Hi Paula thanks for sharing this. The difficulty is that the more this is set up as a blame culture the more difficult it becomes for people be honest and open about the mistakes that occur. Because we are driven by being seen to be successful to admit there are problems is problematic in our capitalist target driven culture. This leads to depersonalisation and turning away from real human suffering thus poor care will continue sadly
