-care. The report suggests that empowering individuals to utilise OTC medicines could save the NHS at least £1.7 billion per year in costs. Furthermore, it says that eliminating these appointments could save the economy an additional £350m per year, as employees would not need to take time off work for unnecessary treatments. PAGB welcomed the government's manifesto commitments to take pressure off GP surgeries by "improving access to services and treatment through new routes" and to expand the role of pharmacists.

Over the last few years, we've seen the aftermath of Covid-19, with the demand for consumer health products being unprecedented and unpredictable, placing pressure on supply chains and labour market. In turn, the industry has experienced acute shortages across the healthcare sector, placing huge pressure on pharmacists, doctors, nurses, and other healthcare professionals. Europe has an estimated shortage of around 50,000 public-sector doctors[1], which is set to increase in the coming years. While this shortage stems from several complex problems, there is a hidden force that could play a key part in helping to alleviate this burden. That force is the potential of self-care. Encouragingly, last month the UK government recognised the unique role that self-care can play through the introduction of its Delivery plan for recovering access to primary care. Through this, it has pledged to empower patients to manage their own health through several commitments, including improving accessibility to online tools and reclassifying medicines to make formerly prescribed options available over the counter.

[1] With the growing prevalence of diabetes, it's even more important to utilise all options to support people with diabetes (PwDs) in managing their daily life with diabetes. Where do PwDs most often access healthcare support? On average, PwDs visit the pharmacy three-to-eight times more than the general population to obtain medicines and testing supplies.[2] It is clear that pharmacists play an integral role in empowering the self-management needs of PwDs beyond the traditional role of supplying medicines. This relationship, however, may be underutilised. Research suggests there is a lack of broad recognition of pharmacists as a key source of diabetes care, and that resource constraints - particularly following the COVID-19 pandemic - can interfere with patient education provision and training.[3,4] Furthermore, the circumstances and role of the pharmacy vary, nationally and internationally. At the recent Prime Minister's Questions last Wednesday, Angela Richardson, the Conservative MP for Guildford, raised the issue of rising pharmacy closures, revealing that Guildford recently lost two neighbouring pharmacies. Highlighting that empowering local pharmacies is a key part of this government's plan to cut waiting lists, she asked Sunak whether he would agree that it is vital for residents to have access to "a good, efficient, and above all, local pharmacy." A new report from the Royal Pharmaceutical Society (RPS) has called on the government to create a national strategy to manage medicine shortages and to change legislation to empower community pharmacists to amend prescriptions when medicines are in short supply. Currently, patients are required to return to their GP to get their prescriptions amended, leading to delayed access to medication, increased bureaucracy, and additional strain on an already overburdened system. The RPS report, backed by charities and patient groups, proposes allowing pharmacists to make minor amendments, such as adjusting the quantity, strength, or form of prescribed medicines to ensure timely access for patients. Additionally, the report highlighted key factors causing medicine shortages, including manufacturing and supply chain issues, exacerbated by cost pressures and consolidation of manufacturing outlets. This along with unplanned spikes in demand, such as shifts in prescribing practice or increased diagnosis of some conditions, have created "a perfect storm of unstable supply"

The Pharmacists' Defence Association (PDA) has secured the right to negotiate and agree on proposed changes to the NHS Agenda for Change (AfC) terms and conditions for its members. This milestone follows an agreement with other NHS trade unions, enabling the PDA Union to join the NHS Staff Side. As part of this group, the PDA can now actively participate in the NHS National Joint Council, which oversees changes to AfC. Participation in the NHS Staff Side brings significant benefits for PDA members, especially those employed in the NHS under AfC. In addition to providing the PDA the right to negotiate and agree proposed changes to AfC, it empowers the trade union to advocate for pharmacists at the highest levels in the NHS, participate fully in the NHS Social Partnership Forum, and collaborate with other unions on critical issues such as NHS pay. It also gives PDA members the right to be represented at local trust level on matters handled by local staff sides.

Throughout the past decade, digital transformation has been at the forefront of the agenda for the NHS which has been accelerated by the abundance of digitally enabled ways of working during the pandemic. While the last year has unquestionably heralded the spring shoots of transformation, there is so much to be done to build upon this and accelerate further. For example, do both pharmacists and patients comprehend the value of digital to drive change at a fundamental level? The issue is not simply ensuring that the temporary fixes inspired by the pandemic successfully transition to standard practice, but also rapidly building on that foundation to enable the greatest value from digitisation within pharmacy. Covid-19 has increased the need for digitally enabled and empowered ways of operating. From the backlog in outpatient appointments to postponed screening programmes and, crucially, the potential exodus of overworked and stressed pharmacists, the efficient, effective and intelligent care enabled by the right digital technologies is now critical.
