The purpose of this FOA is to promote the creation of developing research centers that can build capacity to study high priority areas of cancer control implementation science, build implementation laboratories, improve the state of measurement and methods, and improve the adoption, implementation, and sustainment of evidence-based cancer control interventions. Specifically, this FOA targets the following areas designated as scientific priorities by the Blue Ribbon Panel (BRP): Prevention and Screening: Implementation of Evidence-based Approaches; Symptom Management, Prevention and Screening: High-Risk Cancers and other cross-cutting Moonshot priorities. The CDC, Division of Cancer Prevention and Control (DCPC) announces the availability of Fiscal Year 2019 funds to implement DP19-1905, The CDC Cancer Genomics Program: Translating Research into Public Health Practice. This program addresses the utilization of cancer genetic services as a strategy for cancer prevention and control. The primary outcomes of this NOFO are to increase screening of cancer family history and to increase the use of genetic counseling and testing for Hereditary Breast and Ovarian Cancer (HBOC) and Lynch Syndrome (LS) among individuals meeting recommendations for referral. This funding opportunity supports the implementation and evaluation of public health activities within four core strategies to build the evidence base and disseminate best practices. These priorities will be accomplished by funding up to 5 awardees to develop, implement, and evaluate state-level programs in cancer genomics. Funding will support applicants that demonstrate: Appropriate experience with surveillance systems, partners and similar work in cancer genomics Appropriate organizational capacity to perform, implement, and evaluate activities and strategies An approach that is consistent with the strategies and outcomes described in the NOFO An ability to collect data towards key performance measures and outcomes Innovative methods and approaches to program and strategy implementation

This announcement provides funds to recipients to partner with health systems and individual primary care clinics to implement evidence-based interventions (EBIs) to increase colorectal cancer (CRC) screening uptake among applicant defined populations age 50-75 years that have CRC screening rates lower than the national, regional, or local rate. Recipients will: 1) establish partnerships with health systems and primary care clinics to implement at least two of four EBIs recommended in The Community Guide (client reminders; provider reminders, reduction of structural barriers, and provider assessment and feedback); 2) establish partnerships with organizations that provide expertise to support the implementation of EBIs in primary care clinics; 3) conduct a formal assessment of each clinics capacity/readiness to implement EBIs; 4) utilize the clinic assessment to select appropriate EBIs to implement; 5) provide resources to partner clinics to provide and support completion of follow-up colonoscopies after a positive or abnormal screening test; and 6) collect and submit high-quality clinic-level data including baseline and annual CRC screening rates.

The UG3/UH3 projects in response to this FOA must apply for both the UG3 and UH3 phases together in the single application. The goal of the Pancreatic Cancer Action Network-AACR Research Acceleration Network (RAN) Grants is to provide funding and strategic assistance to research projects with the potential to double survival for pancreatic cancer by the year 2020. These projects must be implemented by a multi-institutional team and include a clinical component with an endpoint relevant to improving the detection or treatment of pancreatic cancer. The RAN Grants will provide $1,000,000 in funding for direct and indirect expenses related to the research project over a one- to three-year time frame. In addition to the grant, the Pancreatic Cancer Action Network will provide strategic project management support to help optimize project implementation and progress during the grant term.

This Funding Opportunity Announcement encourages research projects focused on the process of effective communication of research findings about breast cancer and the environment. A central objective of this new initiative is to ensure that risk messaging is designed for dissemination and effectively implemented, and leads to individual behavior change or policy change.The ultimate goal is to provide the development, dissemination, and implementation communication-based prevention efforts to reduce the risk for breast cancer from environmental factors.

The proposed program's purpose is to increase the availability of supportive services for young breast cancer survivors (YBCS), young women with metastatic breast cancer (mBC), and their caregivers and families by supporting organizations and entities that serve this population. The awardees will address strategies relevant to the target audience. These include: the establishment of a network of survivors and caregivers to facilitate policy, system, and environmental change interventions that increases access to lifestyle programs, clinical preventive services, and cancer care among survivors; development and implementation of innovative technological educational opportunities for health care providers on topics relevant to the delivery of appropriate treatment and care of YBCS and mBC; and utilization of patient navigation and community health worker program strategies to reach underserved populations to enhance their access to and utilization of YBCS services and programs. To this end, activities funded by this NOFO, are intended to support organizations in closing the disparity gap needed to support young breast cancer survivors. Specifically, organizations will implement strategies that aim to reduce disparities observed in survival and quality of life due to race, ethnicity, or other social determinants of health. The FOA encourages two types of applications: 1) observational research that includes pilot testing of intervention to understand and address predictors of cancer care/treatment and outcomes in rural low-income and/or underserved populations; or 2) intervention research to address known predictors of cancer care/treatment and outcomes in rural low-income and/or underserved populations. Specifically, the focus for observational studies (with pilot testing) is understanding and addressing the predictive and/or mediating role of social determinants of health, barriers to care, and treatment; and the focus for interventional research is on addressing quality of care related to cancer diagnosis, treatment and/or survivorship. Most existing cancer control interventions are not ready for direct implementation and dissemination in low-income rural areas, so proposals should seek to develop, adapt, and/or implement, and test interventions.

A cancer survivor is a person diagnosed with cancer, from the time of diagnosis throughout the person’s lifespan. As of 2008, nearly 12 million cancer survivors were living in the United States; this number is expected to increase to 18 million in 2020. Cancer survivors have long-term adverse physical and psychosocial effects from their diagnosis and treatment, and have a greater risk for additional cancer diagnoses compared with persons without a cancer history. Cancer survivors commonly report negative behavioral, medical, and health care access issues that may contribute to poor long-term medical and psychosocial outcomes. An analysis of over 45,000 U.S. cancer survivors showed that: 1) 15% of cancer survivors continue to use tobacco; 2) 20-25% do not receive recommended cancer screenings; 3) 31% do not engage in any leisure time physical activity; 4) 40 to 50% do not receive flu or pneumonia vaccines; 5) 60% do not have a summary of their cancer treatment; and 6) 25% do not have any instructions (written or oral) for their treatment or follow-up care . CDC’s National Comprehensive Cancer Control Program (NCCCP) supports collaborative cancer control and prevention efforts in all states, the District of Columbia, tribal organizations, territories, and Pacific Island jurisdictions to address the cancer burden in their jurisdictions. In 2010, NCCCP developed six priorities areas of focus for the greatest public health impact; one of the six priorities is to address the public health needs of cancer survivors. The purpose of this FOA is to implement a broad set of evidence-based survivorship strategies in a subset of NCCCP grantees that will have the short-term results of increasing knowledge of cancer survivor needs, increasing survivor knowledge of treatment and follow-up care, and increasing provider knowledge of guidelines pertaining to treatment of cancer.
