Inadequate end-of-life and palliative care (EOLPC) health literacy is a significant barrier to receiving high-quality care for individuals living with serious, advanced illnesses. Palliative care includes patient and family-centered care that seeks to anticipate, prevent, relieve or reduce disease-related symptoms across the continuum of a patient's illness. Palliative care may integrate the emotional, psychological, social, and physical aspects of care with a focus on enhanced quality of life. Historically, palliative care referred to treatment available to patients at home and enrolled in hospice. More recently, palliative care has become available to acutely ill patients and its meaning has evolved to encompass comprehensive care that may be provided along with disease-specific, life-prolonging treatment. End-of-life care refers to care that meets the patient's medical, physical, psychological, spiritual and social needs when facing an advanced, life-limiting illness. Hospice care is an end-of-life care delivery system that emphasizes comfort through symptom management and psychosocial support without life-prolonging treatment, to enhance the quality of life, increase communication, and decrease care burden for both patients with a limited life expectancy and their families. Purpose This Funding Opportunity Announcement (FOA) invites R01 grant applications for funding health services research that improves the quality of care and patient outcomes during transitions of care. The purpose of this FOA is to support large-scale research projects that rigorously test promising interventions aimed at improving communication and coordination during care transitions. Background Transitions of care are the movements of patients between providers or clinical settings which typically occur when primary care providers refer patients to specialty care, or when patients are discharged from the hospital to subsequent care settings. During care transitions, critical information aimed to improve the patient's condition and health outcomes needs to be accurately communicated and coordinated between health professionals, the patient, and the family to ensure that safe, high-quality care is provided and care continuity is maintained.

This notice announces the opportunity to apply for funding under the Rural Health Care Services Outreach Program (Outreach Program). The Outreach Program is a community-based grant program aimed towards promoting rural health care services by enhancing health care delivery to rural underserved populations in the local community or region. Through consortia of local health care and social service providers, communities can develop innovative approaches to challenges related to the specific health needs in rural areas that expand clinical and service capacity in rural communities. The overarching goals for the Outreach Program are to: * Expand the delivery of health care services to include new and enhanced services exclusively in rural communities; * Deliver health care services through a strong consortium, in which every consortium member organization is actively involved and engaged in the planning and delivery of services; * Utilize community engagement and evidence-based or innovative, evidence informed model(s) in the delivery of health care services; and * Improve population health, and demonstrate health outcomes and sustainability In addition to funding Outreach programs through the Regular Outreach track, in FY 21, FORHP will also afford applicants a unique opportunity to take part in a national effort that targets rural health disparities through a second track called the "Healthy Rural Hometown Initiative."

The intent of the PFCRA is to target the emerging need to provide extended trauma care prior to reaching a location that can provide definitive hemorrhage and contamination control. Trauma care during this period is often called "Prolonged Field Care" (PFC). Traditionally, improvements to the trauma care system have focused on shortening evacuation times from the point of injury to the first surgical site. However, in future conflicts or mass trauma events, it is anticipated that the initial evacuation time, and thus initial surgical hemorrhage and contamination control, may be delayed for hours or days.This challenge also requires research to develop new solutions to provide for prolonged Damage Control Resuscitation (pDCR) including: support for medical providers in the out-of-hospital setting (point of injury, austere environment, or en route care) with limited resources; understanding the physiologic impact of pDCR; and techniques to mitigate the negative effects of delayed surgical intervention. The research and solutions must be focused on patient-level interventions and outcomes, rather than the broader trauma system. However, proposed research and solutions should consider the entire continuum of trauma care.The JPC-6/CCCRP has identified three overarching Focus Areas for funding under this Program Announcement/Funding Opportunity.

The goal of the Hillman Innovations in Care Program is to advance leading edge, nursing-driven models of care that will improve the health and health care of vulnerable populations, including the economically disadvantaged, racial and ethnic minorities, LGBTQ people, the homeless, rural populations, and other groups that encounter barriers to accessing quality health care services. We seek bold, evidence-based, patient- and family-centered approaches that challenge conventional strategies, improve health outcomes, lower costs, and show potential for national replicability. The Rita & Alex Hillman Foundation specifically seeks proposals that address the health care needs of vulnerable populations in the following areas: Maternal and Child Health Care of the Older Adult Chronic Illness Management

CMF works to improve health care outcomes through grant funding of: * Programs and initiatives that improve patient safety and quality of care through education and training of health care professionals or through systems changes and improvements; * Pilot programs that are designed to reduce medical errors; and * Development and implementation of checklists and other tools to improve disease management and transitions in care. The 2021 focus area is fragmentation across care settings. Breakdowns in care from a fragmented healthcare system can lead to readmissions, missed diagnoses, medication errors, delayed treatment, duplicative testing and procedures, and reduction in quality of care leading to general patient and provider dissatisfaction. Healthcare organizations must collaborate with each other and decide how to transfer information about patients they share. For example, health information technology can help providers share information about their patients. Organizations must devise strategies to "close the loop" and ensure that providers are aware of and address actionable patient information, such as test results.

Nurses stand at the eye of the storm that is our current health-care crisis. It is a unique vantage, demanding an intimacy not only with the difficulties-and rewards-of hands-on patient care, but with the challenges presented to patients and their caregivers by economic uncertainty and irresolute public policy.  In response, nurses are leading innovative efforts to improve care in hospitals, homes, clinics and communities-wherever patients are in need. The Rita & Alex Hillman Foundation cultivates nurse leaders, supports nursing research and innovations, and disseminates new models of care-all critical to transforming our health-care system into one that is more patient-centered, accessible, equitable, and affordable; one that delivers the high-quality care patients need and deserve.

This notice announces the opportunity to apply for funding under the Rural Telementoring Training Center (RTTC) Program. The purpose of this program is to train academic medical centers and other centers of excellence (COEs) to create or expand technology-enabled collaborative learning and capacity building models (such as Project ECHO, ECHO-like models, and other emerging models in the field). The RTTC will develop and share freely accessible tools and resources that are adaptable to culturally and regionally diverse populations to provide training nationwide to facilitate the dissemination of best practice specialty care to primary care providers and care teams in rural and underserved areas. This cooperative agreement aligns with HRSA's goals of fostering a health care workforce to address current and emerging needs, improving access to quality health services, achieving health equity, and enhancing population health. This RTTC's focus is to train and support academic medical centers and other centers of excellence to provide technology-enabled telementoring that will ultimately better equip the health care workforce in rural communities to meet critical demands while improving access to health care.

This notice solicits applications for the Nurse Education, Practice, Quality and Retention (NEPQR) - Registered Nurses in Primary Care (RNPC) Training Program. The purpose of this four-year training program is to recruit and train nursing students and current registered nurses (RNs) to practice to the full scope of their license in community-based primary care teams to increase access to care, with an emphasis on chronic disease prevention and control, including mental health and substance use conditions. The program aims to achieve a sustainable primary care nursing workforce equipped with the competencies necessary to address pressing national public health issues, even the distribution of the nursing workforce, improve access to care and improve population health outcomes by strengthening the capacity for basic nurse education and practice and addressing national nursing needs under three priority areas: education, practice and retention, as authorized by PHS Act sections 831(a)-(c) and 831A(a)-(c).

Through the PA-16-421 (R18) grant program, AHRQ addressed "patient safety and medication research by focusing on the safe usage of medications". This perspective centers on how medications move through the health care system and how this systemic process can be improved so that patients are not harmed, while health care delivery is improved. In addition, this program encouraged "the involvement of all members of the health care team, especially patients and families; nurses, pharmacists, technicians (pharmacy and medication administration technicians), health care administrators, risk managers, and physicians across all settings of care (including in the home)". Through this mechanism, AHRQ funded "investigative research demonstration projects that examine the effective implementation of processes, policies, and behaviors that support safe use of medication as well as its sustainment and dissemination."

This funding opportunity announcement (FOA) seeks to stimulate research that tests optimal end-of-life and palliative care (EOLPC) interventions/models of care that are based on individual- and family-centered outcomes. The testing of EOLPC interventions and models of care are urgently needed that address racial, ethnic and/or cultural diversity in children and adults for those with serious, advanced illness. Trials are needed to test efficacy and effectiveness of these interventions and/or models of care.

This FOA aims to increase testing of populations most affected by chronic hepatitis B and/or hepatitis C; and training of health care providers. Part A is Networking & Capacity Building and Part B is Professional Education & Training. Part A strategies & activities should focus on leading and growing an existing national coalition that is geographically diverse, is in at least different 10 states; and whose members are actively involved in viral hepatitis. The aim is to share strategies, materials, and lessons learned among coalition members; provide technical assistance to conduct culturally competent educational outreach; build the capacity to test the target populations for chronic viral hepatitis; and engage members in implementing CDC’s national viral hepatitis educational campaign(s).Outcomes for Part A are: increased number and capacity of coalition members to deliver culturally competent educational outreach to target population; increased numbers of individuals tested for chronic viral hepatitis; and increased exposure to CDC’s national education campaign. Part B activities are: developing free, accurate, up-to-date, CE-accredited comprehensive web-based, on-demand hepatitis materials, resources and trainings for health care professionals. The outcomes for part B are increasing the knowledge and skills of health care professionals, including the ability to screen, diagnose, medically manage, and provide care to people with chronic viral hepatitis.

The Rita and Alex Hillman Foundation supports nurse-led healthcare innovation and research, with the goal of improving patient care in hospitals, clinics, homes, and communities. Nurses operate at the junction of public policy, medical science, systems-based healthcare delivery, and one-on-one patient care. Drawing on its knowledge of this unique and important role, the foundation believes it has valuable insights with respect to relevant modes of research and alternative models that can lead to more equitable, patient-centered health care. To that end, the foundation is accepting applications for its Hillman Emergent Innovation Serious Illness and End of Life Emergent Innovation program. Through the program, up to five grants of $50,000 will be awarded in support of pre-evidence or untested innovations that challenge traditional approaches to serious illness and end-of-life care and lead to bold, nursing-driven interventions targeting the unmet needs of vulnerable populations, including the economically disadvantaged, racial and ethnic minorities, LGBTQ people, the homeless, rural populations, and others.

Purpose The purpose of the Sigma/Hospice and Palliative Nurses Foundation End of Life Nursing Care research grant is to encourage qualified nurses to contribute to the advancement of nursing care through research. Proposals for pilot and/or developmental research may be submitted for the grant. Research Funding Funds for this grant are provided jointly by Hospice and Palliative Nurses Foundation End of Life Nursing Care and Sigma Foundation for Nursing. Funds for this grant do not cover expenses incurred prior to the funding date.

The award program, which was created in 2004 to acknowledge nurses who have made a significant contribution to nursing education or practice through the development and/or adoption of a new technology, offers a $10,000 prize to an educator or practicing health professional whose innovation leads to improved patient care and patient care outcomes, and to an educator or practicing health professional whose innovation leads to improved health professions education and student outcomes. Entries will be judged on the technology's innovativeness and its impact on nursing education or direct patient care. The innovation must be new and be in use for six months or longer prior to submission of application. Applications must be submitted under one of the following categories: 1) Patient Care: improving efficiency of healthcare delivery, preventing/reducing errors, and improving outcomes, 2) Healthcare Education (both didactic and clinical): innovation in curricular delivery methods and improving student competency and learning outcomes.

The AABC Foundation, was established in 1994 by American Association of Birth Centers to advance public knowledge and understanding of issues related to family-centered childbirth. The foundation raises funds to support the advancement of the AABC birth center model as the standard to improve maternity and women's health care. Based on wellness model of pregnancy and birth, the birth center is a homelike facility, operating with a healthcare system. Birth centers are guided by principles of prevention of complication, sensitivity to the needs of the individual family, safety for both mother and baby, appropriate use of medical intervention, and cost effective care. Birth centers provide family-centered care for healthy women before, during, and after normal pregnancy, labor, and birth. To advance its mission, the foundation will award grants of up to $5,000 to support research projects on issues related to the birth center model of care. The program is open to individuals or organizations and nonprofit status or sponsorship is not required. However, applicants must demonstrate capacity and a plan to complete the research in accordance with the project's timeline.

In an effort to support cardiovascular quality and performance improvement, the ACC provides funding opportunities that support future cardiovascular quality initiatives (QI) or performance improvement (PI), particularly those that highlight the importance of accreditation. Funding to support these projects is made possible by the Society of Cardiovascular Patient Care (SCPC), which was a nonprofit organization dedicated to improving the care and outcomes of patients worldwide with suspected acute coronary syndrome, heart failure, and atrial fibrillation through innovative cross-disciplinary processes. Its merger with the American College of Cardiology Foundation (ACCF) in 2016 provided an opportunity to leverage the accreditation strength of the SCPC into the large portfolio of services the ACCF offers. To preserve the legacy of the SCPC, an endowment was created to support future cardiovascular QI or PI, particularly those efforts that highlight the importance of accreditation. This is an invitation for collaborative proposals which focus on addressing the challenges Primary Care Providers experience in serving Adults with Intellectual and Developmental Disabilities during pandemic and/or emergency situations. Proposals should use one of the following approaches: Educational materials: Develop additional educational resources related to pandemic and/or emergency response for Primary Care Providers who serve adults with I/DD Implementation: Supports implementation of models, i.e. projects that support community-based PCPs in providing care to adults with I/DD during pandemic and/or emergency situations Research: Supports regional or national research related to Primary Care Providers who serve adults with I/DD role during pandemic and emergency situations A secondary goal of this RFP is to foster new partnerships or significantly enhance existing partnerships between disability organizations, advocates, community organizations, and healthcare providers. Grants of up to $50,000 (each) for a 1-year period will be awarded.

Stark disparities in prematurity are the primary contributor to widening disparities in Ohio's infant mortality rate. Current models of care have not resulted in reductions in disparities in prematurity, suggesting an alternative care model is necessary. The model should address both health and social factors that contribute to adverse birth outcomes. The results of this project will be used to create an evidence-based program that can be implemented throughout the State to reduce disparities in prematurity. The objective of this project is to solicit professional services to develop and test an Ohio-specific group prenatal care model and determine impact of the model on reducing disparities in prematurity among Ohio's high-risk women. The purpose of this funding opportunity is to stimulate research aimed at determining needs and best practices for the integration of palliative care into home and community settings. Home and community in this FOA refer to the place where an individual resides or lives. Home- and community-based palliative care programs ensure those with serious, advanced illness who do not require hospitalization but are not appropriate for hospice have access to high quality end-of-life and palliative care.
