The Royal Pharmaceutical Society (RPS) has launched a new position paper on 'How pharmacy can do more to tackle health inequalities'. The RPS paper aims to help pharmacists and pharmacy teams to take action against health inequalities. It is designed to provide a reference source for pharmacy teams to use over time to continually improve services. RPS said: "Like all health services, these pharmacy services do not always reach everyone. Often it is people who are suffering the greatest inequalities that find it most difficult to access the services that are currently available. This can be for a large number of reasons including poverty, education, being part of a group vulnerable to exclusion, language and power." Therefore, the RPS paper focuses not on "what" services pharmacy provides but "how" they are provided, with an aim of making them more accessible for people who are currently struggling. Claire Anderson, President of Royal Pharmaceutical Society said: "It's fantastic that pharmacy teams in every sector of pharmacy across Great Britain are providing care that can improve health inequalities, with many patients able to access pharmacy services very easily. The drug, alpelisib, which is the 100th cancer drug that has being fast-tracked to patients under the NHS Cancer Drugs Fund (CDF) will be used in combination with the hormone therapy, fulvestrant, to target the gene that causes fast-growing tumours. Up to 3,000 people a year with a certain type of secondary breast cancer will benefit from the treatment. The approach has contributed to people in England having access to nearly one third more cancer drugs compared to the European average. The drug which is manufactured by pharmaceutical company Novartis, is part of a growing number of precision treatments that target a tumour based on mutations in its DNA and that the NHS is rolling out. An alliance of companies has pledged to ensure equitable access to vaccines and treatments for pandemics, as the friction around intellectual property rights for Covid-19 interventions between the pharmaceutical industry and developing nations endures. At the heart of the plan is a commitment to set aside part of the production of vaccines and treatments upfront for vulnerable populations in low-income countries when the next pandemic arises, given how fragmented access to Covid tools has left many populations unprotected. In order to do better next time - and without knowing which companies will develop the first drugs and vaccines for the next pandemic - having the industry collectively make this commitment is potentially transformative, said Thomas Cueni, head of the International Federation of Pharmaceutical Manufacturers & Associations (IFPMA). The pledge, called the Berlin Declaration, was made on July 19 by members of the global pharmaceutical industry group that include many of the companies involved in developing Covid interventions, such as AstraZeneca, GSK, Moderna, Pfizer and Merck. Britain's health regulator is poised to announce that hormone replacement therapy (HRT) will be made available over the counter for the first time, the Daily Telegraph has revealed. The newspaper first reported on Tuesday (February 1) that watchdogs were set to propose a reclassification of the medication so women are able to access it in a pharmacy without a prescription. Every year, some 1.5 million women experience difficult menopausal symptoms, such as hot flushes, night sweats, sleep disturbance and a variety of emotional problems. But only a tenth of them are prescribed HRT, following a consultation with a GP or specialist. According to the newspaper, the latest proposal aims to improve convenience so that women are able to access HRT more easily, without needing a GP appointment.

Cutting the red tape that blocks pharmacists to alter the HRT prescription could 'quickly fix' the problem of women unable to access their HRT medicines, the Royal Pharmaceutical Society (RPS) told Daily Mail. RPS has suggested that allowing pharmacists to prescribe alternative HRT treatment amid nationwide shortage of HRT medicine could help the women who are struggling to access these medicines. Thorrun Govind, chair of the English Pharmacy Board, told MailOnline 'changes in prescription rules need to be changed urgently.' She added, 'For the pharmacists on the ground, they need the ability to get rid of this bureaucracy. When you think about it - who's best able to offer an alternative - that tends to be the pharmacist.' MailOnline quoted Professor Claire Anderson, president of the Royal Pharmaceutical Society, as said, 'We welcome the Health Secretary meeting with key suppliers and manufacturers to find solutions to the current shortages. But cutting the red tape holding pharmacists back could provide a 'quick fix' that would allow 'women to access their HRT medicines more speedily'.

The Department of Health and Social Care (DHSC) launched a new Innovative Medicines Fund on Tuesday (June 7) under which £340 million has been made available to purchase potentially life-saving drugs early. This will allow NHS patients in England to have early access to potentially life-saving and cutting-edge treatments Health secretary Sajid Javid said: "I want NHS patients to be the first in the world to access the most promising and revolutionary treatments that could extend or save their lives. "The launch of the Innovative Medicines Fund delivers another manifesto pledge and will fast-track cutting-edge medicines to adults and children to give people renewed hope for a better future." A total of £680 million has been ringfenced for the Innovative Medicines Fund and Cancer Drugs Fund - £340 million each - to fast-track medicines to NHS patients. DHSC said: "The Innovative Medicines Fund will provide quick access to novel treatments, including potentially lifesaving gene therapies for serious conditions with few treatment options. It often takes longer for pharmaceutical companies to collect data on a medicine's clinical and cost effectiveness for rare diseases due to the smaller patient cohort.

The Pharmacy Supervision Practice Group held its second meeting to discuss the future modelling of pharmacy practice. Some of the leading bodies from the community pharmacy sector have joined the group. The members explored themes centred around accountability, delegation and responsibility. The session included a wide-reaching discussion, including what future guidance might look like and the continued patient access to the pharmacist through a comprehensive, accessible and equitable community pharmacy offer. Chair of the group, Dr Michael Twigg, Associate Professor of Primary Care Pharmacy, University of East Anglia, said: "The primary objective of the group is to develop a co-designed and co-produced solution that will not only enhance the patient experience within community pharmacy practice, but that will enable the community pharmacy workforce to maximise role and professional skills. "At its second meeting, the group was unanimous that any changes in the delegation, responsibility and accountability space should be in the patient interest, distil confidence for patients and ensure that community pharmacy remains accessible and equitable."

Superdrug has unveiled its new fertility test service to help women to easily access information about their own fertility and enables them to take greater control of their reproductive health. The service comprises a Superdrug online doctor consultation and personalised fertility test recommendations to help women make more informed decisions about the options available to them. Poverty not only takes a significant toll on people's health but also leads to additional costs for the National Health Service (NHS). Rise in deep poverty, cost-of-living crisis, and high pressure on NHS services have worsened the situation, according to a study published by The King's Fund this week. The report underscored that poverty and deprivation contribute to a greater prevalence of diseases, difficulties in accessing health care, late or delayed treatment, and worse health outcomes. These challenges could be seen across various NHS services, spanning from emergency care to dental services Additionally, it revealed that 30 per cent of people living in the most deprived areas have turned to 999, 111, A&E or a walk-in centre because they could not access a GP appointment. In 2016, the Joseph Rowntree Foundation (JRF) estimated the cost of poverty on health care at £29 billion (£34 billion in current prices). As the proportion of people living in deep poverty has risen, the situation has worsened. In 2021/22, six million people were living in very deep poverty, up from 4.5 million two decades ago. Currently, more than one in five people in the UK are estimated to be living in poverty, the report noted. Deprivation is linked to a range of diet-related health problems, including cardiovascular disease and diabetes, as well as mental illness. According to the report, the depression rate is two times higher among people living in the most deprived areas, compared to the least deprived areas.

The NHS is on track to miss out on savings of over a billion pounds as patents for a new lineup of 85 biologics are set to expire within the next five years, the British Generic Manufacturers Association has revealed in its new study. The government's Voluntary Scheme for Branded Medicines Pricing and Access is expected to lead to a cost of more than £1 billion for the NHS in the coming years. The BGMA research found that more than 85 biological medicines will experience loss of exclusivity during the upcoming VPAS Scheme period from 2024 to 2028. "This includes blockbuster products like the cancer medicine Keytruda and wet macular product Eylea, which together generate approximately $25 billion in global sales," BGMA said. "The molecules coming off-patent also cover other disease areas including oncology, diabetes, arthritis, and asthma." While biological medicines dominate the medicines budget, constituting the largest cost and cost growth sectors, NHS England aims to expedite biosimilar availability, yielding substantial savings and expanding patient access to vital treatments. Yet, the report found that "this is jeopardised by the influence of the VPAS Scheme".

The British Generic Medicines Association (BGMA) has called for exemption from the Voluntary Scheme for Branded Medicines Pricing and Access (VPAS), an agreement between the UK Government, NHS England, and the pharmaceutical industry for branded generics and biosimilars. "Due to the rising rate of VPAS on top of existing competition, manufacturers are finding the additional levy economically unviable given their already low prices," the association said. According to research by the Office of Health Economics OHE and Professor Alistair McGuire (LSE) the rising rate of VPAS will force manufacturers pull out of the market which may lead to prices rise due to a lack of competition and critical savings to the NHS will be lost. The new study stated that Government levy on medicines designed to increase access to new treatments and promote affordability could actually be denying the NHS billions of pounds of annual savings due to the impact it is having on branded generics and biosimilars.

The NHS Confederation and the Association of the British Pharmaceutical Industry (ABPI) has published a report that demonstrated the importance of the collaboration between industry and the NHS to improve patient outcomes across the UK. Analysis from the report has shown a significant variation in access to innovative medicines for patients across the UK and showed that uptake of new treatments in these areas continues to be below the average of similar countries in Europe. The report, 'Transforming Lives, Improving Health Outcomes', has highlighted four initiatives where effective partnerships between the NHS, patient organisations and industry have helped to tackle unwarranted variation in the uptake of innovative medicines. Transforming Lives, Improving Health Outcomes also called for a systemwide secondary prevention strategy covering all parts of the health system creating a barrier to wider and consistent uptake of innovative medicine. It also noted that newly created Integrated Care Systems have the potential to improve preventative treatment. The report data also showed a 51% variation of uptake of three types of medicines related to diabetes between NHS Trusts in England.

In its response to the Health and Social Care Committee inquiry into 'Digital Transformation in the NHS', the Community Pharmacy IT Group (CP ITG) has highlighted issues concerning the NHS community pharmacy. CP ITG, of which PSNC is a member, has urged that pharmacy teams should have access to up-to-date records information, e.g. from Shared Care Records, GP Connect and Summary Care Records with Additional Information. It has also stated that the Booking and Referral Standards (BaRS) should be extended so that future referrals into and from pharmacy can be communicated seamlessly. In its written response, CP ITG said: "Ensure that BaRS is expanded and aligns with other referral IT standards such as GP Connect referrals. Ensure it is used across community pharmacy, the GP sector, other health care sectors and across the NHS, and by all health IT suppliers, so that messages and referrals can flow smoothly within and across sectors and patients can have a seamless experience and safe care." The group has asked the Health and Social Care Committee to create a framework for the development of pharmacy IT to ensure IT suppliers are better equipped to support pharmacy teams and their delivery of services.

The Department of Health and Social Care has updated the list of community pharmacies eligible for the Pharmacy Access Scheme (PhAS) and approved 43 cases out of 63 applications, bringing the the total number of eligible pharmacies in England to 1,445. Earlier this year, NHS England and NHS Improvement (NHSE&I) invited community pharmacy contractors to apply for a review if they believed there were any inaccuracies in relation to pharmacy premises addresses or unforeseen circumstances affecting access, such as a permanent roadblock. Submitted applications were reviewed by the relevant NHSE&I regional pharmacy contract team, and determined by the relevant pharmaceutical services regulations committees (PSRC). Due to the workload pressures seen over winter, the review deadline was extended, giving contractors applying for a review two full months to complete their applications. This also pushed back the announcement of the outcome of the review, but where an application has been successful, PhAS payments will be backdated to the start of the scheme.

Acknowledging the ongoing challenges in delivering the required IT systems for the scheduled launch of the Pharmacy First service on January 31, NHS England reported progress in fulfilling 'many' commitments outlined in its plan to enhance primary care access during a board meeting on Thursday. However, it noted that the digital infrastructure supporting Pharmacy First, included in the £645 million fresh funding for community pharmacy, will require additional time for implementation. "Delivery remains challenging due to changes needed in digital infrastructure, and we are working with suppliers to support January implementation," NHSE said. "A public campaign is planned for 2024." The Pharmacy First service allows pharmacists to supply prescription-only medicines for seven common health conditions (sinusitis, sore throat, earache, infected insect bite, impetigo, shingles, and uncomplicated urinary tract infections in women), eliminating the need for a GP visit. Meanwhile, NHSE is leveraging the widespread accessibility of pharmacies by expanding blood pressure monitoring and introducing direct prescribing of clinically safe oral contraceptives-an initiative that has been well-received.
