Using innovation and technology is the best way for pharmacies to boost income and ensure efficiency in services, said tech-savvy independent prescriber and community pharmacy contractor Amish Patel. Patel has invested heavily in technological refurbishment at his pharmacy and installed the first dispensing robot of its type in the UK. Highlighting the downward trend in pharmacies' income, he said: "We know for the last five years plus, pharmacy income, pharmacy services - they have just been reducing… "We are still very heavily dependent on dispensing, and dispensing fees, and profit margins with the supply chain they have been ever reducing. "This (using technology) has been a new way of generating new income. But again, just to highlight it is just enjoyment of pushing the boundaries what I can do personally."

Increased demand and shortages of lateral flow tests are pushing staff at pharmacies to bear the brunt from frustrated customers who are unable to get their free lateral flow tests, Daily Mail reported. Some Britons claimed that they have been unable to get lateral flow tests at their local pharmacies for up to a fortnight while some pharmacies were forced to put up sign warning stating they have run out of free tests. Given the patchy supply of test kits, pharmacy bosses warned that staff are facing 'unfair' and 'abusive' behaviour by customers, the newspaper reported.Dr Leyla Hannbeck, chief executive of the Association of Independent Multiple Pharmacies (AIMP), told the newspaper: "Up until yesterday the supply to pharmacies was still patchy. The past few days the situation has not improved. "Pharmacies are having to put up with a lot of abuse and aggressive behaviours by members of the public because of this situation which is unfair. "The demand is still very high now because the governments guidelines put a lot of emphasis on testing as a key out of self isolation, so naturally as Omicron cases are high people are reliant on tests to get on with daily life."

DE Group, one of the UK's leading independent pharmaceutical wholesalers, has gone into administration. Pharmacy Business has learnt that the group was put into administration on Monday (May 9), putting hundreds of jobs at risk and stoking fears of a "sudden shock" to pharmaceutical supply chain. A community pharmacy contractor who has done business with the wholesaler for over 20 years told Pharmacy Business: "I fear for the worst. I don't think there will be enough supply of medicines to cope up with a surge at such a short notice." Bharat Shah, founder and CEO of Sigma Pharmaceuticals, another leading short-line wholesaler, said the news will send shock-waves and have "a massive impact on the market" because DE Group enjoyed a "substantial coverage and market share" in the independent pharmacy sector. "It won't be easy for anyone to take on the extra volume as it could jeopardise their own business," he said, adding that Sigma was actively looking into expanding its own operations in the foreseeable future.

Community pharmacy bodies have said that the current crippling financial pressures and severe staff shortages will limit the sector from fully supporting the new health secretary's ambition to improve patient care in England. Thérèse Coffey announced her plans on Thursday (September 22) which would be looking to reduce the country's reliance on general practice by expanding the range of services available from community pharmacies and allowing pharmacists more "prescribing powers". "Pharmacists will be able to manage and supply more medicines, without a prescription from a GP. We will look to go further on enabling pharmacists with more prescribing powers and making more simple diagnostic tests available in community pharmacy," she said in her foreword to Our Plan for Patients. However, the National Pharmacy Association has lamented that the plan stops short of promising any fresh funding for community pharmacies to deliver patient care and develop clinical services. NPA vice-chair, Nick Kaye, said: "The life is being choked out of independent pharmacy businesses by the continuation of a fundamentally under-resourced contract in England.

Phoenix UK has partnered with 2San Global Ltd, a diagnostic and digital tools company, to supply home testing kits to its 5,200 independent pharmacy members through Numark and selected Rowlands Pharmacy branches nationwide. Both Numark and Rowlands Pharmacy are owned by Phoenix. Numark will be the first UK wholesaler to supply 2San's home tests, granting members swift access to these screening products for essential health insights in minutes, all at reasonable prices, the company said in a statement. Numark will offer 10 essential tests to its 5,200 independent pharmacy members from August, while Rowlands Pharmacy will provide a selection of tests in over 380 branches beginning September. This collaboration aligns with Phoenix's goal of improving diagnostic accessibility and easing strain on the NHS.

Health and Social Care Secretary Sajid Javid and Madelaine McTernan, head of the HRT supply taskforce, continue to take urgent action to resolve the shortage of HRT medicines by meeting the drug manufacturers and representatives from community pharmacies on Thursday (May 5). In the meeting, manufacturers outlined the steps they're taking to boost supply, and pharmacists shared their experiences on the frontline, as well as sharing their thoughts on wider solutions including improved communications. Aspen Pharmacare, Besins-Healthcare, Gedeon Richter, Novartis, Novo Nordisk, Orion, Pfizer, Theramex, Viatris, and representatives from community pharmacies were part of the meeting. As the government confirmed its intention to work with industry to do what is necessary to fix the HRT supply issue, Javis said he wanted to understand the issues facing suppliers and what can be done to address them. "We will leave no stone unturned in our national mission to boost supply of HRT. Along with appointing Madelaine McTernan as head of the HRT supply taskforce to implement lessons learned from the pandemic, and ensuring prescriptions are issued in shorter cycles for now, we are working collectively with the sector to urgently resolve this issue," he commented. The Association of Independent Multiple Pharmacies (AIMp) said the meeting discussed 'why we got into this position' and the way forward.

The recent announcement that Royal Mail will be partnering with distance selling pharmacy (DSP) giant pharmacy2U highlights how standards of regulatory enforcement are being ignored to accommodate the DSP model. The brunt of these double standards hinges around the levelling down of temperature enforcement standards by the Medicines and Healthcare products Regulatory Agency (MHRA) which demands mapping must be audited from the point of dispensing to the patient. The MHRA has a well recognised duty to ensure medicines reach patients in a safe condition. The current anomaly appears to turn a blind eye to this step in the supply chain at the point the wholesaler releases goods to the pharmacy hub. Equally the training on delivering medicines safely and effectively direct to patients should apply fully to all hubs including DSPs. Why is it that DSPs are being treated differently to bricks and mortar pharmacies? It's essentially the same patients receiving the same medicines from the same wholesalers. A further regulatory disparity exists around how parcels must be "tracked and signed for" to be reasonably certain medicines are delivered into the hand of the intended recipient, as per existing regulations. Clearly an untracked, unsigned package cannot be guaranteed to finish in the hands of the intended recipient. There is a very real possibility that such omission could lead to community pharmacy closures which will, in turn, lead to unemployment and a reduction in the care services. At a time when integrated care systems have just gone live, the removal of vital support services leading to further inequalities is the wrong message for both providers and patients alike.

In a recent meeting with the head of the government's HRT Taskforce, Madelaine McTernan, the National Pharmacy Association (NPA) board members gave a gist on the ongoing medicines supply issues in community pharmacy. The meeting, which explored possible solutions to end the nationwide shortage of some Hormone Replacement Therapies, took place on Friday (May 20). Independent contractors Reena Barai and Olivier Picard gave the so-called 'HRT tsar' an overview of the medicines supply issues in community pharmacy and discussed the new Serious Shortage Protocols (SSPs) which the government introduced in response to the on-going HRT disruption. Earlier this month, the NPA attended an HRT summit organised by the Department of Health and Social Care, alongside other pharmacy bodies, wholesalers and manufacturers. Health secretary Sajid Javid and pharmacy minister Maria Caulfield said they would be working collaboratively with manufacturers to meet demand and boost supply.

A Company Chemists' Association (CCA) analysis has shown that 'Pharmacy First' service in England could free up 30m+ GP appointments each year. It has urged the Government and NHS to be even 'bolder in their ambition and go further and faster'. The recent 'Delivery plan for recovering access to primary care' announced a 'Pharmacy First' service for England, mirroring similar approaches in Scotland and Wales. The association has estimated that with the added capability to supply non-prescription medicines and prescribe additional prescription-only medicines, an ambitious 'Pharmacy First' service could free up 30m+ GP appointments annually. Harnessing community pharmacies to deliver care for minor health conditions will effectively create 11,000 urgent care centres in England.

A full CPE Committee Meeting was organised in London on April 17 and 18, 2024, to discuss crucial sectoral issues. Key topics discussed during the two-day meeting included intolerable pressures on pharmacy owners, the ongoing Community Pharmacy Contractual Framework (CPCF) negotiations, implementation of the Pharmacy First service, and governance changes. Committee Members reviewed the progress of CPCF negotiations with the Department of Health and Social Care (DHSC) and NHS England. These critical negotiations are being led by the Negotiating Team (NT), which includes independent pharmacy owners and representatives of CCA and non-CCA multiples. The CPE has been advocating for an uplift to the core global sum, margin write-offs, an agreed mechanism for regular funding increases linked to activity and inflation, annual uplifts to service fees, more fundamental reform of the margin delivery framework and an economic review of the medicines supply chain. The Committee also discussed the results of the 2024 Pharmacy Pressures Survey, which was conducted during March and April.

The National Pharmacy Association (NPA), representing independent community pharmacies, has called for tougher regulation of the online sale of weight loss jabs to protect patients amid a prediction of a new year's boom in demand. The association argues that current regulation "leaves the door open for medicines to be supplied without appropriate patient consultation and access to patient records." It has urged the General Pharmaceutical Council (GPhC) to require greater consultation with patients before dispensing weight loss jabs and other high-risk medication online. The association stressed that online sellers should not just rely on information provided in online questionnaires but also gather important historical medical information before a full two-way consultation between prescriber and patient. Draft guidelines on weight loss medication The GPhC proposed changes to their current guidance for online pharmacies to improve patient safety. It launched a consultation on the revised guidance on 17 September, inviting feedback until 9 October 2024 from the public, carers, pharmacists, pharmacy technicians, other healthcare professionals, and pharmacy owners. In its response to proposed guidelines on prescribing weight loss medication, the NPA highlighted critical gaps, stating that proposed safeguards "still leaves the door open for medicines to be prescribed/supplied without appropriate two-way direct patient consultation and access to patient records for a full clinical picture particularly where high-risk medicines are involved, and the risk to patient safety remains".

Ahead of the launch of UK's first state-of-the-art automated hub for independent community pharmacists, HubRx has announced the appointment of Karl Wooldridge as operations director and Matthew Tollefson as chief financial officer. Karl brings more than 14 years of sector experience to his new role with HubRx. Having begun his career as an independent community pharmacist, Karl has gone on to hold key roles within pharmacy automation, including managing Phoenix Medical Supplies' offsite dispensing service. Commenting on his appointment, Karl said: "I'm delighted to have joined the team. The pandemic has further highlighted the important role pharmacists can play in keeping the nation healthy, not least in the delivery of the latest vaccination programme. The Department of Health and Social Care (DHSC) has published its response to the 2016 hub and spoke consultation which has paved the way for a new discussion following engagement with community pharmacies and other stakeholders this year. The response showed that many respondents supported use of hub and spoke dispensing models in independent pharmacies, but expressed concerns about some practicalities related to it. Respondents raised concerns around "patient data, liability, implications for the supply chain, and risks to the existence of small community pharmacies." They also raised question about the speed of access to medicines in an emergency and security of data transfers between hubs and spokes, DHSC stated.

The National Pharmacy Association (NPA) has emphasised that the community pharmacy sector must be involved right from the start in all local planning meetings pertaining to the roll-out of vaccine services in potential future pandemics. This includes crucial discussions regarding supply logistics and resource allocation, lawyers representing the NPA told COVID-19 public inquiry hearing. Chaired by Baroness Heather Hallett, the independent public inquiry is examining the UK's response to the Covid-19 pandemic and lessons for the future. The NPA has been designated as a 'core participant' in Module 4 (vaccines) of the inquiry, which considers various issues related to vaccine development and rollout, as well as Module 3 (healthcare). During the hearing on September 13, NPA lawyers outlined three key areas the association says must be improved in future pandemic vaccination programs, including properly utilising existing primary care expertise, better planning and engagement, and improved access to information for community pharmacists.

The General Pharmaceutical Council (GPhC) has published updated guidance on online pharmacy services in Great Britain, introducing additional safeguards to prevent unsafe supply of high-risk medicines, including weight loss injections. Under the new rules, medicines used for weight-management and medicines requiring physical examination before a prescribing decision is made have been added to the list of high-risk medicines. When prescribing these medicines, prescribers can no longer rely solely on online questionnaires. Instead, they must independently verify patient information, either through timely two-way communication with the person, accessing the person's clinical records, or contacting the person's GP, their regular prescriber, or a third-party provider. These measures aim to prevent individuals from providing false information to obtain medicines that may not be clinically appropriate for them, reducing potential harm. The pharmacy regulator updated its guidance in response to concerns identified relating to unsafe prescribing and supply of medicines online.

A report released by the Independent Commission has blamed Brexit supply issues for medicine shortages. NHS is forced to pay extortionate prices to fulfil the demand for vital antibiotics, anti-depressants, Attention Deficit Hyperactivity Disorder (ADHD), and Hormone Replacement Therapy (HRT) drugs. The report highlights the impact of affected medicine supply issues on community pharmacies and patients. Janet Morrison, the chief executive of Community Pharmacy England, backed the report and said that the "medicine shortages and market instability appear to be as bad as they have ever been". She also explained how the Ukraine conflict, the COVID-19 pandemic, and broader economic instability also play a major factor in the situation.

Pharmacists in the UK are reporting significant shortages of a common antibiotic used to treat whooping cough (pertussis), amid rising cases of this bacterial infection. Dr. Leyla Hannbeck, chief executive of the Independent Pharmacies Association (IPA), told BBC Radio 4's Today programme that clarithromycin, one of the key antibiotics for treating whooping cough in children, is "completely out of stock." Clarithromycin is an antibiotic commonly recommended for babies and infants under the age of 1 month. Dr. Leyla said that as pharmacies cannot get the antibiotic, they are being forced to turn patients away or send them back to their doctors to have their prescriptions changed to azithromycin or other antibiotics for treating whooping cough.

Recognising the changing practices in healthcare, the Royal Pharmaceutical Society (RPS) has revised and updated its position on the separation of prescribing and dispensing or administration of a medicine by the same healthcare professional. It's previous guidance, published in March 2020, restricted prescribing and dispensing by the same healthcare professional only in exceptional circumstances. The new guidance, jointly developed with the Royal College of Nursing (RCN), has made prescribing and dispensing of medicines more flexible. It clarifies that "where there is a risk assessment in place and in the best interests of the patient, the same healthcare professional can be responsible for the prescribing and dispensing / supply / administration of medicines." The change in the position statement follows an extensive year-long engagement exercise with medical associations, members, fellows, expert advisory groups, patient groups and the RPS Country Boards.
