Review of Cognitive Assessments for Older Adults - 1 views

    The aim of this study is to identify and evaluate the psychometric properties of cognitive assessments for older adults. Assessments were categorized based on length of time to assess, less than or more than 30 minutes and body function versus activity or participation. The authors reviewed evidence to determine the strength of each assessment (validity, reliability, sensitivity, specificity) and the amount of literature supporting the use of these cognitive assessments for older adults. The following assessments demonstrated the best rigor for psychometric properties for cognitive assessment with older adults: for body function(under 30 minutes): the MMSE and 3MS, for body function (over 30 minutes): the Cognistat and CASE/Pecpa, and for the activity and participation level: the AMPS.

The Relationship Between the Assessment of Motor and Process Skills (AMPS) and the Larg... - 1 views

    The study examined two assessments, one an occupation based assessment (AMPS) and body function assessment (LACL). 30 clients who recently experienced a stroke took both assessments at home. Two groups of participants were formed, one group took the AMPS first and the other group took the LACLS first. Subjects were allowed to choose tasks that were known to them from the AMPS. Data was analyzed to assess the ability for the AMPS and LACLS to predict independence with ADLs and subsequently IADLs. The results confirmed the hypothesis of this study: a moderate positive correlation was found between the LACL and both the motor and the process scales of the AMPS, suggesting that although correlations between the tests were found, they do not fully examine the same concepts. The results also indicated moderate positive correlation between the LACL and the AMPS, yet there were discrepancies between the two tools in the determination of independence in instrumental activities of daily living (IADL). Clients who were found to be independent by one assessment were found to be dependent by the other. The authors suggest the use of these tools in conjuction with observation of ADLs and IADLs to predict independence levels. The authors also identified areas of each tool that was assessed in conjunction with predicting levels of independence with focus directed at judgement and comprehension to predict functional performance as supported by previous literature (Fong, Chan, and Au, 2001).

    The results of this current research support the SMAF as a suitable assessment for use with older adults with moderate construct validity and responsiveness and a clear minimal detectable change of five, being defined. The SMAF focuses on activity limitation experienced in everyday activities, mobility, mental functions and communication. Any limitations in activities/ abilities are then considered in reference to available resources to determine if any handicap exists. Another assessment that may be suitable for use with older adults, especially as an outcome measure in community populations of older people, is the AMPS with moderate evidence for construct validity and responsiveness to change. Assessments were often found to have been modified to suit the needs of the clinical environment or to shorten the tool, but the actual modifications were poorly reported in the papers.Once the assessment is modified, the assessment can no longer be assumed to hold equivalent measurement properties to the original. The lack of information regarding the assessment tools measurement properties signals the need for further research including validation in older populations.

    This article discusses a new method for evaluating students' competencies and performances during clinic experiences. Due to the increasing demand for higher education and increasing clinical experiences for medical professions, more and more student are required to participate in on-site learning experiences in order to obtain their degree. Unfortunately, there is a lack of literature to support assessments to accurately determine occupational therapy students' level of learning and progress during on-site teaching scenarios. The author of this article discusses an in-depth analysis of a new assessment developed for occupational therapy on-site clinical experiences. The assessment includes a five-point scale with 42 items in six domains. These domains include communication, documentation, safety and judgment, evaluation, intervention and professional behaviors. The five points range from dependent to self-directed. Professional literature and reasoning for domain and scale development are included in the article. The author concludes by stating that further research development and psychometric development needs to be completed in order to effectively incorporate the assessment into current clinical sites. The authors encourage occupational therapy programs to use the assessment and collaborate with the authors to further contribute to these areas of development.

    This is an ADL assessment we're considering to use at my facility to help with "standardizing" our outcomes. The hospital in which we receive most of our referrals uses this assessment...the carryover would benefit to document progress throughout the therapy process. The article itself examines the accuracy of this assessment for discharge planning and concludes the accuracy is good/fair.

    This assessment is based on observation of a patient in a more natural environment/setting and looks at if they initiate expected activities such as, turning off appliances, closing doors when they leave, doing dishes, prepping dinner.. etc within 15 min of expected time. This is looked at and recorded each day of the week. In acute care this is just not very applicable, as we don't watch patients all day, every day of the week and the Vets are in an unfamiliar setting where they likely won't initiate such activities even if cognition is unimpaired. Even the activities that do occur in the hospital setting, such as eating lunch, don't apply very well because the score is acquired by adding up how they did each day of the week and it is not very practical to see a patient every day for the entirety of their lunch for a week straight. This may be a nice assessment for the nursing home to use in some instances.

This article discusses the importance of utilizing a departmental assessment system in practice for new employees and students that is composed of orientation, education, as well as assessment and evaluation. Through an orientation, students and new employees would complete a self-analysis of their known strengths and weaknesses as well as gain knowledge regarding the department's values and expectations. The educational portion of the departmental assessment not only reinforces the importance of CEUs, but also promotes seeking additional training and education in the employee or student's identified areas of weakness. Lastly, employees and students should be assessed and evaluated on their ability to obtain education in areas of weakness, exhibit good critical thinking skills, and make ethical decisions. Through promotion of ethics in the departmental assessment an employee and student will be able to comprehend that the facility not only cares about productivity, but also providing compassionate care.

    In every scientific profession, practice is constantly evolving in response to new knowledge. However, new knowledge presents challenges for all practitioners, not only in terms of how to stay up-to-date but also in terms of how to deliver the best quality practice. Another issue with the use of brief cognitive assessments is the use of specialized assessments based on the diagnosis typically presents. All presentations are different and therefore this reduces the effectiveness of a screen and attempts to act as a tool to diagnose. There is call in Australia for brief cognitive assessment with supported validity, reliability, sensitivity and specificity to better screen and address deficits. This same call has been made by AOTA.

    This article discusses the challenge OT is facing to validate and justify its services due to health care reform, funding caps, and decreased access to services. This can be difficult as the core of OT lies in evaluating and treating function, however, due to the wide scope of practice defining and measuring function in a unified way can be difficult. For example there are two widely used ways of looking at function: the top-down and the bottom-up approaches. There is also challenges because some facilities rely on "house assessments" versus utilizing standardized assessments. A more unified method of measuring and defining function needs developed in order to show OTs value across the board.

    The MoCA also measures a wider range of abilities including executive functions, a higher level cognitive ability required for the implementation of goal-directed behavior and the process of creating and fulfilling a plan. For these reasons, the MoCA was chosen as the comparator assessment of cognition in this study. The cut off score for the MoCA has widely been research and inconclusive. The original cut off score was 26/30 for normal cognition, however many argue the score is too low. This study suggests a computerized battery (CBS- Cambridge Brain Sciences Battery) for patients scoring 23-26, that have a borderline cognitive impairment in order to classify them as impaired or unimpaired. Five tasks from the original 12 of the CBS were chosen to assess aspects of short-term memory, attention, reasoning, and planning. Results: Only two of the tasks were able to distinguish between impaired and unimpaired. Utilizing these tasks in conjunction with the MoCA provides more information regarding testers that fall in the "borderline" category 23-26. The older adults were accepting of the electronic technology.

    Cognitive impairment is under-recognized and misdiagnosed among community-dwelling older adults. At present, there is no consensus about which cognitive screening tool represents the 'gold standard.' However, one tool that shows promise is the Brief Cognitive Assessment Tool (BCAT), which was originally validated in an assisted living sample and contains a multi-level memory component (e.g. word lists and story recall items) and complex executive functions features (e.g. judgment, set-shifting, and problem-solving). 75 community dwelling adults took the assessment and the MoCA with a cut off score of <26. A cut off score for the BCAT was 45 after utilizing the results of the study to balance sensitivity and specificity at .81 and .8 respectively. In addition scores can be separated to interpret BCAT memory and executive dysfunction component scores.

    This article discusses the two most used cognitive assessment tools and how they help in predicting therapy outcome measures. I did my inservice presentation on the Montreal Cognitive Assessment and the Mini-mental state exam and how the OTs at Grant can use one or the other using clinical judgement as a standardized cognitive assessment with mild TBI, stroke and other patients who present with neuro deficits. The MOCA has been proven to help in identifying mild cognitive deficits that the MMSE is unable to identify.
