Study finds that Testosterone provided a small anti-inflammatory effect in post-menopausal women and men, whereas Estrogen increased macrophage cytokine production in post-menopausal women with elevated LDL.  Now, this study did not differentiate PCOS versus non-PCOS, nor did it look at the effects of adiposity in these hormonal effects in women.  Both of which, will effect the outcome.

Women with PCOS have increased inflammation, CRP, compared to women without PCOS.

Study finds that increasing body fat and associated insulin resistance in women with PCOS is associated with an increase in CRP. These women with PCOS are hyperandrogenic. It makes absolutely no sense to give these women more Testosterone.

Chinese men with low T, as defined by serum T levels < 250 ng/dl, is associated with higher gleason scores in prostate disease.

study finds high incidence of prostate cancer in those with low Testosterone at the time of biopsy for HGPIN. What is also interesting, is that the authors found a higher SHBG.  It is well known that Estradiol increases SHBG.  It is also well known that Testosterone to Estradiol in the prostate plays a role in prostate cancer.

low Testosterone in men at the time a diagnosis of prostate is found to be associated with a poor prognosis in the Prostate cancer.

retrospective study found low Testosterone levels associated with increased mortality in male veterans.

increased ADMA is associated with a proatherogenic environment in men.  Testosterone therapy was found to be associated with a reduction in ADMA in men.  This is one mechanism of Testosterone therapy and reduced CVD.

Testosterone therapy in men with low T found to reduce glucose, aid loss of abdominal adiposity, and reduced hsCRP in men with metabolic syndrome.  This study followed therapy for 24 months.  The authors of this study state that Testosterone levels need to be restored to that of a normal range for young adults to reduce CVD.

3 months of IM Testosterone found to improve glycemic control, improved insulin sensitivity, improved cholesterol, and aided weight loss in men with low T and type II diabetes.

In men with low T as defined by serum TT < 250 ng/dl, Testosterone therapy was found to provide a 39% reduction in mortality.  That is profound!

Increasing Estradiol is associated with an increasing carotid IMT in men.

Study finds that low Total T and "bioavailable", also known as free, Testosterone  is associated with an increase in carotid intima-media thickness, which is a marker of CVD.  Higher CRP increased the IMT in those with low T.

Life extesnsions rebuttal to recent JAMA study.  There was several significant flaws in that study and thus limited evidence can be gained by that study.  In fact, I find no useful clinical information from that study. In the rebuttal, there are flaws. The reference the low serum T after treatment and correctly discuss aromatase activity.  But they fail the mention that the less than optimal serum T is likely the result of the high aromatase activity in these men.  Age, stress, and weight are primary causes of increases estrogen production in these men.  These would be why likely high estrogen production occurred and less than optimal serum T resulted.   And, increased E2 will cause an increase in CRP which can precipitate CVD events.

pubmed review of 33 years finds mix bag on low T. Low T clearly associated with increased hypertension, dyslipidemia, atherosclerosis, thrombosis, cardiac dysfunction, and CV events.  Studies on Testosterone therapy in resolution of these dysfunctions are low.  This article claims there is a lack of evidence.  That is not true.
