Researchers at Duke University, after analysing millions of patient visits to primary care physicians, found that mental health concerns are second only to musculoskeletal complaints, such as aches and pains, in everyday care. According to the study, one in nine patients sought care primarily for a mental health concern, a figure that increases to one in five for those in mid-life Published recently in Nature Mental Health, the study indicated that the prevalence of mental health concerns is nearly equal to that of respiratory and cardiovascular complaints, surpassing infections, injuries, digestive health, skin conditions, urological problems, and sensory issues. Led by Avshalom Caspi, the Edward M. Arnett Professor of Psychology and Neuroscience at Duke University, the researchers examined over 350 million primary care visits involving 4.8 million people from January 2006 to December 2019, as recorded by the Norwegian government. Caspi noted that 12 per cent of all encounters with primary care physicians were for mental health problems, i.e. more than 40 million mental health visits out of the total.

Dementia and Alzheimer's disease remained the leading cause of death in England and Wales in 2023, maintaining the pre-pandemic trend, according to the latest data from the Office for National Statistics (ONS). The figures reveal that there were 66,876 deaths with an underlying cause of dementia and Alzheimer's disease, accounting for 11.6% of all deaths registered that year. The Integrated Care Systems (ICSs) provide an opportunity to break the silo mentality in the health and care sector, National Pharmacy Association (NPA) chair Andrew Lane told MPs. While giving evidence to the Health and Social Care Select Committee on 6 December about the effectiveness of ICSs, Lane also poured cold water on newspaper reports that community pharmacists will be drafted in to break the NHS strike, as he reiterated the call for a "properly costed and funded environment." "We've been in different silos historically and ICSs are an opportunity to pull all systems together for the benefit of patients," he told committee members. Lane pointed to dementia-trained pharmacy delivery drivers and the Discharge Medicines Service as instances where the influence of community pharmacy already stretches beyond primary care into hospitals and social care. "We've seen pharmacists prescribe and get UTIs off doctors' desks, so we are starting to release capacity [in the system] and we're on that clinical journey. We welcome that, but it has to be with the right level of funding," he added.

Campaigners have submitted an open letter to the Department of Health and Social Care (DHSC), urging the Minister for Primary Care and Public Health to freeze prescription charges to keep people with long-term conditions alive and well. The campaign is led by the Prescription Charges Coalition, which represents over 50 organisations, including Royal Pharmaceutical Society (RPS) and Pharmacists' Defence Association (PDA). Currently, the prescription charge is £9.65 per item, and campaigners have asked the government to freeze it for 2024 and 2025 as people living with long-term health conditions in England are "being forced to choose between heating, eating, and taking their vital medication on a daily basis." In 2023, a study conducted by the Prescription Charges Coalition revealed that almost 10 per cent of survey participants had skipped medication in the previous year due to the cost of prescriptions. This led to increased physical and mental health problems, as well as impacted the time they took off work. Laura Cockram, Chair of the Prescription Charges Coalition and Head of Campaigns at Parkinson's UK, expressed deep concern that a further rise in the charge this year will lead to people skipping or not taking the full dose of their medication, which will affect their health and put more pressure on the already under pressure NHS.

This involves implementing a 'Community Pharmacist Prescribing Service,' to empower pharmacists with independent prescribing rights where clinically suitable. This initiative aims to alleviate the strain on GP practices and enhance service accessibility. Additionally, the manifesto proposes piloting Neighbourhood Health Centres, to integrate various healthcare services -such as family doctors, district nurses, and mental health specialists under one roof.

Lack of staffing and increased demand from patients is mounting unsustainable pressure on the NHS, putting patient safety and care at risk, a new poll of NHS leaders by the NHS Confederation revealed. NHS leaders in England warned that the service has reached a "tipping point" with nearly 88 per cent saying "the demands on their organisation are unsustainable." The survey, published ahead of the monthly performance figures for the NHS England, covered leaders across hospitals, ambulance services, mental health providers, community services, primary care and integrated care systems. It highlighted primary care, urgent and emergency care as greatest areas of concern, with record levels of demand on A&E departments and increasing bed occupancy rates.

"It is crucial that patients can access care when they need it, whether from a pharmacist or a GP, " Paul Rees, Chief Executive of the National Pharmacy Association (NPA) has said. In response to a recent survey conducted by the General Medical Council (GMC) highlighting alarming trends among General Practitioners (GPs), the NPA has issued a warning about the growing crisis in the UK's primary care system. Rees emphasised the urgent need for government intervention, saying, "Only by reversing these cuts and providing pharmacies with a new funding deal will we be able to end the 8am scramble for appointments." The GMC survey reveals that there is a significant increase in the number of doctors reducing their working hours to safeguard their wellbeing, spotlighting concerns about the long-term impact on patient care. According to the report, nearly half of GPs ( 48 per cent) are struggling to manage their workload, with several resorting to decreasing their hours or declining additional work to protect their mental and physical health.

This initiative is viewed as pivotal in recognising the clinical capabilities of pharmacists and their potential to alleviate pressures within primary care. Additionally, Labour's plan advocates for broader healthcare access improvements, including enabling opticians to directly refer patients to specialist services and expanding self-referral options where suitable. The manifesto also introduces proposals to trial Neighbourhood Health Centres, designed to consolidate various healthcare services under one roof, such as family doctors, district nurses, and mental health specialists.

Workforce issues, staff retention and capacity continues to be a big concern for the Local Pharmaceutical Committee as it discussed the topics at a recent meeting with the Pharmaceutical Services Negotiating Committee (PSNC). The meeting, attended by delegates from 66 LPCs, focused on the current challenges faced by community pharmacies as well as the planning for the next year. Around 64 per cent of delegates said workforce was the top challenge faced by pharmacy contractors, and suggested reducing the migration of pharmacists to Primary Care Networks (PCNs). Other suggestions included - "improving the development opportunities available for pharmacists and their teams, and rebranding the profession to make community pharmacy a more attractive route for young graduates." On the development of Integrated Care Systems (ICS), the majority of respondents were positive about their LPC's engagement in the new setting.
