The Royal College of General Practitioners (RCGP) has warned of a potential 'mass exodus' of qualified GPs due to mounting work pressures. A recent RCGP survey found that over 40 per cent of GPs are unlikely to remain in general practice over the next five years, with a quarter stating it is "very unlikely." The situation is particularly concerning in England, with the highest rate of GPs considering leaving the profession reported to be in the East of England and the South East (47 per cent) and the lowest in the North West (36 per cent). Stress was cited as the main factor for leaving the profession, with more than half of respondents indicating that they find the job too stressful. Additionally, 13 per cent of respondents intend to leave the UK and work overseas. Almost 4 out of 10 GPs said they regularly worked more than 40 hours a week despite fewer than 1 in 10 being contracted to do so. The pressures faced by GPs are so severe that 22 per cent report feeling overwhelmed and unable to cope once or twice a week. The College noted that GPs and their teams have delivered an average of 30 million appointments per month in the past year - over 4 million more each month than in 2019 - despite the number of fully qualified, full-time equivalent GPs decreasing by 601 over the same period.

Community Pharmacy England (CPE) has urged pharmacy owners to maintain the safety of their services while managing the additional workload brought on by the collective action of GP services, starting today (Thursday, 1 August). General practices across England have begun implementing a work-to-rule strategy indefinitely, following a ballot of GP partners conducted by the British Medical Association (BMA). Under this approach, GP practices will strictly adhere to contracted hours and duties, performing only the tasks specified in their NHS contract. With fewer GP appointments available, it is anticipated that more patients will turn to their community pharmacies for help and advice. Therefore, CPE has stressed the importance of pharmacy owners maintaining the safety of their services, with a focus on the safe supply of medicines, during this period of GP collective action.

Health and Social Care Secretary Wes Streeting has on Wednesday laid out his vision for reforming the National Health Service, stating that the NHS is "broken but not beaten" after years of Conservative governance. In his address to the annual Labour Party conference in Liverpool, Streeting highlighted the current challenges facing the NHS, including record-high waiting lists, ambulance delays, and difficulties in accessing GP appointments. Stressing that "reform or die" is the choice facing the NHS, he rejected the idea of simply increasing funding without systemic changes. Streeting noted some initial successes since Labour took office, including employing 1,000 more GPs and negotiating an end to junior doctors' strikes. He revealed that crack teams of top clinicians will be deployed to hospitals across the country to roll out reforms: to treat more patients and cut waiting lists.

As part of a new blueprint for primary care, the government today (May 9) announced an investment of £645 million over two years to expand community pharmacy services in England. In a statement, NHS England said: "For the first time ever, patients who need prescription medication will be able to get it directly from a pharmacy, without a GP appointment, for seven common conditions including earache, sore throat, or urinary tract infections." Prime minister Rishi Sunak hopes that the measures will help end the "all-too stressful wait on the end of the phone for patients" by freeing up 15 million slots at doctors' surgeries over the next two years. "We will end the 8am rush and expand the services offered by pharmacies, meaning patients can get their medication quickly and easily," he said Almost half a million women will no longer need to speak to a practice nurse or GP to access oral contraception and will instead be able to pop into their local pharmacy for it, according to the government announcement. Blood checks for people suffering from moderate risk of heart attack or stroke conducted in community pharmacies will more than double from 900,000 last year to 2.5 million next year. NHS chief executive Amanda Pritchard said the "ambitious package" would help transform how care is provided within the health service "with pharmacies playing a central role in managing the nation's health including providing lifesaving checks and medication for common conditions for the first time.

"Our main concerns are that the use of what they call an asynchronous supply, where the prescriber neither sees nor talks to the patient at the time they issue the prescription, does lay itself open to perhaps people getting hold of medication that they shouldn't," the Head of Policy, PDA said. The Pharmacists' Defence Association (PDA) engages in a critical conversation on BBC Radio 4's Woman's Hour, shedding light on the online prescribing of weight loss drugs and advocating for safer practices. The PDA's spokesperson, Head of Policy Alima Batchelor, emphasised the importance of adhering to professional guidelines to ensure patient safety and mitigate regulatory concerns. During the feature, the PDA highlighted the complexities and risks associated with online supply chains, stressing the necessity for direct clinical consultations and pharmacist involvement in medication management. ""There are guidelines and advice about verification within the DiCE guidance, but it doesn't go quite far enough for us. We are concerned when patients decline to allow the online prescriber to advise their GP of what they're taking," Batchelor highlighted.

NHS England and NHS Improvement published on May 26 the findings of the Fuller 'stocktake' - setting out how primary care can work with partners across health and care to best meet the needs of their local communities. Dr Claire Fuller's report lays emphasis on the essential role of primary care and the potential of integrated neighbourhood teams in reducing the burden of ill health and tackling health inequities. It commends community pharmacy for keeping "its doors open to the public throughout" the pandemic whilst being "among the most recognisable of a multitude of dedicated staff delivering care around the clock in every neighbourhood in the country". The report highlights "recruitment and retention challenges across the wider primary care workforce" including in community pharmacy. Stressing the importance of community pharmacy teams in urgent care and prevention, including early diagnosis of cancers, the report points out that pharmacists could play "a more active role in signposting eligible people to screening and supporting early diagnosis, building on a number of successful pilots such as those from the Accelerate, Coordinate, Evaluate (ACE) programme".
