Many of us know that oral health plays a hugely important role in our general health and wellbeing. We're continuing to see a rise of cosmetic dentistry such as veneers and teeth whitening[1] but fundamentally, it shouldn't be forgotten that oral health encompasses so much more than aesthetics. Our diets, the oral hygiene products we are using, and our brushing techniques are all elements that impact our oral health. It's been hard to miss in the news that there is a dental crisis in the UK, with 'dental deserts' leaving some areas with over 3,000 patients for every NHS dentist[2]. With oral health being of such importance to our overall health[3], it's only reiterated to us at Haleon that we must continue to promote access to dental care and understanding of better dental health. As I've mentioned in previous articles for Pharmacy Business, an understanding of preventative, self-care is hugely beneficial, not just to improve health inclusivity but to help reduce these current burdens on both healthcare and dentistry professionals. We know that at Haleon, we, and the consumer healthcare industry as a whole, have a responsibility to empower people to equip themselves with the tools and information to participate in their own and loved ones' health - and dental health is such an important part of that.

Nine more new products have been classed as 'Drugs for which Discount is Not Deducted' (DND) from 1 July 2022, announced Department of Health and Social Care (DHSC). This takes the total number of products granted DND status over the past 2 years to over 500 following checks made by the Pharmaceutical Services Negotiating Committee. The American healthcare company, CVS Health, has decided to remove the cold and cough pills containing phenylephrine from the drug stores. The following decision comes after the health regulator, the US Food and Drug Administration (FDA) determines the decongestant is ineffective towards flu if taken orally. However, when ingested in pill or tablet form, this ingredient fails to reach its destination from the stomach to the nasal passages. CVS Health spokesperson told USA Today, "We are removing a small number of oral decongestant products that contain phenylephrine as the only active ingredient from CVS Pharmacy stores but will continue offering many other oral cough and cold products to meet consumer needs." They almost run more than 9,000 stores in the US whereas their rival, Walgreens has not yet taken the step to remove over-the-counter medications from their shelves.

The Department of Health and Social Care (DHSC) has issued a medicine supply notification for Dioralyte oral rehydration sachets. "Dioralyte oral rehydration sachets are expected to be in limited supply until late December 2022. St Mark's solution is available as an unlicensed special and can support an uplift in demand," said DHSC. "Kidderminster formula is available as an unlicensed special and can support a partial uplift in demand. As an alternative to the unlicensed specials, the individual components can be purchased from community pharmacies and supermarkets or prescribed, but there have been intermittent supply issues with glucose powder." Diluted apple juice has been reported to be a potential alternative to electrolyte maintenance fluids in children with mild gastroenteritis and minimal dehydration. If there is currently no listing on dm+d for the imported product for prescribers to select using their prescribing systems an EPS prescription for unlicensed St Mark's solution cannot be issued.

The Department of Health and Social Care (DHSC) has issued a medicine supply notification for Sulfasalazine 250mg in 5ml oral suspension sugar free; Pilocarpine hydrochloride 4% eye drops; Fluticasone 50microgram / Salmeterol 25microgam (Combisal) pressurised metered dose inhaler (pMDI); and Dalteparin (Fragmin) 10,000units/1mL solution for injection ampoule on Wednesday (21 December). "Sulfasalazine 250mg/5ml oral suspension sugar free is out of stock from w/c 19th December until early June 2023. Sulfasalazine 500mg non-enteric coated tablets remain available "IPS Pharma, Nova Labs, and Rokshaw have currently confirmed they can manufacturer sulfasalazine 250mg/5ml oral suspension," said DHSC. Pilocarpine 4% eye drops are out of stock until w/c 26th December 2022. Alternative strengths of pilocarpine (1% and 2%) eye drops remain available and will be able to support increased demand. Specials of pilocarpine 4% preservative free eye drops are available if the licensed alternatives are not suitable. For off-label use of the 4% drops in the treatment of xerostomia (dry mouth) in palliative care, pilocarpine 5mg tablets are available and are licensed for this indication.

The Scottish government has issued a circular to inform community pharmacy contractors that the Serious Shortage Protocol (SSP) for Clarithromycin 125mg/5ml oral suspension, an antibiotic product, due to expire on 15 December 2023, has been extended until Friday 12 January 2024. The extension is applicable to SSP053 which allows community pharmacists to substitute this product with Clarithromycin 250mg/5ml oral suspension for patients presenting with an NHS or private prescription. Scotland's chief pharmaceutical officer, Alison Strath, has advised health boards to spread this information to community pharmacy contractors on their pharmaceutical lists and area pharmaceutical committees, as well as general practices. Community pharmacists have been advised to review and familiarise themselves with the new end date for the noted SSP.

Anyone who is concerned should speak to their pharmacist about whether they can take their medicine at different times, as stopping it could have serious consequences. Mr Khan, who will be observing Ramadan himself, said: "Confusion can sometimes arise when it comes to medication and managing health conditions whilst fasting", adding it is important for patients to keep managing health during Ramadan. Having injections or vaccinations is fine, as are blood tests, using eye or ear drops, and using anything that patients don't consume orally - so nicotine and other transdermal patches, pessaries and dialysis, for example. It's also ok to eat and drink as a result of a condition which causes forgetfulness.

The Community Pharmacist Consultation Service (CPCS) will be expanded to enable urgent and emergency care settings to refer patients to a community pharmacist for a consultation for minor illness or urgent medicine supply from Monday (15 May), the DHSC and NHSE said. The service builds on the progress made in GP referrals via the CPCS and hospital referrals under the Discharge Medicine Service. It was originally planned to launch in March, and fee for this service will be the existing CPCS fee of £14, as per the agreement for both year 4 and year 5 of the Community Pharmacy Contractual Framework (CPCF) 2019 to 2024 5-year deal. In an update on the CPCF, published today (12 May), the Department of Health and Social Care (DHSC) and NHS England (NHSE) has also committed to the 4 October 2023 launch date for the Tier 2 of the Pharmacy Contraception Service, subject to a positive evaluation of the pilot. The Tier 1 of the service was launched on 24 April, delayed by over three months. This new service enables community pharmacists to provide ongoing management, via a patient group direction (PGD), of routine oral contraception that was initiated in general practice or by a sexual health clinic. The fees for this service are as follows: a fee for each consultation of £18; and a set-up fee of £900, paid in instalments. The Tier 2 will "enable community pharmacists to also initiate oral contraception, via a PGD, and provide ongoing clinical checks and annual reviews," Alette Addison, deputy director for pharmacy, dental and optical at the DHSC and Ali Sparke, director for dentistry, community pharmacy and optometry at the NHSE, said in a letter.

The National Health Service (NHS) has said that the roll-out of blood-thinning drugs has helped save thousands of lives. In January 2022, the NHS launched a drive to rapidly expand the use of life-saving direct oral anticoagulants (DOACs) in people who are at increased risk of strokes. Since then, more than 24 million prescriptions have been given to such patients and around 460,000 more people have started taking the drugs. This prevented an estimated 17,000 strokes and 4,000 deaths in the last 18 months, as revealed by the new NHS data. Amanda Pritchard, the NHS chief executive, hailed the "lifesaving NHS rollout" at the King's Fund annual conference. It is part of a major NHS drive on "prevention" - to catch more killer conditions earlier and save more lives.

The pharmacy bodies have welcomed Health Select Committee's inquiry into pharmacy services and are calling the community pharmacies to engage fully. On Thursday (8 June), MPs launched a new inquiry to examine the 'readiness of pharmacy services'. At the end of the inquiry, the committee will be making recommendations to the government on what action needs to be taken to ensure the potential of pharmacy is realised. It is currently seeking views and evidence from anyone who can answer any or some of the questions listed here by Thursday 6 July. National Pharmacy Association (NPA), Company Chemists' Association (CCA) and Royal Pharmaceutical Society (RPS) have confirmed that they will be submitting the written evidence. RPS said it will be submitting written evidence, and if they are called to give oral evidence they'll do so. Malcolm Harrison, Chief Executive of the CCA commented: "We welcome the Health Committee's much-awaited inquiry into pharmacy. Whilst the recent announcement of investment into the sector is welcome, this is new money for new activity. The historic underfunding of community pharmacy remains, and Primary Care Networks (PCNs) continue to directly recruit community pharmacists.

The National Institute for Health and Care Excellence (NICE) has released conclusive draft guidance, suggesting Pfizer's Vydura (rimegepant) as a choice for treating acute migraines in adults with or without aura. However, there is a caveat. This recommendation applies exclusively to individuals with a prior history of migraines, meeting one of the following criteria: At least 2 triptans were tried and they did not work well enough or Triptans were contraindicated or not tolerated, and nonsteroidal anti-inflammatory drugs (NSAIDs) and paracetamol were tried but did not work well enough. This drug was developed by the US-based Biohaven Pharmaceuticals, acquired by Pfizer in 2022, in an $11.6 billion deal. Nurtec ODT is the commercial name for the drug in the US, while the European Union approved the drug under the name Vydura in April 2022. NICE recommended rimegepant, an oral lyophilisate (dissolving wafer) and the first oral calcitonin gene-related peptide (CGRP) receptor antagonist for acute migraine treatment, to be taken at the onset of a migraine attack. Migraine affects one in seven people in the UK, often with a debilitating impact, Pfizer UK said in a statement. The condition entails symptoms such as head pain, vomiting, nausea, altered vision, fatigue, and heightened sensitivity to light, sound, and odours.

The Department of Health and Social Care (DHSC) has issued a further five new Serious Shortage Protocols (SSPs) for Phenoxymethylpenicillin (Pen V) to enable the continued supply of antibiotics to patients. "The new SSPs, introduced with immediate effect, allow community pharmacists to consider different oral antibiotic preparations, to enable the continued supply of antibiotics to patients and mitigate the ongoing supply disruptions affecting Phenoxymethylpenicillin," said DHSC. On Friday (16 December) SSP043-SSP047 has been authorised by the Secretary of State to provide pharmacists with procedures to follow in providing suitable alternative oral antibiotics to substitute Phenoxymethylpenicillin. "For each SSP, DHSC has included specific patient counselling points which must be taken into account when deciding whether supply in accordance with an SSP is suitable for a patient."

Bambuterol 10mg tablet, used in the treatment of asthma and chronic obstructive pulmonary disease, is facing supply disruption as sole supplier of the medicine, Astra Zeneca, is discontinuing its production, according to an update by the Department of Health and Social Care (DHSC). The available stock of the medicine is expected to be exhausted by the end of June 2022. Meanwhile, other oral β2-agonists, which are the preferred route of administration, remain available, DHSC said.

Community Pharmacy England has submitted a full and detailed written response to the Health and Social Care Select Committee's pharmacy inquiry. The Committee is expected to hold oral evidence sessions when Parliament returns in the Autumn. The commencement of this inquiry follows on from those held on other primary care sectors, including Dentistry and General Practice. According to the Committee website this inquiry 'will explore issues impacting different types of pharmacy, with a particular focus on community, primary care and hospital pharmacy services'. Key themes of CPE's submission include- Suggestions on future service provision; Challenges with the pharmacy workforce; Longstanding underfunding of the sector; and Current pressures. Janet Morrison OBE, Chief Executive of Community Pharmacy England, said: "This is an extremely important opportunity for all who work with and in the Pharmacy sector, to highlight not only the extreme challenges we face, but also ideas and suggestions for what more we can do to help patients and the public in the future.

As part of a new blueprint for primary care, the government today (May 9) announced an investment of £645 million over two years to expand community pharmacy services in England. In a statement, NHS England said: "For the first time ever, patients who need prescription medication will be able to get it directly from a pharmacy, without a GP appointment, for seven common conditions including earache, sore throat, or urinary tract infections." Prime minister Rishi Sunak hopes that the measures will help end the "all-too stressful wait on the end of the phone for patients" by freeing up 15 million slots at doctors' surgeries over the next two years. "We will end the 8am rush and expand the services offered by pharmacies, meaning patients can get their medication quickly and easily," he said Almost half a million women will no longer need to speak to a practice nurse or GP to access oral contraception and will instead be able to pop into their local pharmacy for it, according to the government announcement. Blood checks for people suffering from moderate risk of heart attack or stroke conducted in community pharmacies will more than double from 900,000 last year to 2.5 million next year. NHS chief executive Amanda Pritchard said the "ambitious package" would help transform how care is provided within the health service "with pharmacies playing a central role in managing the nation's health including providing lifesaving checks and medication for common conditions for the first time.

The Medicines and Healthcare products Regulatory Agency (MHRA) on Wednesday (9 January) announced that the novel type 2 oral polio vaccine (nOPV2) has been quality-assured (prequalified) by the World Health Organization (WHO). Developed by a team including scientists from the MHRA, nOPV2 is considered a safer polio vaccine that helps to protect children from the disabling and life-threatening disease and lower the risk of vaccine-derived outbreaks. The vaccine received WHO Emergency Use Listing three years ago, and since then 950 million doses have been delivered worldwide. The UN health agency's decision to add it to the list of prequalified vaccines is based on the analysis of outcomes in vaccinated populations that has confirmed strong safety profile and effectiveness.
