Tens of thousands of smokers in England will be offered an improved anti-smoking pill by the NHS to help them quit. NHS England has announced the rollout of a new generic version of Champix, called Varenicline, which has been proven to be as effective as vapes and more successful than nicotine replacement gum or patches. When used alongside behavioural support, such as counselling, the daily pill has been shown to help one in four smokers quit for at least six months. NHS chief executive Amanda Pritchard called the introduction of Varenicline a "vital step in shifting NHS further towards prevention" and highlighted that it could be a "game-changer" for people who want to quit smoking. Speaking at the NHS Providers annual conference in Liverpool on Tuesday, she emphasised that smoking remains "one of the biggest public health issues facing the NHS."

The new Smoking Cessation Service (SCS) will commence on March 10, 2022, allowing community pharmacy contractors to register to provide the service on the NHS Business Services Authority's (NHSBSA) Manage Your Service (MYS) portal from March 1, 2022. This is the second of the two new Advanced services to be introduced this financial year, after the Hypertension case-finding service, which were agreed outcomes from negotiations for Year 3 of the Community Pharmacy Contractual Framework. Since the service is an Advanced service, contractors are free to decide if they wish to provide it, as most would be busy completing work for the Pharmacy Quality Scheme 2021/22 alongside winter-related workload, the Pharmaceutical Services Negotiating Committee said. Therefore, those who decide to start this service should consider the likelihood of receiving sufficient referrals to make provision of the service worthwhile, the PSNC said.

Generic pharmaceutical company Brown & Burk has announced that it is "the first and the only company" to launch the new anti-smoking pill 'Varenicline' in the UK. Available through NHS stop smoking services, the daily pill has been shown to be as effective as vapes and more successful than nicotine replacement gum or patches in helping smokers quit. When combined with behavioural support, such as counselling, Varenicline was found to help one in four smokers quit for at least six months. The company told Pharmacy Business that its Varenicline 1mg and 0.5mg tablets are being distributed by Teva UK, branded with Brown & Burk's livery. Health secretary Wes Streeting recently highlighted the potential benefits of the pill, stating that its rollout could "save the NHS millions of pounds, save appointments to help other patients be seen faster, and save lives." A study by University College London suggested that Varenicline use could help over 85,000 people try to quit smoking each year, potentially preventing up to 9,500 smoking-related deaths over the next five years.

Smoking is the most significant cause of preventable deaths in England, and the health risks associated with smoking well documented. This not only puts a heavy burden on the healthcare system but also has a substantial social impact. Its link to health inequalities is more evident because it is more prevalent amongst the most vulnerable in our society, with low educational attainment and poor socioeconomic status. Even as smoking rates progressively reduced, these patients remained the most difficultto reach with cessation services. Recent years have also seen many local authorities stop providing locally commissionedNHS stop smoking service, citing funding constraints. The postpandemic financial reckoning may yet see more strain on what is left in the coffers. Online vape store GoSmokeFree analysed data from NHS Stop Smoking Service in England from April to September 2023 to determine the number of individuals in each of the nine UK regions who reported successfully quitting smoking after setting a quit date. The findings showed that the South East has the highest number of successful quitters, with 1,226 smokers per 100,000 reporting to have given it up. The Isle of Wight is the most successful area to quit smoking in the South East with 3,124 people quitting per 100,000 smokers. Of the 4,294 individuals who set a quit date in this region, 803 admitted to not being able to achieve their target. A spokesperson from the online retailer noted that overcoming nicotine withdrawals and mental challenges can be exceptionally difficult when attempting to quit smoking.

The Pharmaceutical Services Negotiating Committee (PSNC) and NHS England have agreed that community pharmacy contractors will have to participate in one mandatory national health campaign. This campaign on weight management commenced from Tuesday (03 January) and will run until Sunday (29January). PSNC said: "Pharmacies can be required to participate in up to six health campaigns at the request of NHS England per year (1st April to 31st March). Pharmacies will be involved by displaying and distributing leaflets provided by NHS England. In addition, pharmacies are required to undertake prescription-linked interventions on major areas of public health concern, such as encouraging smoking cessation. "Contractors can order pharmacy specific posters, wallet cards ('info' cards with a QR code signposting people to the Better Health website and free tips and tools to lose weight) and a dispenser for these cards on the Campaign Resource Centre website. A guidance leaflet to support community pharmacy teams' conversations with patients is also available," said PSNC.

Along with pharmacists, pharmacy technicians in England are now able to perform blood pressure checks as part of the Hypertension Case-Finding Service and deliver the Smoking Cessation Service (SCS). The government has amended Drug Tariff and includes the amendments to The Pharmaceutical Services (Advanced and Enhanced Services) (Amendment) (England) Directions 2023. Pharmaceutical Services Negotiating Committee (PSNC) said: "The service specifications for both services are currently going through NHS England's publication process; when these are published PSNC will alert contractors through their normal communication channels. PSNC resources for both of these services are also currently being updated."

Pharmacy owners will be given additional time to claim for the provision of four services if IT issues, outside their control, prevent timely submissions. Effective February 2025, this provision applies to Hypertension Case-Finding, Pharmacy Contraception Service, Pharmacy First, and Smoking Cessation Services. Currently, claims must be submitted via the Manage Your Service (MYS) portal by the 5th of the following month, with late claims accepted only within three months of the actual submission date. Under the new rule, pharmacy owners will have an additional twelve months (from the date by which the claim should have been submitted) to claim payment if they can provide evidence of an IT issue beyond their control that delayed the claim submission. This new provision will be included in Part VIC of the February 2025 Drug Tariff, which will also have a clarification about de-registration from the four advanced services. The clarification will state that pharmacy owners who de-register from providing one of these services cannot re-register for four months after their 30-day notice period ends.
