The Department of Health and Social Care (DHSC) recently published Lord Darzi's report on the state of NHS England, revealing that the health service is in "serious trouble." "The service has been chronically weakened by a lack of capital investment, which has lagged behind other similar countries by tens of billions of pounds," he stated. Shortly after taking office, Health Secretary Wes Streeting commissioned an independent investigation to assess the challenges facing the healthcare system in preparation for a new 10-year health plan. Former surgeon Lord Ara Darzi was appointed to lead this review. In his report, Lord Darzi pointed out that the NHS has failed to meet key promises made to the public since 2015, and patient satisfaction is now at its "lowest ever level." Rising waiting times and difficulties accessing GPs were highlighted as major concerns. "There are huge and unwarranted variations in the number of patients per GP, and shortages are particularly acute in deprived communities," he said. Lord Darzi described the state of A&E as "awful," noting that nearly 10% of patients wait 12 hours or more today.

Community Pharmacy England (CPE) has submitted evidence to Lord Darzi's investigation into NHS performance, highlighting the sector's value, challenges, and potential opportunities. Professor Ara Darzi, Baron Darzi of Denham, is leading the investigation, which has been commissioned by the Secretary of State for Health and Social Care to examine the challenges facing the healthcare system ahead of the development of a new 10-year plan for health. The investigation's findings are expected to be published in September 2024. As the sector's representative body, CPE has provided evidence to ensure that the performance and challenges faced by community pharmacies are considered as part of his review. Their submission, based on internal analysis and independent reports, highlighted the severe funding crisis facing the sector, which is significantly impairing access to care for those most in need.

A call to action has been issued by the Company Chemists' Association (CCA) in its submission to the Darzi Review, highlighting the critical need for recognition and revitalization of the community pharmacy sector. The CCA warns that chronic underfunding and a flawed Community Pharmacy Contractual Framework (CPCF) have resulted in a reduction of over 1,200 pharmacies since 2015. This reduction, coupled with increasing workforce pressures, is exacerbating health inequalities, particularly in deprived communities. The CCA's submission stresses that the government must address these longstanding funding issues to fully unlock the potential of community pharmacies in addressing the nation's health challenges. The report argues that while initiatives like Pharmacy First have been positive, they only scratch the surface of the sector's capabilities.

Community pharmacy is stuck in a vicious circle. Pharmacies are closing at an alarming rate whilst the government is talking about moving care away from hospitals and into the community to focus on preventative care. While the government talks about this vision for the future, the stark reality is that pharmacies are already struggling to meet the current Pharmacy First thresholds. And now we discover that one of the key components that could create capacity in pharmacy to deliver more clinical services has been put on hold…indefinitely. The recently published Darzi review, which was commissioned after the new government came to power to assess the state of the NHS, praised the value of community pharmacies and preventative services. But it recognised the level of pharmacy closures across the country and warned pharmacy access could be 'at risk'. Community pharmacy is ideally placed to deliver the government's vision of preventative care in the community. But let's all be clear, moving preventative services into community pharmacies has, so far, been nowhere near as successful as it could have been due to the current climate pharmacies find themselves in. The Pharmacy First scheme is a testament to this. Just this week it was announced that consultation thresholds for the Pharmacy First scheme have been reduced from 30 consultations to 20. It's the second time in three months that the thresholds have been reduced and it follows an intervention from Community Pharmacy England who wanted to "save many pharmacies from missing out on a vital payment this month." In June alone, 3,269 participating pharmacies failed to meet the threshold - that's almost a third of all community pharmacies in England.
