70 per cent of community pharmacies still waiting for government nod to start Covid booster jabdesc-Nearly 70 per cent of community pharmacies in England that offered to participate in the Covid-19 booster programme are still waiting for approval or have been refused, ministers admitted on Monday (December 13). The Liberal Democrats' health spokesperson, Daisy Cooper, exposed the low approval rate and long delays through a written parliamentary question to the health secretary as to "how many community pharmacies were providing Covid-19 vaccinations as of 3 December 2021."In response vaccines minister Maggie Throup said: "As of the end of September 2021, 4,733 contractors submitted 5,376 expressions of interest to take part in the booster vaccination programme, with 1,454 accepted."She added: "As of 3 December 2021, 1,336 community pharmacy vaccination sites were actively providing the service." Some applications were decided by NHS England rather than the Department of Health and Social Care (DHSC).

Surrogacy itself calls up a lot of moral and ethical concerns, mostly because of the money that is involved. This method is used because the intended mother either can not carry a baby full term, has had a part hysterectomy, or has other medical reasons that prevent her from becoming pregnant. In the majority of these cases, the surrogate mother will be giving this service due to altruistic reasoning. She will still receive reimbursement for her and her family for any out of pocket expenses she may incur in the 9 months while she is carrying the child'. This amount of money is called the base fee. This is an amount of money that is usually split up into 9 monthly payments. The amounts are never equal in their amounts because of the fact that a miscarriage is more likely in the first three months than at any other time during the pregnancy. So, understandably, the first couple of payments will be less than offered later in the pregnancy so that the intended parents will not have to pay too much for a failed pregnancy but the surrogate mother

There are pros and cons to both sides of this argument. With the private arrangement, you will first of all need to find your surrogate mother. This is usually much easier done through an agency, as all surrogate mothers to be will have been screened and will have gone through a comprehensive health screening. Also, all of the surrogate mothers with an agency will have had psychological screenings to guarantee the fact that they will be able to deal with the very strong emotions that will be coursing around their bodies during the pregnancy. Unless, of course, your surrogate mother is a relative or close friend of the family in which case it would seem to be a lot easier. Ethics, moral code and emotions will still loom large in all of the concerned parties worlds. So it would still be wise to involve an attorney and legal contracts so that everyone knows exactly where they stand, and what is expected of them. In most countries and states legal and binding contracts written up by attorneys, are needed by the parents to be anyway. As they are going to have to be legally adopting the child everything, will need to be written clearly and legally about the birth and how it came about. Very often during the pregnancy the doctors and nurses will need to know why there are two women, one pregnant, the other not, and very often two fathers who are so interested in this one child. If you have gone through an agency, they will be able to advise you on what to do and say in this situation. If you are sorting out surrogacy arrangements privately then yes but if anything unexpected should happen and you are unable to deal with the problems there is a reasonable chance you will end up wishing you had gone to the agency. Agencies do have trained professionals that can help you with all of the different issues that are involved in surrogacy. As we transition in England through yet another NHS organisational change, I ask myself what does this mean for community pharmacy? I would like to think that this change will bring about opportunity and a chance for community pharmacy to showcase and continue the excellent work that was carried out during the height of the pandemic and is still ongoing today. I hope that it allows community pharmacy to be regarded as part of the NHS rather than sitting on the side lines. This change has to lead to better funding for community pharmacy, without sufficient funding we will see more pharmacies close. We are hearing a lot about winter pressures but this year it feels like all year round pressure. What I have seen, whilst the NHS is under such pressure, is North East London (NEL) CCG transitioning to an integrated care board (ICB) almost seamlessly. I have seen people transitioning into new roles, whilst working hard to ensure that all plates are still spinning, which at the moment is no mean feat. I spent a day out recently visiting pharmacies with the chief medical officer of NHS NEL, Dr Paul Gilluley. The visits were positive, we felt listened to and understood. The feedback was great, it was recognised that community pharmacy is often the informal front door to the NHS and that we have so much to offer in terms of ill health prevention. Community pharmacy can offer a total solution as long as we have the tools to do so, which can save so much time and money. An example is the GP CPCS service, which has launched well across NEL.

It's time for pharmacists to get their technology forecasting hats on and be selective in choosing digital technologies, commented Prof James Woudhuysen, of London South Bank University, at the National Pharmacy Association (NPA) Conference, Pharmacy Odyssey, held on Thursday (13 October). In a session on 'Healthcare in tomorrow's digital environment', hosted by Fergus Walsh, Prof Woudhuysen opined that digitisation has its merit, but it's quite tricky, and slower than we imagine. "Mechanisation has a digital component that we can look to as a way out of the gloom of the moment. And in dispensing, labelling, prescribing, and many other features of the conventional retail pharmacist, there's a lot that could be done for productivity and automation," he said. Replying to a question on 'where are we at the digital front now and where can it go', Woudhuysen replied: "It's taken 20 years for the electronic prescription. It was Tony Blair, who wanted every prescription electronic. The mishaps on repeat prescriptions and all the rest of it are still quite intense. So, we need to remember that electronics is only as clever as software and then human input into it. There're still many mistakes.

Two weeks prior to the closing of the hub and spoke consultation, issues around patient safety, costs and competition as well as practicalities such as what to do with uncollected medicines were discussed in a webinar on hub and spoke dispensing on Wednesday (May 26). The webinar was organised by Sigma Pharmaceuticals in association with the National Pharmacy Association (NPA). NPA director of corporate affairs Gareth Jones chaired the event and was joined by Sigma's Hatul Shah and Raj Haria as well as NPA vice chair Nick Kaye and NPA head of advice and support services Jasmine Shah. Kaye said: "There are still many 'unknown unknowns' with hub and spoke and the jury's still out on any potential benefits. I have lots of reservations about cost and it worries me how efficient this is for the business and the long term sustainability for the sector as a whole. Above all we need to think about the patient at the centre of all this and the potential confusion for them." Jasmine Shah felt patient safety was going to be the key in regards to whether the spoke and the hub would hold the ultimate responsibility and "who is going to take the accountability as far as patient care goes". She added: "It is most important that GPhC standards and NHS requirement are both met in identifying all the risks associated with the arrangement with hub and spoke and ensuring that (patient safety) measures are in place. Everything that needs to be looked at is by putting the patient at heart of the arrangement and seeing what is the safest way for them to receive care from both spoke and hub."

Covid-19 vaccine makers are shifting gears and planning for a smaller, more competitive booster shot market after delivering as many doses as fast as they could over the last 18 months. Executives at the biggest Covid vaccine makers including Pfizer and Moderna said they believe most people who wanted to get vaccinated against Covid have already done so - more than five billion people worldwide. In the coming year, most Covid vaccinations will be booster shots, or first inoculations for children, which are still gaining regulatory approvals around the world, they said. Pfizer, which makes its shot with Germany's BioNTech and Moderna still see a major role for themselves in the vaccine market even as overall demand declines. Upstart US vaccine maker Novavax and Germany's CureVac, which is working with GlaxoSmithKline, are developing vaccines they hope to target at the booster market. The roles of AstraZeneca and Johnson & Johnson, whose shots have been less popular or effective, are expected to decline in this market.

Well Pharmacy is urging those eligible to take up the offer of a further Covid booster jab this spring. Adults aged 75 years and over and anyone over 5 years old and are immunosuppressed - such as those who are undergoing chemotherapy or have had an organ transplant - are entitled to have a further Covid booster jab from mid-April, said one of the UK's largest community pharmacy chains. Taking part in the vaccination programme again, Well Pharmacy is encouraging people to book another booster as soon as they are contacted by the NHS - which should be at least six months after their previous booster. The national booking site for those eligible opens this week. Biological Hazards Biological hazards are ever-present, and prescient in the aftermath of the coronavirus pandemic.
