Paydens Pharmacy Group hub and spoke model - 0 views

    Paydens Pharmacy Group, one of the first multiple pharmacies to adopt hub and spoke model last year, revealed that the move has resulted in significant time savings in store. Centralising dispensing of repeat prescriptions relieves pressure in pharmacy branches and releases time to deliver revenue generating services, the group said. The group created a hub in Maidstone, Kent, to service its highest dispensing branches. The hub uses Centred Solution's FLOWRx Hub Auto to dispense their original pack repeat prescriptions. The group put in the first FLOWRx Hub production line in spring 2022 and then added in a second line in the autumn as they increased production and moved more stores to the hub and spoke model. The innovative solution interfaces with EMIS PMR, Omnicell's Robotic Dispensing System and Universal Logistics Management software as well as Victoria OS ordering software. The hub operation runs during the day Monday to Friday and produces an average of 33,289 packs per week for just under 9,000 patients, leaving plenty of scope to ramp up. The hub is currently dispensing 79.7% of repeat prescription original packs requested by the group's busiest stores.

Pharmacists must change focus from accuracy to safety - 0 views

    Professor Harry McQuillan has said community pharmacists in the UK must change their focus from an 'accuracy of supply' to a 'safety of supply' model when it comes to dealing with patients. "The main take away message from tonight's lecture is for pharmacy professionals to really challenge themselves about whether they are focused on accuracy of supply or safety of supply," he said at this year's UCL School of Pharmacy Lecture in London on Thursday, 15 June. "For our community pharmacists it must be about safety, including prescribing, and ensuring patients and citizens get the maximum benefit from prescribed medicines. "To deliver this, we need to invest in our teams - harness technology and always be willing to take the next step in a more clinical future." The Chief Executive Office of Community Pharmacy Scotland, however, noted that at a time when the UK was grappling with an unprecedented national debt crisis exacerbated by severe cost of living and spiraling inflation, community pharmacy will need to make a "compelling case" of where the priority should be in the sector. Expanding on the financial woes of the country, he compared the current national debt of the UK, which stood at a staggering 270 percent of GDP in September 2022, with the previous record-breaking national debt of 250 per cent at the end of WWII.

    The Royal Pharmaceutical Society (RPS) Wales has welcomed proposals from the Welsh government to increase access to PrEP via community pharmacies for people at risk of HIV. The new HIV Action Plan for Wales consultation proposes a shared care model between primary care and specialist sexual health services to improve access to PrEP. Commenting on the proposal, chair of RPS in Wales Cheryl Way said: "Increasing access to the medicines has been a long-standing issue of concern for us at RPS and we're therefore delighted to see the proposals outlined by the Welsh government to develop a model which will enable PrEP to be provided by community pharmacies across Wales. "PrEP are highly effective medicines, that reduce the chances of getting HIV, as proven by the Welsh Government's three year PrEPARED study which found no new diagnoses of HIV among over 1,200 people taking PrEP in Wales. This is clear evidence that PrEP should be made as easily accessible as possible to those who need it.

    The Department of Health and Social Care (DHSC) has published its response to the 2016 hub and spoke consultation which has paved the way for a new discussion following engagement with community pharmacies and other stakeholders this year. The response showed that many respondents supported use of hub and spoke dispensing models in independent pharmacies, but expressed concerns about some practicalities related to it. Respondents raised concerns around "patient data, liability, implications for the supply chain, and risks to the existence of small community pharmacies." They also raised question about the speed of access to medicines in an emergency and security of data transfers between hubs and spokes, DHSC stated.

    While responding to the consultation on hub and spoke dispensing, trade unions for pharmacists have emphasised that the patient safety and care must be the priority in hub and spoke dispensing. The Royal Pharmaceutical Society (RPS) has welcomed the opportunity provided by a change in legislation to enable community pharmacies to make use of hub and spoke dispensing but reaffirmed that patient care must be at the heart of future changes. With regards to patient safety, the Pharmaceutical Services Negotiating Committee (PSNC) considers that only Model 1 is appropriate, with manageable risks related to patient safety, and is a model that has the potential to allow the whole sector to benefit fairly. In its response to the consultation on Hub and Spoke dispensing, PSNC highlighted that Model 2 in the consultation, a hub direct to patient supply of dispensed medicines, raises patient safety issues and it cannot be supported. RPS President Professor Claire Anderson said: "In all models, patients need to continue to have access to a pharmacist at the time of supply of medicines so they have the opportunity to discuss, ask questions or raise concerns, and receive appropriate information with counselling and advice.

    The Company Chemists' Association (CCA) has called for an evidence-based approach to hub and spoke dispensing proposals, stating that it is vital to back the changes based on robust data and lived experience of pharmacy businesses. Community pharmacy contractors need to to respond to a Department of Health and Social Care initiated consultation on the hub-and-spoke dispensing models by June 8. Malcolm Harrison, CEO of the CCA, said: "I am very concerned by both the number and scale of the assumptions contained within the impact assessment, which are unlikely to come to fruition. "The impact Assessment also makes predictions on a yet un-born market, stretching 10 years into the future. Hub and spoke technologies do have the potential to enable new dispensing models in the future if new commissioning can support a change in operations. I would caution against firm predictions about likely benefits without stronger commitments to enablers of change."

Stating that a more integrated healthcare system could enable pharmacists to provide effective and more efficient care to patients, the PDA proposed a model where at least two pharmacists would be working in each community pharmacy, empowered, and enabled with two-way referral pathways, having more clinical input and full access to patient records.     The Competition and Market Authority (CMA) has suggested regulatory levers to curb "potential competition risk" from hub and spoke dispensing. In its response to the hub and spoke dispensing consultation, led by the Department for Health and Social Care (DHSC) last week, the competition watchdog said that a pro-active approach to monitoring the effect of hub and spoke dispensing and subsequent early consideration of any competition concerns was likely to be more effective and potentially less costly than any "ex-post enforcement any ex-post enforcement or unpicking of subsequently embedded competition issues". The competition watchdog has made a few suggestions to identify and remove the barriers to competition that might emerge with the new business models entering the market. It has asked the department to enable a "more level playing field". "Smaller independent pharmacies should, have improved access to automation and new dispensing models," it said.

In the statement, published on September 27, FIP backed expansion of vaccine schedules and strategies so that patients of all age groups could be vaccinated throughout their entire lifespan, from infancy to old age. A life-course approach in public health and healthcare focuses on an individual's health and well-being, including vaccination, administered throughout their entire lifespan, from infancy to old age. FIP urged governments and policymakers to eliminate regulatory barriers, enabling pharmacists to prescribe and administer all relevant vaccines throughout the life-course. Additionally, FIP recommended that policymakers develop remuneration models for pharmacies to deliver sustainable life-course vaccination services within the private and public sectors.

The Society has urged its members to fill out the survey before May 20. RPS president Claire Anderson said: "It is vital that RPS members have their say on issue that impact you in your roles. Currently only single legal entities can make use of this model of dispensing. "Our survey, which opened today, seeks your views and comments and will help to inform the RPS submission to this consultation." The consultation on the proposals to enable all community pharmacies to access hub and spoke dispensing published by the Department of Health and Social Care (DHSC) will run for three months and will close on June 8, 2022.

"Large future waves of infection that need active management to prevent detrimental pressure on the health and care sector are, at least, a realistic possibility (high confidence) or likely (medium confidence)," SPI-M-O said in a consensus statement published on Friday.

    The Royal Pharmaceutical Society (RPS) is refreshing its Professional Standards for Homecare Services and will be shared for consultation in August to allow comment and input on the updated content. The final version is expected to be completed in the autumn. The standards were originally published in 2013 and are a framework to support teams providing and commissioning homecare services. Homecare medicines services deliver medication, and any necessary associated care, direct to the patient's home with their consent. The standards help patients experience a consistent quality of homecare services, irrespective of provider, will protect them from avoidable incidents and help them get the best outcomes from their medicines. Jennifer Allen, The Chief Pharmaceutical Officer's Clinical Fellow leading the refresh at RPS said: "It has been 10 years since the first set of Homecare Standards were published, so it is important that they are reviewed and brought up to date to reflect current service design, medicine pathways and delivery models to ensure patients receive safe and effective medication supply and associated care."

    Within the last few weeks, at least six pharmacists have been subjected to interim order applications - either suspended or had significant restrictions placed upon their registration pending a full 'Fitness to Practise' hearing into serious allegations being made about their prescribing practices, the Pharmacists' Defence Association (PDA) has found. It said that there has been a "dramatic increase in action being taken by the pharmacy regulator (GPhC) against pharmacist prescribers associated with certain types of online prescribing services". It has warned pharmacist prescribers who work for online pharmacies about the specific risks associated with remote prescribing using a questionnaire-based model, typically with no direct prescriber/patient interaction. Common features that underpin the allegations include the following: Overreliance on a patient questionnaire to inform clinical decision making; No patient/prescriber interaction; Prescribing high risk medicines without adequate safeguards; Inadequate systems and processes leading to inappropriate prescribing; Very high volumes of prescriptions being authorised in short periods of time; and Prescribers/patient relationship established via an unregulated online portal.

    From over-the-counter flu tablets to crucial antibiotics and antidepressants, medicines are running scarce in UK pharmacies this year, causing concerns among patients, the government, and the wider pharma industry. Drug shortages have accelerated over the past year due to a clutch of problems including the after-effects of the pandemic on supply chains, the war in Ukraine, and soaring input costs weighing on manufacturers. More recently, a sudden spike in respiratory infections - another by-product of Covid-19 that neither pharma companies nor the government were able to predict - has deepened the crisis, with 70 commonly taken drugs out of stock in Britain as of February. The problem is not unique to the UK. In a recent survey of groups representing pharmacies in 29 European countries, three quarters said shortages were worse this winter than a year ago, with a quarter reporting more than 600 drugs in short supply. The US is also facing significant shortages of popular prescription drugs like amoxicillin and Adderall, an ADHD medication. To some extent, the current disruption is laying bare wider challenges facing the industry for several years that were only exacerbated by the pandemic. The over-reliance on foreign suppliers for most active pharmaceutical ingredients (APIs) is one of them. This model has left companies more susceptible to supply shocks, which have in turn increased in frequency and severity due to pandemic lockdowns, the war in Ukraine and other issues such as the shortage of shipping containers. Pharma's long and opaque supply chains, alongside regulatory complexities, also mean it is taking longer for drugmakers to reconfigure manufacturing and distribution in times of additional need.

    The Royal Pharmaceutical Society (RPS) Scotland the BMA's Scottish GP Committee has called for improvements to the pharmacotherapy service. The joint statement released by the organisations on Friday (13 January), has recognised that much progress has been made but states that further improvements are needed to fully maximise the benefits of the service. It highlighted the need for better use of skill mix, including more clearly defining roles and responsibilities; improved IT enablers to reduce administrative burden; and further developing a tandem model of working comprising hubs with in-practice activity The two organisations state that inadequate funding and workforce shortages are a real threat to the pharmacotherapy service. They have urge Scottish Government to reconsider its recent decision to reduce funding for training pharmacy technicians. Clare Morrison, RPS Director for Scotland, said: "In the statement, we stress that the use of pharmacists' clinical skills must be maximised.
