The Janssen Pharmaceutical Companies of Johnson & Johnson announced that the Scottish Medicines Consortium (SMC) has approved Ponvory (ponesimod) for treating adults with relapsing-remitting multiple sclerosis (RRMS) within NHS Scotland. Ponesimod offers an additional treatment choice for the ailment, which affects over 15,000 people in Scotland. Multiple sclerosis (MS) is a chronic autoimmune inflammatory disease of the central nervous system in which immune cells attack myelin, causing inflammation. The common symptoms of this disease include fatigue, balance and walking problems, numbness or tingling, dizziness and vertigo. National Institute for Health and Care Excellence (NICE)'s diagnostic advisory committee has recommended the use of HM-JACKarc or OC-Sensor quantitative faecal immunochemical tests (FIT) in a draft guidance release for consultation on Wednesday (5 July). Under existing NICE guidance, FIT was already offered to some people presenting to primary care with symptoms suggestive of colorectal cancer, while others were immediately referred on the suspected cancer pathway. The new draft guidance will now see everyone receive a FIT. A sample is sent in the post to a laboratory where the amount of blood in the faeces is measured. The results are usually available within a week and people with 10 or more micrograms of haemoglobin in their faeces should then be referred for further investigation. Further assessment using colonoscopy, or CT colonography, is required to diagnose cancer. The tests cost between £4 and £5 per sample, and can correctly identify about 9 out of 10 people with colorectal cancer. The committee agreed it is important that GPs can refer people for colonoscopy without a positive FIT result if they think it is necessary and where symptoms persist. The institute believes that the recommendation of the tests should reduce the number of unnecessary colonoscopies, thus freeing up appointments for more non-urgent referrals. This should lead to 50% fewer referrals for urgent colonoscopies being made by GPs in primary care settings each year.

The Scottish Medicines Consortium (SMC) has accepted AbbVie's RINVOQ (upadacitinib) for use within NHS Scotland for the treatment of adult patients with moderately to severely active ulcerative colitis (UC). Upadacitinib offers an additional treatment choice in the therapeutic class of janus kinase inhibitors. "Daily life with ulcerative colitis is difficult due to the debilitating and unpredictable nature of its symptoms", said Dr Jonathan Macdonald, Consultant Gastroenterologist at NHS Greater Glasgow and Clyde. "Clinical trials have shown that upadacitinib as a once daily pill controls symptoms in eight weeks for many patients and provided sustained responses at one year. The SMC's decision is good news for people with ulcerative colitis in Scotland as it provides an additional treatment option to help them gain control of their condition." The SMC decision is based on data from the induction studies U-ACHIEVE and U-ACCOMPLISH, as well as the Phase 3 U-ACHIEVE maintenance study, which demonstrated the efficacy of upadacitinib versus placebo in its ability to achieve clinical remission. The SMC's decision follows the granting of Marketing Authorisation for upadacitinib in UC in July 2022.

The Department of Health and Social Care (DHSC) has included Tibolone 2.5mg tablets (Livial) and Prasterone 6.5mg pessaries (Intrarosa) to the list of Hormone Replacement Therapy (HRT). It has updated the June 2023 Drug Tariff (Part XVI) to include these additional HRT medicines for which patients will be able to purchase an HRT PPC. "HRT is the replacement of female sex hormones oestrogen and progesterone in women to control symptoms of the menopause. Some medicines that are converted or break down into oestrogen, progestogen or androgen hormones are prescribed for relief of menopausal symptoms. For the purposes of the HRT PPC these are included within this definition of HRT," said DHSC.

A total of 37 more cases of monkeypox have been detected in England and Scotland, public health officials said on Monday, taking the total to 57. Thirty-six confirmed cases were found in England, and one north of the border, the UK Health Security Agency and Public Health Scotland said. The UKHSA said it was now advising high-risk contact cases of confirmed cases who have not tested positive or developed symptoms to isolate for up to 21 days. It has also bought supplies of smallpox vaccine, which is being offered to close contacts to reduce the risk of symptomatic infection and severe illness. Chief medical adviser Susan Hopkins said contact tracing was helping to limit the close-contact spread of the virus, which causes a chickenpox-like rash. "Because the virus spreads through close contact, we are urging everyone to be aware of any unusual rashes or lesions and to contact a sexual health service if they have any symptoms," she added in a statement.

Community pharmacies in England could soon be allowed to make cancer referrals to hospitals under new plans to improve early detection of the disease. An NHS pilot scheme, which starts later this year as a pilot in an undisclosed number of places of England, will see community pharmacy staff spotting signs of cancer in people who might not have noticed symptoms. Pharmacy teams will be trained to spot red flags, such as patients with symptoms such as a cough that lasts for three weeks or more, difficulty swallowing or blood in their urine, and send them for scans and checks without needing to see a GP if they think it could be cancer. Amanda Pritchard, the NHS chief executive, will unveil the plans at the NHS Confed Expo conference in Liverpool on Wednesday (June 15), and say: "These plans have the power to truly transform the way we find and treat cancer, and ultimately spare thousands of patients and their families from avoidable pain and loss." The plans include new "roaming liver trucks" - which will be parked near GP surgeries, in town centres and food banks - to encourage people most at risk of getting liver cancer to get "on the spot scans" for diagnosis.

The Omicron variant of coronavirus is less likely to cause long Covid than previous variants, according to the first peer-reviewed study of its kind from the UK. Researchers at King's College London, using data from nearly 100,000 people who logged their Covid symptoms via an app, found the odds of developing long Covid after infection were 20 per cent to 50 per cent lower during the Omicron wave in the UK compared to Delta. The figure varied depending on the patient's age and the timing of their last vaccination. Long Covid, which includes prolonged symptoms ranging from fatigue to 'brain fog', can be debilitating and continue for weeks or months. It is increasingly being recognised as a public health problem, and researchers have been racing to find out if Omicron presents as big a risk of long Covid as previously dominant variants. The study from King's is believed to be the first academic research to show Omicron does not present as great a risk of long Covid, but that does not mean long Covid patient numbers are dropping, the team said. While the risk of long Covid was lower during Omicron, more people were infected, so the absolute number now suffering is higher. "It's good news, but please don't decommission any of your long Covid services," lead researcher Dr Claire Steves said, appealing to health-service providers.

The Department of Health and Social Care (DHSC) has introduced a new prescription pre-payment certificate (PPC) for Hormone Replacement Therapy (HRT) which is said to benefit around 400,000 women who suffer from negative symptoms of the menopause in England. PPC will be rolled-out from 1 April and it reduce the HRT costs to less than £20 a year. Women prescribed HRT - the main treatment for menopause symptoms - will have access to a new scheme enabling access to a year's worth of menopause prescription items for the cost of two single prescription charges (currently £18.70). As part of commitments to reduce the cost of HRT for menopausal women, the prescription PPC will be valid for 12 months. It can be used against a list of HRT prescription items, and a patient can use this against an unlimited number of HRT items, such as patches, tablets and topical preparations. There will be no limit to how many times the certificate can be used while it is valid. "The introduction of the certificate delivers one of our year one priorities for the Women's Health Strategy for England. Published last summer, the strategy sets out an ambitious new agenda for improving the health and wellbeing of women and girls and to improve how the health and care system listens to women. Menopause was announced as a priority area within the strategy," said DHSC.
