The Pharmacists' Defence Association (PDA) is supporting pharmacists in England whose employment has been impacted by the creation of new NHS structures. As a result of the 2022 Health and Care Act, Integrated Care Systems (ICSs) were established in England, and Clinical Commissioning Groups (CCGs) were closed on 1 July 2022. Each of the ICSs includes two key parts: an Integrated Care Board (ICB) and an Integrated Care Partnership (ICP). The association said: "The previous structures in England, established by the Health and Social Care Act 2012, originally had over 200 clinical commissioning groups. This number had reduced over recent years via mergers." Although each ICS and its activity can be subdivided into 'places', and within places into 'neighbourhoods', it is at the ICS level that the employer is formed." NHS England is encouraging people to get their flu and Covid vaccines as hospitals dealt with the highest number of norovirus and flu cases this winter. An average of 688 patients with norovirus were in hospital each day last week, 75 per cent higher than the same time last year (393), according to the latest data from NHS England. The data also showed an average of 2,226 patients with flu in hospital each day last week, which is three-quarters higher than the same week last January (1,290). Compared to last month, there has been an 80 per cent increase in the daily count of norovirus cases and 70 per cent in flu cases. In the week ending Dec 31, NHSE recorded 1,312 hospital admissions due to norovirus and 376 and 1,312 flu cases each day.

Pharmacies are valuable healthcare assets, meeting increasing public demand. However, a substantial portion of their capacity is consumed by unfunded work. According to Community Pharmacy England's (CPE) 2024 Pharmacy Advice Audit, community pharmacies conduct over 1.3 million unfunded consultations each week, equating to 69 million annually. The average pharmacy completes around 22 of these consultations daily, each taking approximately 5.6 minutes of staff time -adding up to over two hours daily. This represents nearly 50 per cent more consultations than in 2020 and a 78 per cent increase in time spent on unfunded healthcare advice since the 2020 audit. The audit, which surveyed over a third (3,916) of community pharmacies in England earlier this year, also highlights that pharmacies handle nearly 150,000 informal referrals from GPs and NHS 111 each week. All these referrals could have been routed through the Pharmacy First service, which would have potentially secured over £115 million in funding for the sector, the CPE noted.

A new report from the Company Chemists' Association (CCA) is calling on NHS England to commission a community pharmacy-based national Type 2 diabetes screening service, which could save the NHS £50m each year. The CCA report, titled 'Increasing Access to Diabetes Screening and Prevention Through Community Pharmacy', highlights the urgent need for a national patient pathway for diabetes detection and management. It outlines how NHS-commissioned diabetes screening in community pharmacies across England could deliver numerous health benefits each year, including: Screening 1.5 million adults and identifying 180,000 prediabetics; Detecting 45,000 undiagnosed cases of diabetes and preventing them from developing serious complications Preventing nearly 7,000 heart attacks and strokes, and thus improving patients' quality of life Avoiding severe sight loss in around 15,000 people over their lifetime Saving the NHS £50 million annually by reducing diabetes-related costs. Type 2 diabetes and its complications are currently estimated to account for 10 per cent of the NHS annual budget, and this figure is projected to rise to 17 per cent by 2035/36.

Top Tips by PSNC for pharmacists on HRT SSPs: Always double check that endorsements reflect the requirements outlined in the supporting guidance published for each SSP on the dedicated page of the NHSBSA's website. When endorsing using EPS, contractors are reminded to select the SSP endorsement and input the correct three-digit reference number leaving a space in between 'SSP' and the three-digit reference number for example, SSP 019. Remember to insert the leading zero in the three digit reference number. Each of the affected HRT medicines has its own SSP reference number. Check that the correct SSP number is endorsed for e.g. for Oestrogel® Pump-Pack 0.06% gel the SSP endorsement should be 'SSP 019'. Please note the 'NCSO' endorsement is no longer accepted for SSPs. The Department of Health and Social Care (DHSC) has introduced new arrangements for the application of discount deduction to community pharmacy payments which will be implemented in six financial quarters beginning in October 2022 and concluding in January 2024. Under the new discount deduction system, the current single scale will be split into three groups: one each for generic medicines, branded medicines, and appliances. Separate fixed deduction rates have been determined for each group. This will provide all pharmacies the same rates of deduction applied to their reimbursement for the three different groups, regardless of the total value of that reimbursement. Fin McCaul, PSNC member and independent community pharmacy contractor, said: "The discount deduction scale has been a point of contention for contractors for many years, and PSNC has long been pushing to remedy this. "The incoming changes are designed to both improve equity of access to margin and manage the distortions presented by branded medicines, which just don't have the same level of discount available as generics.

Frontline doctors have sounded the alarm over the rising pressure on emergency departments in hospitals across the country as winter virus cases are "creeping up". An average of 234 people were in hospital with flu every day last week, a 53 per cent increase compared to the week before, according to new NHS data. Hospital norovirus cases increased by 15 per cent last week, with an average 406 people hospitalised each day, a 28 per cent increase from the same week last year. Measures to stop the spread of norovirus to other patients led to the closure of an average of 92 beds were closed each day last week.

Southwark Crown Court on Thursday sentenced two pharmacists to two years of imprisonment each, suspended for 24 months, after they were found guilty of selling 'industrial' quantities of Class C controlled drugs. Mandip Sidhu (47) of Littleover, Derby and Nabeil Nasr (42) of Cheadle, Greater Manchester, pleaded guilty at an earlier hearing following an investigation by the Criminal Enforcement Unit of the Medicines and Healthcare Products Regulatory Authority (MHRA). Both Sidhu and Nasr were pharmacists registered with the General Pharmaceutical Council at the time of the offenses, which took place between May 2013 and June 2017. Sidhu was the director of Pharmaceutical Health Limited (PHL) in Derby, while Nasr owned several pharmacies across the North West of England. Sidhu was sentenced to two years imprisonment on each of five counts of supplying Class C drugs and four months for forgery, all to run concurrently and suspended for 24 months. Additionally, she must complete 200 hours of community service for her role in the illegal supply of diazepam, zolpidem, and zopiclone.

Many community pharmacies are under severe financial strain due to chronic underfunding and a mismatch between medicine costs and reimbursement rates. A report by the National Pharmacy Association (NPA) revealed that NHS underpays pharmacies by up to £75 a pack for common medicines, resulting in losses of thousands of pounds each month. The NPA analysis found that, in many instances, NHS funding covers only one-fifth of their purchase costs for medicines. Nemesh Patel, Managing Director of the Southdowns Pharmacy Group, said the current pharmacy funding contract is "completely broken and beyond farcical." Sharing their struggles, he tells Pharmacy Business: "Our teams are wasting hours and hours trying to just source medication for patients when they could be better using that time to clinically support our patients, and then when it comes to reimbursement, the drug tariff prices or concessionary prices are structured such that on hundreds of medications we dispense per month, we will be making a significant financial loss, each and every single time our pharmacies dispense that medication."

NHS England has launched the first major update to the 'Act FAST' campaign since 2009, as new data reveals that many stroke patients wait almost an hour and a half on average before making an emergency call. The updated campaign urges the public to call 999 immediately if anyone experiences one of the three common stroke symptoms: difficulty smiling (Face), trouble raising an arm (Arms), or slurred speech (Speech). Around 100,000 people are estimated to suffer a stroke each year in the UK, which occurs when the blood supply to part of the brain is cut off. Without prompt treatment, a stroke can lead to death or long-term disabilities such as paralysis, memory loss and communication problems. Strokes remain the fourth single leading cause of death in the UK, resulting in about 38,000 deaths each year. An analysis of NHS data for 2023-24 indicated that among 41,327 patients with a recorded time of symptom onset, the average time from the first symptom to making a 999 call was 88 minutes. Additionally, a poll of 2,001 adults in England revealed that 57 per cent of respondents believed you should have 2-3 symptoms of a stroke before calling 999, despite the fact that even one symptom is a sign of a medical emergency.
