The National Institute for Health and Care Excellence (NICE) has recommended pembrolizumab in combination with chemotherapy (paclitaxel or nab-paclitaxel) which is said to benefit people with triple negative breast cancer. Clinical trial evidence shows that, compared with paclitaxel, pembrolizumab combination increases how long people have before their cancer gets worse and how long they live. NICE already recommends atezolizumab in combination with chemotherapy for untreated PD-L1-positive, locally advanced or metastatic, triple-negative breast cancer. Atezolizumab combination is recommended by NICE for people with IC equal to or greater than 1%. "This means that some people who can't have atezolizumab combination could be eligible for pembrolizumab combination," said NICE. NICE's independent appraisal committee also agreed that the pembrolizumab combination meets the higher cost-effectiveness threshold for life-extending treatments for people with a short life expectancy. Given by injection every 3 weeks, pembrolizumab (also called Keytruda and made by Merck Sharp and Dohme) is a type of immunotherapy that specifically targets triple negative breast cancer. It works by blocking the activity of a protein known as PD-L1 which is produced in larger amounts on cancerous cells than normal cells. By blocking PD-L1 it helps the person's own immune cells to attack the cancer.

Pfizer said on Thursday (June 1) data from late-stage trials showed its experimental combination of antibiotics was effective in treating deadly infections caused by drug-resistant bacteria. Deaths from antibiotic-resistant bacteria, also known as superbugs, have been on the rise globally, and health regulators have called for the development of newer treatments as resistance to older antibiotics grows. The late-stage studies compared the experimental combination of the antibiotics aztreonam-avibactam (ATM-AVI) and existing generic drug metronidazole with a combination of two older antibiotics - meropenem and colistin - to treat complicated intra-abdominal infections and types of hospital-acquired pneumonia. Hospital-acquired pneumonia occurs in patients at least two to three days after being admitted, or in those who have life-threatening lung infections with high mortality rates and who are on mechanical breathing machines. Pfizer said the data from the studies shows the antibiotic combination of ATM-AVI is effective and well-tolerated in treating infections caused by gram-negative bacteria. The cure rate in patients with complicated intra-abdominal infections with Pfizer's combination therapy, along with existing generic drug metronidazole, was 76.4%, versus 74% using the other antibiotics.

AstraZeneca on Wednesday (April 5) said a combination of its cancer drugs Imfinzi and Lynparza met the main goal in a late-stage trial in patients with advanced ovarian cancer. The drugmaker said treatment with a combination of those drugs, along with chemotherapy and bevacizumab - the existing standard of care - improved progression-free survival in newly diagnosed patients with advanced ovarian cancer without certain mutations. Lynparza is jointly developed with U.S.-based Merck & Co as a treatment for breast cancer in early stage with certain mutations. Imfinzi alone, along with chemotherapy and bevacizumab, did not reach statistical significance in its interim analysis, the drugmaker added. Philipp Harter, director, Department of Gynaecology and Gynaecologic Oncology, Evangelische Kliniken Essen-Mitte, Germany and principal investigator for the trial, said: "DUO-O showcases the power of academia and industry collaboration in advancing new treatment combinations for patients with ovarian cancer. These data from the DUO-O trial provide encouraging evidence for this this Lynparza and Imfinzi combination in patients without tumour BRCA mutations and reinforce our continued commitment to finding new treatment approaches for these patients. It will be important to understand the key secondary endpoints as well as data for relevant subgroups."

A pioneering study looking into the possibility of combining existing therapies to slow down cancer growth is showing encouraging early results. Elly Earls reports. A first-of-its-kind medical study at The Care Oncology Clinic on Harley Street, London, has started work on evaluating whether the use of a combination of safe, tolerable, existing, generic therapies could slow down cancer growth and improve survival times in patients for whom other treatments are no longer available or working.

To complete the evaluations of Lateral Flow Device (LFD) test kits, the UK Health Security Agency (UKHSA) will approach pharmacies and send 15 combined kits to pharmacies in December. The combined kits will contain a standard LFD pack, one PCR test along with comprehensive instructions. For the evaluation, a part of meeting Medicines and Healthcare products Regulatory Agency (MHRA) obligations, UKSHA needs "dual swabbing through the community pharmacy Covid-19 lateral flow device distribution service." Pharmacies receiving these kits need to finish the current open carton of LFD packs and then open the next standard carton. Pharmacy negotiator PSNC has advised pharmacies to be mindful of "the expiration date of the kit and ensure these are handed out in sufficient time."

Dispensing activity by community pharmacy in England has witnessed a 4.2 per cent increase across the year 2021 till March 2022, the Pharmacy Market Review 2022 report revealed. The report launched by Christie & Co, said: "All pharmacy settings saw improvements in dispensing numbers, with standard community settings seeing the highest increase of 4.2 per cent, with the average moving to 7,173 items, albeit still below the combined average." "The independent sector fared better than corporate pharmacy, with a combined average increase of 3.5 per cent versus a 1.8 per cent increase in corporate dispensing. Despite some improvement, integrated pharmacies remained the lowest at 1.9 per cent, in part due to the continued restrictions on patient access." Dispensing activity for England across the year to March 2022 increased by 4.5 per cent to an average of 7,765 items per month, reversing the decline witnessed in 2021.

AstraZeneca said on Thursday (June 30) that a combination of its cancer drug, Imfinzi, and chemotherapy showed promise in a late-stage trial in patients with an aggressive form of lung cancer, when given before surgery. Data showed the combination was more effective in removing cancer cells in tissue samples taken during surgery when compared with just chemotherapy in patients with non-small cell lung cancer, the drugmaker said. The interim result is a boost to the company's oncology efforts - a major area of focus - following disappointing data for Imfinzi earlier in the year in another area of therapy. AstraZeneca added the trial would continue as planned to assess the additional main goal of event-free survival, and the interim data would be shared with health authorities globally. Imfinzi belongs to the immunotherapy class of treatments, which boost the body's defences to fight cancer by using antibodies that block or bind to foreign substances in the body. The treatment generated $2.41 billion in 2021 sales.

After receiving approval from the National Institute for Health and Care Excellence (NICE), the NHS is preparing to roll out a new combination treatment for brain tumours in children and young people in England. Dabrafenib with trametinib would be the first ever targeted treatment for children (aged 1-17) with gliomas that have a specific genetic mutation, the health service said. The treatment, which can be taken at home, has been found to slow the progression of the disease by over threefold compared to standard chemotherapy in children with low-grade gliomas that have a BRAF V600E mutation. Clinical trials have shown that it causes fewer side-effects than chemotherapy. NICE approved the combination treatment on Wednesday 24 April. It will be available on the NHS in the coming months.

Cancer patients in the UK are being given a new immunotherapy treatment at Imperial College Healthcare NHS Trust as part of a global phase 1/2 clinical trial, which aims to evaluate its safety and potential for treating 'solid tumour' cancers such as melanoma and lung cancer. The experimental therapy, called mRNA-4359, has been designed to train patients' immune systems to recognise and fight cancer cells, according to researchers at Imperial College London. For the first time in the UK, cancer patients received the treatment at the National Institute for Health and Care Research (NIHR) Imperial Clinical Research Facility at Hammersmith Hospital. In this non-randomised trial, mRNA-4359 is administered to patients either alone or in combination with an existing cancer drug called pembrolizumab, a type of immune checkpoint inhibitor. England won't have any new Covid-19 restrictions before the end of 2021, health secretary Sajid Javid said on Monday (December 27) whilst the government awaits more evidence on whether the NHS can cope with high infection rates in the new year. "There will be no further measures before the new year," Javid told reporters, adding: "When we get into the new year, of course we will see then whether we do need to take any further measures." He said that the highly transmissible Omicron variant of the virus now accounted for around 90 per cent of cases across England and urged people to celebrate New Year cautiously. The government's attention is focused on the number of patients being hospitalised with Omicron after early data last week suggested the variant carried a lower risk of admission. The latest data showed the number of patients in hospital in England with Covid-19 was its highest since March, at 8,474, but a long way off peaks above 34,000 in January. A combination of factors, including Britain's vaccination programme, the lag between infections and hospitalisations and the potentially less harmful effects of the Omicron variant have all been put forward by health experts as possible explanations for lower numbers.

Covid-19 vaccines made by Pfizer and Moderna that use mRNA technology provide the biggest boost to antibody levels when given 10-12 weeks after the second dose, a new study has found. The "COV-Boost" study was cited by UK officials when they announced that Pfizer and Moderna were preferred for use in the country's booster campaign, but the data has only been made publicly available now. The study found that six out of the seven boosters examined enhanced immunity after initial vaccination with Pfizer-BioNTech's vaccine, while all seven increased immunity when given after two doses of AstraZeneca's vaccine. "A third dose will be effective for many of the vaccines we've tested and in many different combinations," Professor Saul Faust, an immunologist at the University of Southampton and the trial's lead, told the media.

The full-time equivalent community pharmacy workforce saw a 6 per cent decline in a year, while the overall number of pharmacists remains nearly constant, according to NHS England's Community Pharmacy Workforce Survey 2022. However, vacancy rates among pharmacists rose 16 percent, suggesting an increased reliance on locum pharmacists who, on average, work fewer hours, the data revealed. According to NHSE, the mandated survey released (Aug. 3) achieved a 95 per cent completion rate among pharmacy contractors, marking a substantial increase compared to only 47 per cent in the previous year. The survey results indicate a shift in working patterns within community pharmacy, with locum pharmacists being utilised more frequently as part of the staffing model. The survey revealed a 6 per cent decrease in the community pharmacy workforce across all roles combined. The number of full-time equivalent pharmacists working in community pharmacy decreased by 2,411, from 20,255 in 2021 to 17,844 in 2022, the survey found. The employed pharmacists decreased by 16 per cent from 12,774 in 2021 to 10,943 in 2022, while locum pharmacists increased by 26 per cent from 4,297 in 2021 to 5,477 in 2022, NHSE said.

The NHS plans to train hundreds of pharmacy staff, expanding their ability to provide advanced clinical services and assume wider roles in dispensing medicines. With a targeted launch in September, up to 840 pharmacy technicians can join in the training programme. The online modules will cover consultation skills, therapeutics, clinical decision-making, assessment skills, and service improvement. "Community pharmacies are pivotal in local communities, and this novel training scheme empowers them to extend their service offerings to a wider patient base," said Health Minister Neil O'Brien. "Through our investment in the workforce, we're bolstering the implementation of the Pharmacy First initiative with £645 million, ensuring patients have access to expert assistance for various common ailments, including urinary tract infections and earaches." NHS said the training will offer flexibility to align with the working schedules and prior experience of community pharmacy technicians. It will combine independent online e-course study, educational supervision, and clinical skills training. Facilitated workshops will concentrate on enhancing clinical assessment skills and applying knowledge and skills through case studies, practice activities, and group discussions.
