  • Every day, it seems, some verifiably intelligent person tells us that we don’t know what consciousness is. The nature of consciousness, they say, is an awesome mystery. It’s the ultimate hard problem. The current Wikipedia entry is typical: Consciousness “is the most mysterious aspect of our lives”; philosophers “have struggled to comprehend the nature of consciousness.”
  • I find this odd because we know exactly what consciousness is — where by “consciousness” I mean what most people mean in this debate: experience of any kind whatever. It’s the most familiar thing there is, whether it’s experience of emotion, pain, understanding what someone is saying, seeing, hearing, touching, tasting or feeling. It is in fact the only thing in the universe whose ultimate intrinsic nature we can claim to know. It is utterly unmysterious.

  • By all accounts, these programmers turned entrepreneurs believed their lofty words and were at first indifferent to getting rich from their ideas. A 1998 paper by Sergey Brin and Larry Page, then computer-science graduate students at Stanford, stressed the social benefits of their new search engine, Google, which would be open to the scrutiny of other researchers and wouldn’t be advertising-driven.

  • Emoji are now used in around half of every sentence on sites like Instagram, and Facebook looks set to introduce them alongside the famous “like” button as a way of expression your reaction to a post.
  • If you were to believe the headlines, this is just the tipping point: some outlets have claimed that emoji are an emerging language that could soon compete with English in global usage. To many, this would be an exciting evolution of the way we communicate; to others, it is linguistic Armageddon.

And what do they help us to express that words alone can’t say?When emoji appear with text, they often supplement or enhance the writing. This is similar to gestures that appear along with speech. Emoji serve this function too – for instance, adding a kissy or winking face can disambiguate whether a statement is flirtatiously teasing or just plain mean.

  • Soon after my arrival at the Transgender Center, I was struck by the lack of formal protocols for treatment. The center’s physician co-directors were essentially the sole authority.
  • At first, the patient population was tipped toward what used to be the “traditional” instance of a child with gender dysphoria: a boy, often quite young, who wanted to present as—who wanted to be—a girl.

  • The girls who came to us had many comorbidities: depression, anxiety, ADHD, eating disorders, obesity. Many were diagnosed with autism, or had autism-like symptoms. A report last year on a British pediatric transgender center found that about one-third of the patients referred there were on the autism spectrum.
  • This concerned me, but didn’t feel I was in the position to sound some kind of alarm back then. There was a team of about eight of us, and only one other person brought up the kinds of questions I had. Anyone who raised doubts ran the risk of being called a transphobe.

The next stop was a single visit to the endocrinologist for a testosterone prescription. 
  • When a female takes testosterone, the profound and permanent effects of the hormone can be seen in a matter of months. Voices drop, beards sprout, body fat is redistributed. Sexual interest explodes, aggression increases, and mood can be unpredictable. Our patients were told about some side effects, including sterility. But after working at the center, I came to believe that teenagers are simply not capable of fully grasping what it means to make the decision to become infertile while still a minor.
  • Many encounters with patients emphasized to me how little these young people understood the profound impacts changing gender would have on their bodies and minds. But the center downplayed the negative consequences, and emphasized the need for transition. As the center’s website said, “Left untreated, gender dysphoria has any number of consequences, from self-harm to suicide. But when you take away the gender dysphoria by allowing a child to be who he or she is, we’re noticing that goes away. The studies we have show these kids often wind up functioning psychosocially as well as or better than their peers.” 
  • Frequently, our patients declared they had disorders that no one believed they had. We had patients who said they had Tourette syndrome (but they didn’t); that they had tic disorders (but they didn’t); that they had multiple personalities (but they didn’t).
  • Here’s an example. On Friday, May 1, 2020, a colleague emailed me about a 15-year-old male patient: “Oh dear. I am concerned that [the patient] does not understand what Bicalutamide does.” I responded: “I don’t think that we start anything honestly right now.”

He was sent to another unit of the hospital for evaluation and immediately taken off the drug. But clinics like the one where I worked are creating a whole cohort of kids with atypical genitals—and most of these teens haven’t even had sex yet. That in 2012, in other words, demographics would trump economics.

I think it is interesting that an area that is about the environment, and therefore I associate with science, can have such big debates included within it. It seems here that some of Trump's administration does not believe in climate change, so it will also be interesting to see what happens in this area with the country, especially if signs of it become more pronounced.

The people I met were mostly unemployed, fired for their transgressions, and they seemed broken somehow — deeply confused and traumatized.

  • The offendee, it seems to me, would have done herself and others a favor by addressing the issue directly with him. Why the need to bypass any direct communication when you can post it and shame the person for the world? That's the act of a coward and someone who's out to punish.
