    What:
    This training will provide clarification on steps in the transition process that relate to initial evaluation and eligibility determination practices for the transition of young children and their families from IDEA Part C Early Intervention (Birth to age 3) into IDEA Part B special education preschool services. This training is meant to assist Montana Part C providers and Montana school district (Part B) personnel in facilitating a smooth and effective transition for eligible families and children.

Salish Kootenai College: Demonstration Grants for Indian Children Program - 0 views

    Salish Kootenai College (Montana) S299A 120078 The Transitions Project will provide interventions at two points in a child's life - prekindergarten and high school. The Transitions Project will be implemented on the Flathead Reservation in Montana. The preschool component will implement a three and four year old, culturally appropriate active learning High/Scope curriculum program to prepare them for successful entry into kindergarten. This project collaborates with a Bureau funded tribal school - Two Eagle River School, where the Transitions Secondary component will serve high school students to provide an intensive one-on-one college preparatory coaching program.     What:
    MYT is an opportunity for students, parents, teachers, vocational rehabilitation counselors, case managers and others to network, explore resources, and discover pathways to meaningful futures! During the conference, there will be a Real Life Fair with vendors representing education, independent living, recreation, employment, and other supportive services. Presenters will include well known keynotes as well as Montanans who have themselves gone through the transition process and can offer insight, guidance and support. A youth track will also be provided to start youth on their own individualized transition journey.

Input Needed from Family Members in Montana: Money Follows the Person Survey - 0 views

    What:
    DPHHS is applying for a Money Follows the Person Grant. The "Money Follows the Person" Rebalancing Demonstration Program (MFP) helps States rebalance their long-term care systems to transition people with Medicaid from institutions to the community. Forty-three States and the District of Columbia have implemented MFP Programs.  From spring 2008 through December 2010, nearly 12,000 people have transitioned back into the community through MFP Programs. The Affordable Care Act of 2010 strengthens and expands the "Money Follows the Person" Program to more States. Click here to fill out the survey. MFP Program Goals Increase the use of home and community-based services (HCBS) and reduce the use of institutionally-based services. Eliminate barriers in State law, State Medicaid plans, and State budgets that restrict the use of Medicaid funds to let people get long-term care in the settings of their choice. Strengthen the ability of Medicaid programs to provide HCBS to people who choose to transition out of institutions. Put procedures in place to provide quality assurance and improvement of HCBS. Transition from High School - Webinar - Multiple Dates - 0 views

  • The OPI-Division of Special Education will be offering a series of 5 webinars on secondary transition.  Providing great secondary transition services to high school youth can be challenging.  This webinar series, presented by Montana teachers and providers, will give multiple strategies on implementing secondary transition in a rural district or a large district that has provided great results for the Montana school districts that have used them.  A flyer with descriptions of each of the webinars can be found at: http://www.opi.mt.gov/pdf/SpecED/Link/10NovWebinarBestPractices.pdf The webinars are free and no registration is required. Secondary Transition Para-Academy - Billings/Missoula - Jan. 11-12, 2011 / Feb. 10-11, ... - 0 views

  • The Division of Special Education and the Comprehensive System of Personnel Development (CSPD) are offering a new para-educator academy on Secondary Transition. Two in-person workshops will occur this year.  In addition, there will be a follow-up webinar.  Secondary transition education settings require that the para-educators assist the certified teacher with a number of instructional and employment-related activities such as: implementing school and community-based instruction; implementing employment sampling and job trainings programs; and monitoring and collecting instructional data. This transition academy is designed to provide participants with the tools to effectively work as a member of a team in a transition setting.
