Amidst the array of modern amenities at the pharmacy stands Atulkumar Patel, the proprietor and Independent Prescriber (IP) at Lincoln Pharmacy, whose vision has propelled the establishment into a beacon of progressive healthcare. Owner of two Pharmacies, Patel's team of 20 dispenses an average of 20,000 prescriptions monthly, maintaining an NHS/OTC split of 80/20. Recipient of the prestigious Independent Prescriber of the Year award at the Pharmacy Business Awards 2023, Patel reflects on the pivotal moment when his pharmacy pioneered the rollout of COVID-19 vaccine jabs within the community. "When doctors' doors closed, pharmacy was the only place where people could get help and care," Patel recounts, emphasising the pivotal role pharmacies played during the crisis. Patel's dedication during the pandemic extended beyond mere service provision; it was about showcasing the untapped potential of pharmacies.

The NHS England has introduced a training programme for 1000 staff to broaden their community pharmacy roles, thereby improving patient access to services. Among them, 500 pharmacy staff will be trained to supervise pharmacists during prescription training, while the remaining 500 will provide support for various training, including pre and post-registration programmes for pharmacists and pharmacy technicians. According to the NHSE, pharmacist and pharmacy technician roles are rapidly evolving due to healthcare changes and increased demand for clinical services in community pharmacy. "There are 3,000 independent prescribing training spots available for 2023/24. Educational supervision is a crucial part of this to enable the ongoing transformation and skills development of the growing pharmacy workforce." The NHSE-funded training, scheduled to begin next month, will ensure that those involved in educational supervision, including Designated Supervisors (DSs), Pre-registration Trainee Pharmacy Technician education supervisors and Designated Prescribing Practitioners (DPPs), can effectively support learners.

Pharmacy minister Stephen Kinnock has pledged to enhance the role of community pharmacies, particularly in deprived areas, by making better use of the skills of pharmacy teams. In a video message at the SIGMA UK Community Pharmacy Conference 2024 on Sunday, Kinnock also announced plans to expand the Pharmacy First initiative and introduce an independent prescribing service, making prescribing an integral part of the service delivered by community pharmacies. He said: "Since taking on the role of minister of state for care in government, my message has been very clear: we must get our fantastic primary care sector back on its feet to be able to deliver for our patients. "This government is committed to shifting care from hospital to community, from analogue to digital, and from sickness to prevention, and pharmacies will play a vital role in that." Kinnock acknowledged the pressures faced by the sector and expressed concerns that so many community pharmacies have exited the market over the past decade.

England's chief pharmaceutical officer (CPhO) David Webb has promised his "wholehearted support" for the community pharmacy sector at the board meeting of the National Pharmacy Association (NPA) on 28 June in St Albans. After hearing the CPhO at the meeting, NPA chair, Andrew Lane, later declared: "David is someone we can do business with." Webb thus listed his priorities as head of profession: integration of independent prescribing as part of pharmacy practice by 2026; promotion of inclusive pharmacy practice for all pharmacy professionals; assurance of post-registration practice; developing the role of pharmacy technicians; support for Integrated Care Systems and Primary Care Network pharmacy teams (including community pharmacy); medicines optimisation; and strengthening of professional leadership for community pharmacy. He also reported that NHS England had recently increased its team of regional pharmacy integration leads from seven to 14, creating seven new senior posts. Webb told NPA board members: "I want sincerely to thank community pharmacy teams for everything they are doing and to say that you have my wholehearted support. I believe in the importance of community pharmacy and will listen and engage as I've already demonstrated.

These changes could significantly reduce pressures on GPs and hospitals, integrating pharmacies more closely into primary care networks and enhancing their role in preventive care. 2. The fusion of the unique in-depth understanding of medicines by pharmacists together with the competence to prescribe offers will improve access to care and increase capacity in the health system, said the Royal Pharmaceutical Society (RPS) Director for England Ravi Sharma to Parliament at the Health and Social Care Select Committee's inquiry on workforce on Monday (May 23). However, he also stated that a new workforce strategy must support and harness the skills of pharmacist independent prescribers in clinical care with investment in training, both for new and existing workforce; access to supervisors; protected learning and development time; and commissioning of services to make best use of independent prescribers across care settings, supported by appropriate prescribing budgets in community pharmacy. "We're about to see new generation of pharmacists independent prescribers that will make a huge difference to the clinical role of pharmacists to support patient care, but this must be underpinned by protected learning time and a more ambitious approach to commissioning new services to use their skills," Ravi said. He spoke to MPs in Parliament at the Health and Social Care Select Committee's inquiry on workforce, highlighting the vital contribution of pharmacy teams during the pandemic and called for action to support the current and future workforce. Some of the key issues highlighted in the parliament included, the risk of burnout and the need to help boost staff retention; staff wellbeing, including a zero tolerance of abuse from the public; the importance of professional development and protected learning time to enhance patient care and support rewarding careers; the potential of new pharmacist independent prescribers and the need for investment in the current workforce and the need for better workforce data and a pharmacy workforce strategy.

The pharmacy inquiry will explore issues impacting different types of pharmacy, with a particular focus on community, primary care and hospital pharmacy services. It will also consider current challenges around funding, workforce and the digital infrastructure. Planned developments within the profession will enable future pharmacists to be independent prescribers from day one of registration, giving them a greater role within primary care teams. Health and Social Care Committee Chair Steve Brine MP said: "It is clear that pharmacy has a central role to play in the future of the NHS. With a greater focus on personalised and patient-centred healthcare, we will be asking what more must be done to make sure that the profession is in the best shape to meet demand. "Better use of the pharmacy workforce would reduce pressures on general practice and hospitals. However, this will not happen without a planned workforce with the funding, supervision and training to support it. "At the end of the inquiry, the committee will be making recommendations to the government on what action needs to be taken to ensure the potential of pharmacy is realised.

As we transition in England through yet another NHS organisational change, I ask myself what does this mean for community pharmacy? I would like to think that this change will bring about opportunity and a chance for community pharmacy to showcase and continue the excellent work that was carried out during the height of the pandemic and is still ongoing today. I hope that it allows community pharmacy to be regarded as part of the NHS rather than sitting on the side lines. This change has to lead to better funding for community pharmacy, without sufficient funding we will see more pharmacies close. We are hearing a lot about winter pressures but this year it feels like all year round pressure. What I have seen, whilst the NHS is under such pressure, is North East London (NEL) CCG transitioning to an integrated care board (ICB) almost seamlessly. I have seen people transitioning into new roles, whilst working hard to ensure that all plates are still spinning, which at the moment is no mean feat. I spent a day out recently visiting pharmacies with the chief medical officer of NHS NEL, Dr Paul Gilluley. The visits were positive, we felt listened to and understood. The feedback was great, it was recognised that community pharmacy is often the informal front door to the NHS and that we have so much to offer in terms of ill health prevention. Community pharmacy can offer a total solution as long as we have the tools to do so, which can save so much time and money. An example is the GP CPCS service, which has launched well across NEL.

A group of 20 parliamentarians has written to health secretary Wes Streeting MP, calling for urgent government action to address the growing financial crisis facing community pharmacies. In the cross-party letter, supported by Community Pharmacy England (CPE), the MPs warned of a real risk of more pharmacy closures, with one in six pharmacy owners saying that they are unsure if their pharmacy will survive the year. They have urged the Department of Health and Social Care (DHSC) and NHS England (NHSE) to prioritise long-term planning for pharmacies, including the implementation of the Community Pharmacist Prescribing Service. The MPs have also highlighted the need for more effective marketing for Pharmacy First to increase both walk-ins and referrals to this critical service. CPE chief executive Janet Morrison welcomed the support of 20 cross-party parliamentarians and recognising the vital role pharmacies play in the local community. She stressed the urgent need for immediate government intervention to ensure the survival of the sector and to maximise its potential in providing accessible care.

The Royal Pharmaceutical Society (RPS) and National Pharmacy Association (NPA) have both urged the new health secretary Steve Barclay to back the 'pharmacy first' approach in England as mentioned by his predecessor on numerous occasions. "Sajid Javid recognised the vital role of community pharmacy and the potential of a 'Pharmacy First' to support patient access to care. I would urge the new Health Secretary to see this through to completion," said chair of RPS in England Thorrun Govind. Thorrun hopes the new health secretary will engage with pharmacy leaders about how we can make the most of our health and care workforce to support the NHS recovery, including reducing health inequalities, managing the growing cost of long-term conditions, and utilising the enhanced skills of Pharmacist Independent Prescribers. Commenting on the new appointment, she said: "This is a crucial time for the future of health and care - with continued pressures on teams, changes to NHS structures and organisations, and the need for long-term investment in the workforce. "With a 'refresh' of the NHS Long-Term Plan and the Government's workforce plan expected later this year, these must support a more ambitious approach to advancing the clinical role of pharmacists across the NHS to better meet changing patient demand, backed by investment in pharmacy education and training.

Health Education England (HEE) has announced launch of a new funded Independent Prescriber courses for pharmacists to be made available before March 2022. Places are available for both Independent Prescribing (IP) and Clinically Enhanced Independent Prescribing (CEPIP), and eligible pharmacists are encouraged to apply to the course providers directly. These courses will be allocated on a first come, first served basis, and a further round of funded Independent Prescriber training will be made available from Autumn 2022. The independent prescriber guide offers practical guidance and support on the prescriber role, such as how to become an independent prescriber, applying for an IP course, or expanding the scope of practice.

Maxine McCabe has made history as the first community pharmacist in the UK to be credentialed as a core advanced pharmacist by the Royal Pharmaceutical Society (RPS) in Scotland. Announcing McCabe's achieving, NHS Education for Scotland (NES) stated that this credential demonstrates her expertise across the four pillars of advanced practice: clinical, leadership and management, educating others and research. Maxine has worked in the community pharmacy sector since qualifying as a pharmacist in 2008, consistently showing a keen interest in developing the wider pharmacy team and enhancing clinical services for the local patient population. Throughout her career, she has managed various community pharmacies, including Boots and Parkhead Health Centre Pharmacy, where she played a crucial role in developing pharmacy practice and nurturing pharmacy support roles and trainee pharmacists. In 2023, Maxine took on a part-time role as a Senior Educator in the Prescribing & Clinical Skills teams at NHS Education for Scotland (NES). She continues to practice as a pharmacist, leading the Teach and Treat Training Hub at Burnside Pharmacy in Lanarkshire. McCabe expressed her excitement about the recognition, saying, "I am delighted to obtain my Advanced Practice credential and showcase that this is achievable in community pharmacy."

Health secretary Wes Streeting has pledged to stabilise the community pharmacy system before enabling it to take on a broader role in healthcare, as he addressed the Health and Social Care Select Committee in Parliament on Wednesday (18 December). "Now that we have got the budget set, we will be consulting with Community Pharmacy England shortly in the New Year," Streeting stated. "I do want to reassure pharmacists across the country that we are taking into account the enormous pressures they are under when thinking about allocations so that we can stabilise the system and create the foundations from which to recover." The health secretary emphasised that his top priority is to "stabilise the [community pharmacy] system" before expanding initiatives such as Pharmacy First and independent prescribing. He also expressed a vision for community pharmacies to play "a bigger role… as part of the shift from hospital to community," but stressed that immediate steps are required to address the sector's current financial strain. The announcement that Community Pharmacy Contractual Framework (CPCF) negotiations will commence early in the New Year was reaffirmed by Streeting, echoing a statement made earlier in the week by minister for care Stephen Kinnock during a Westminster Hall debate.

Healthcare reform emerged as a cornerstone of the Labour Party's manifesto leading up to the recent general election. A standout pledge was the establishment of a Community Pharmacist Prescribing Service, aimed at granting pharmacists independent prescribing rights where clinically suitable. This initiative is seen as a positive step in addressing critical challenges within the National Health Service (NHS). Numark Chairman Harry McQuillan praised Labour's recognition of pharmacists' role in patient care through expanded prescribing rights. "Let's be honest, pharmacists are an underutilised resource within the healthcare system. We possess extensive training and expertise, yet our full potential hasn't always been acknowledged. "By harnessing pharmacists expanded prescribing rights, Labour is acknowledging our significant role in patient care. This initiative aims to recognise our skills and enhance patient care, ensuring quicker access to necessary treatments," he said. However, he pointed out that the concept requires further exploration, as the rights to prescribe are already granted through the recent change to undergraduate training that incorporates prescribing.

Jonathan Burton will receive the 2022 RPS Charter Award for his outstanding work for the Society and the wider profession in promoting the interests of pharmacy. The award will be presented by the President at a future meeting of Fellows and Members to take place at the Society's Edinburgh office on 22nd March 2023. Jonathan's main role is as a practising, patient-facing pharmacist within the community in NHS Forth Valley where he uses his advanced practice skills and independent prescribing qualification to assess and treat acute ENT, dermatology, and respiratory conditions. He was one of the first community pharmacist prescribers in Scotland and has always been at the forefront of innovation and service development. Jonathan has demonstrated outstanding commitment and passion for the pharmacy profession throughout his career. He has excelled as a pharmacist in the Right Medicine Pharmacy group, on local and national committees, in advocacy for the pharmacy profession and as a local and national leader who is highly regarded as an exceptional role model for the profession.

Pharmacy minister Stephen Kinnock highlighted the role of community pharmacies in the Labour government's proposed healthcare reform during his speech at the 24th annual Pharmacy Business Awards on Wednesday. He outlined the government's vision for three strategic shifts: from hospital to community, from analogue to digital, and from sickness to prevention. Emphasising the crucial role that community pharmacies will play in achieving these changes, he said: "Pharmacies are based in, and recruit from, and are part of their communities, and we have to put you right at the heart of those three shifts if we are to deliver the change that our country so desperately needs." Kinnock also acknowledged the contribution of pharmacies in safeguarding the nation's health. He stated: "I've already heard, just in the short time that I've been in government since 4 July, of many examples of just how much patients and communities rely on your services and the lengths to which you go to deliver care."

Pharmacists with 'relevant experience in a pharmacy setting' can enroll for accredited independent prescribing course, as the General Pharmaceutical Council (GPhC) has decided to scrap the requirement of spending at least two years on the register and having previous experience in a specified clinical or therapeutic area before enrolling for the course. The GPhC Council meeting held on Thursday instead proposed that applicants must have "relevant experience in a pharmacy setting and be able to recognise, understand and articulate the skills and attributes required by a prescriber." This experience and awareness will act as the basis of their prescribing practice whilst training. The regulator noted that the majority of stakeholder organisations, including the Chief Pharmaceutical Officers, the Royal Pharmaceutical Society and the statutory education bodies, were in favour of removing the requirement in a consultation on the topic. They highlighted that a specific two-year period was not in itself a robust indication of whether an individual was ready to become a prescriber. They also highlighted that the rapidly developing roles in the profession meant more pharmacists were likely to gain the necessary experience more quickly than in the past. A smaller number of organisations and a larger minority of individuals were opposed, citing that a specific two-year period gave pharmacists the time they needed to develop experience and confidence before being ready to enrol on a course.

Health Education England (HEE) has developed funded training offers for community pharmacists, including locums aiming to become independent prescribers. Almost 3000 independent prescribing training places will be available to pharmacists from March 2023. This training enables pharmacists to support patients from diagnosis to prescribing, and to provide advice and follow-up - while also helping them to feel confident and prepared for the new challenges of their role. Courses will be available between April 2023 and March 2024, with several universities offering multiple dates for cohort intakes. Start dates will depend on the university provider. Training will enable the provision of new models of care: supporting patients from diagnosis to prescribing, providing advice and follow-up, and preparing pharmacists to provide clinical care, as pharmacy services become more widespread within emerging clinical pathways.

Northern Ireland's Health Minister, Mike Nesbitt, has welcomed the launch of the 2024/25 Experiential Learning (EL) Programme, designed to equip future pharmacists with the skills needed for a greater role in clinical care. Starting in 2026, pharmacy undergraduate students will qualify as independent prescribers upon registration, and this programme is designed to prepare them for that responsibility. The EL Programme provides pharmacy students with multi-sector clinical work placements across the region. Speaking at the programme's launch at Ulster Hospital, Minister Nesbitt said: "This is the second year of this successful programme, which will see pharmacy students in Northern Ireland benefit from the training by completing up to twelve weeks of practice-based learning in hospitals, community pharmacies and general practices across NI. "This will provide them with actual practice-based experiences in a range of healthcare settings, facilitating them to interact with patients and engage in interprofessional practice.

Acknowledging publication of the Royal Pharmaceutical Society (RPS) Scotland's new professional vision for the future of pharmacy in Scotland - Pharmacy 2030, a Parliamentary Motion has been lodged in the Scottish Parliament. The vision, produced in collaboration with the National Pharmacy Technician Group Scotland, expects all pharmacists in a patient-facing role to become independent prescribers by 2030. The motion recognised the aim of the publication that by 2030 pharmacy teams will work together, using their expertise to make the best use of medicines and, will take a person-centred approach, providing care holistically rather than by clinical condition. It highlighted the important role that RPS would play by working with partners across health and social care to turn this vision into a reality.
