  This article discusses the results of multiple trials using interventions to decrease hospital readmission rates. Interventions included post discharge follow-up phone calls, home visits, and information relocated to the patient's primary care provider. While the evidence is not yet sufficient to come to a verdict, results of these trials found that nurse practitioner led interventions may have the potential to decrease hospital readmission.
