May I be one of the last people to wish you a 'Happy New Year' The new year is traditionally a time to reflect on the year past and look forward to the new year and all the joys and challenges it will bring, possibly by making a resolution. However, the statistics do not bode well for your resolution lasting. Whether it is to improve your health/lose weight (the no.1 resolution) or performing better at work (3%), the Forbes Health/One Poll survey found that the average resolution lasts just 3.74 months. Only 8% of respondents tend to stick with their goals for one month, while 22% last two months, 22% last three months and 13% last four months. So that's nearly 2/3 gone by 4 months. I only mention this because at the tail end of 2024, I thought Pharmacy, or more specifically Pharmacy Representative bodies had taken an early resolution and decided to get along. I am passionate about Collaboration and am a member of the Institute for Collaborative Working (ICW) and have spoken on the topic at conferences, the Pharmacy Show, etc, and also written in this very magazine on the subject (see Volume 20 March 2024). I have also been pretty dismayed at the lack of collaboration at a representation level leading to headlines such as "Don't underestimate the power of unity" and "Sector disagreement over call for unity" coupled with some 99% of participating pharmacy owners saying they were willing to limit their services unless funding is improved.

According to Baxter the new collaboration "will better enable health professionals across primary and secondary care to make confident decisions about medicines, ensuring patients get the best from their treatment." Moreover, MedicinesComplete has made it easy for health professionals globally to access trusted medicines information since 2004, provided expert guidance on the safe use and administration of drugs and medicines, in one place.

The National Institute for Health and Care Excellence (NICE) has published a new diagnostic guidance to enhance CYP2C19 genotype testing to guide clopidogrel use for patients who have suffered an ischemic stroke or transient ischemic attack. Published in July, the diagnostic guidance also emphasised the vital role of pharmacists and pharmacies in the implementation of CYP2C19 genotype testing used to guide prescribed antiplatelet medication. According to the NICE guidance, pharmacists with their expertise in pharmacogenomics and patient medication management, will be integral in interpreting test results and recommending appropriate antiplatelet therapy. It also highlighted the need for close collaboration with other healthcare professionals to ensure effective implementation across various care settings for stroke patients. NHS England has announced a phased implementation for mandatory multi-sector rotations in the Foundation Pharmacist Training Programme, delaying the full rollout to the 2027/28 training year. Under the General Pharmaceutical Council's Standards for Initial Education and Training of Pharmacists (2021), multi-sector rotations - where trainees spend at least 13 weeks in a different pharmacy setting- were initially planned for all trainees starting in 2026/27. These rotations are designed to enhance pharmacists' adaptability, multi-professional collaboration, and ability to deliver person-centred care across healthcare systems. Acknowledging significant progress from pharmacy employers in developing these programmes, NHS England cited current workforce pressures as a key reason for introducing a phased approach. "We are mindful of the current pressures faced by pharmacy teams and the broader healthcare sector and greatly appreciate your continued commitment to delivering high-quality training. To support pharmacy teams in continuing to deliver high quality training, mandatory rotations will be delayed, and a phased approach will be implemented," David Webb, chief pharmaceutical officer for England and Samantha Illingworth, director of education quality and reform at NHS England, wrote in a letter to training programme providers.

The General Pharmaceutical Council (GPhC) has unveiled its Carbon Net Zero Action Plan for sustainable pharmacy regulation, aiming to achieve carbon net zero by 2040. The action plan focuses on three main elements: setting an example, incorporating sustainability into regulatory standards, and fostering collaboration for sustainable pharmacy care. Roz Gittins, Chief Pharmacy Officer at the GPhC, emphasised the urgent need for pharmacy sector to embrace sustainable healthcare practices. "Climate change is a global emergency and has been declared the biggest threat to public health," she said, adding that it not only poses health risks but also amplifies existing health inequalities, with immediate consequences for patients, the public and health services. She highlighted that the GPhC is actively engaged in addressing climate change, both as an employer and regulator. "Our aim is to positively influence the pharmacy sector to embrace climate action and sustainable healthcare principles in line with our vision to have safe and effective pharmacy care at the heart of healthier communities," Gittins said.

Prescribing in pharmacies is an emerging opportunity, and I'm passionate about supporting proper governance arrangements for businesses. But does the governance and administrative burden reduce if your business is not doing the prescribing? Recently, I've come across several independent pharmacy businesses across the UK who have been approached by prescribing clinics to dispense medicines for their patients. While these partnerships can present lucrative opportunities, they also come with significant responsibilities and potential risks. Pharmacy owners and superintendent pharmacists must be fully aware of the guidance set by the General Pharmaceutical Council (GPhC) and ensure that all requisite checks are performed to maintain compliance and safeguard patient safety. The growing trend of prescribing clinic collaborations For one reason or another, private prescribing clinics are sprouting up all over the place, offering specialised services such as weight management, hormone therapy, or dermatological treatments. These are quickly becoming a prominent feature in our healthcare landscape. Many of these clinics lack in-house dispensing capabilities, leading them to seek partnerships with pharmacies to satisfy a growing number of consumers. In the current economic climate, these collaborations can offer a new revenue stream and an expanded patient base. However, with these opportunities come responsibilities that must not be overlooked.

After struggling with COVID, Hemant Petal is back to good health, and now he is on a mission to reduce "health inequalities". Speaking at the SIGMA Conference 2023, he spoke about his new role as Clinical Lead, Health Inequalities and Population Health Management, SE Essex Alliance, Essex ICB. "So, my new role is outside pharmacy, nothing to do with pharmacy. But it is important, I feel that I share some of my insights about the work that I'm doing that might benefit you," he said. Elaborating on the role of integrated care boards (ICBs), he said that they are designed to enhance coordination and collaboration across different healthcare providers and settings. The representatives from hospitals, community services, retail and social care meet on a monthly basis to "look at where there are issues to be resolved, perhaps what can be done to improve the healthcare services."

In a collaborative effort, the National Health Services England (NHSE) is set to deliver the RSV(respiratory syncytial virus) vaccination programme where pharmacists alongside other healthcare professionals will play a crucial role in administering the vaccine. The UK Health Security Agency has announced the launch of the new vaccination programme targeting the respiratory syncytial virus (RSV), set to begin on 1 September 2024. Along with GPs, NHS England (NHSE) will "commission a number of community pharmacies to deliver the programme", ensuring broad accessibility and coverage to protect two high-risk groups: older adults and pregnant women. RSV is an RNA virus related to mumps, measles, and human parainfluenza viruses. It commonly causes coughs and colds in winter, transmitted through droplets and secretions from close contact with infected individuals. While most RSV infections are mild, infants under six months and the elderly are particularly susceptible to severe diseases like bronchiolitis and pneumonia, which can lead to hospitalisation.

The National Pharmacy Association (NPA) has lashed out at the British Medical Association (BMA) after GP leaders voted in favour of the motion to terminate pharmacy blood pressure checks with immediate effect and redirect the funds into pharmacy dispensing fees. Commenting on the BMA's vote, NPA chair Nick Kaye said: "It benefits no-one, least of all patients, for one set of health care professionals to talk down the services provided by another." He emphasised the importance of investment and collaboration across the healthcare system, particularly between general practice and community pharmacies, to provide the best possible care for patients. Furthermore, Kaye stressed the value of pharmacy-based NHS clinical services in improving access to care, while also providing more convenient care for patients and relieving pressure on other parts of primary care. "It would be terribly shortsighted to row back on preventative services such as hypertension case finding, which will be cost-saving as well as lifesaving in the long term," he warned. He noted that the NHS will only meet the growing needs and expectations of the ageing population by making the most of capacity and skills across the whole system.

Pharmas don't do social media but they are world champs in sales and marketing; if you look at the balance sheet of one of the top 10 innovative pharmas, you will see that over 50% of expenses are for sales and marketing; more than R&D.

NHS England will collaborate with "a number of system suppliers" to increase the number of trusts using electronic prescription service (EPS) over the next few years. Nishali Patel, clinical lead for digital medicines at NHS England, announced it during the Clinical Pharmacy Congress, held at the Excel, London, on Friday, 10 May 2024. According to Nishali, at least half of all NHS secondary care trusts could be using EPS by 2026. This digital service allows prescribers to send prescriptions electronically to a patient-nominated pharmacy. It is widely used in primary care, where over 95 per cent of prescriptions are now electronically generated. NHS recently extended the service to secondary care settings.

Has the government in England learned nothing from the Covid-19 pandemic? Nigel Swift, Managing Director of Rowlands Pharmacy, part of Numark, the UK's largest pharmacy membership association, asks reflecting on the disorderly launch of this year's flu and Covid booster vaccination program as evidence to the contrary. "Community pharmacies play a crucial role in ensuring eligible individuals can easily access their free NHS flu and Covid vaccinations," Swift said, adding that delivering this essential service necessitates pharmacy staff allocating time for preparation and appointment scheduling. "Just a few weeks ago, NHSE informed us that the vaccination program would be postponed until October, prompting pharmacies to plan accordingly and notify patients," he said. "However, NHSE has now reversed its decision and set the program to commence on Sept. 11. Pharmacy recommended an earlier start, but NHSE opted for a delay instead, resulting in avoidable problems." On Aug. 30, NHSE announced plans to move up the start of the autumn vaccination drive in England to September 11, deviating from the initial October schedule, citing concerns about a new Covid-19 variant.
