IHCTs have characteristics and problems that are unique to health care settings and the health providers who work in them. Drinka and Clark present a groundbreaking attempt to develop a comprehensive framework for IHCTs.

The following is the abstract as stated by the author: "Purpose: To describe factors that ensure a smooth transition from nurse practitioner (NP) student to independent NP during the first year of practice. Data sources: Two contrasting case examples, the authors' experiences, and selected professional literature. Conclusions: Key points to consider upon searching or taking a first NP job include patient, colleague, and clinic factors. Patient factors include mix of patient presentations, complexity of patient presentations, insurance status, and population. Colleague factors include mentorship, charting, mix of providers, and availability of providers. Perhaps most importantly, clinic factors include productivity expectations, practice mission, charting systems, on-call requirements, supervision of other staff, and teaching status. Together, these factors can largely determine whether an NP's days are satisfying or frustrating in a new job. Implications for practice: The transition from NP student to independent NP can be daunting. Although nursing schools and practice sites have responsibilities to ensure the transition is smooth, the new NP is ultimately responsible for the transition. [ABSTRACT FROM AUTHOR]"

Hi All- This report provides a lot of great information about the role of the APRN in providing primary care in Georgia with citations to a lot of the information we have learned about in classes.

My intended area of practice is in HIV nursing. In my last post I provided a link to the competencies needed to be an advanced practice nurse in the field, and I talked a little bit about steps I'm taking to close that gap. To hopefully move me along the continuum from novice to expert. I thought this article was interesting and provided a broader approach in how nurses might use residency program for exactly that purpose. The article talks about DNPs but it is no different for the MSN prepared nurse. It seems that residencies may become more the norm in the future - and that might be good for all, new nurses and patients alike.

This article from the AACN provides a good summary of the history of the AGACNP role up to the development of the consensus model, and developments afterwards until 2011, when the article was published. It also provides advice to readers on how to react to the consensus model.

The researchers in this article focused on analyzing what kind of economic impact that giving APRNs full scope of practice in North Carolina would have. They calculated the impact by first projecting the increase in APRN graduates choosing to work in the state after full scope would be awarded. Projections of the increase in demand for providers due to both the ACA and an aging population were also taken into consideration. The findings in this analysis were that giving APRNs full scope of practice in the state would benefit the economy with increased output, employment, and tax revenue all while decreasing provider shortages. The state could save upwards of $495 million in gross product annually.

Hopefully, results such as these will give other states with more restrictive policies incentive to change their practice guidelines.

This article talks about how the use of acute care nurse practitioner helps hospitals deliver better care because the APRNs are able to draw from past experiences in order to provide the best care to their patients.

This article discusses the benefits and pit-falls to autonomous primary care Nurse practitioner practice and advocates for cost effective improvements to our healthcare system. The research suggests that Nurse practitioners who practice independently report greater job satisfaction, improved patient outcomes and provide low cost, high quality services that are equal or superior to primary care physicians. Unfortunately, current healthcare reimbursement models, policies and reluctant physicians have created barriers to autonomous NP practices. The author suggests that autonomous practice requires further research across the U.S, the establishment of a well-defined model and the evaluation of patient outcomes, in order to determine whether the present day Nurse Practitioner requires future collaborative agreements with physicians, in order to provide quality patient care across the United States.

This article chronicles the history of nursing and how expanding the role of nurses was initially justified. It discusses how this expanded role eventually became the role of the nurse practitioner that we are familiar with today.

I found this article to be completely fascinating as I was almost completely unaware of the history of the APRN role. I have heard of Lillian Wald and Mary Breckenridge but didn't know the full extent of their backgrounds. I enjoyed the history provided in this article and feel that it was a perfect reading to begin my APRN transition. It provided some important information on how the role of the nurse changed and was diminished by ourselves (ANA) before being re-expanded in the 1960s with the role of the NP. I'd love to hear what others think and if you have other interesting historical articles on the background of the APRN role!
