The Communities Building Healthier Environments for a Stronger Nation Initiative ('Communities Initiative') intends to demonstrate the effectiveness of community-based networks in improving health outcomes among racial and ethnic minority and/or other disadvantaged populations. This program seeks to improve health outcomes through the establishment of integrated networks that collaboratively employ evidence-based disease management and preventive health activities; build the capacity of communities to address social determinants and barriers to healthcare access; and increase access to and utilization of preventive health care, medical treatment, and supportive services. The Communities Initiative specifically targets the unmet healthcare and supportive service needs of racial and ethnic minority populations at highest risk for poorer health outcomes. Health services provided under the Communities Initiative will not be denied to any person based on race, color, or national origin. Populations at highest risk include, but are not limited to, individuals who are newly diagnosed and lack a medical home; individuals who experience difficulty in adhering to a prescribed medical treatment plan; individuals with a chronic disease that is not well managed; and individuals that are unstably housed. Specific health areas to be addressed by the Communities Initiative include asthma, cardiovascular disease, diabetes, HIV/AIDS, Hepatitis B or C, obesity/overweight, and mental disorders. Community health programs are required to address social determinants of health, and improve coordination of health, social, and supportive services to significantly improve health outcomes among minority and/or disadvantaged communities. Applicants must choose two but no more than three chronic conditions

The Robert Wood Johnson Foundation Health Policy Fellows program provides the nation's most comprehensive fellowship experience at the nexus of health science, policy and politics in Washington, D.C. It is an outstanding opportunity for exceptional midcareer health professionals and behavioral and social scientists with an interest in health and health care policy promoting the health of the nation. Fellows participate in the policy process at the federal level and use that leadership experience to improve health, health care and health policy. The fellowship requires, at a minimum, a 12-month residential experience in Washington, D.C., with additional support for health policy leadership development activities. The program will select up to six fellows.

This Funding Opportunity Announcement (FOA) invites applications for the next 5-year cycle of the National Longitudinal Study of Adolescent to Adult Health (Add Health) to include a sixth wave of data collection (Wave VI). Add Health is a nationally representative, longitudinal study of individuals primarily born from 1976 through 1982 who were first interviewed as adolescents in grades 7-12 (ages 12-19) in 1994-1995. Add Health respondents are now entering middle age. The goals for Add Health Wave VI are to: Re-interview Add Health cohort members in a combination of web-based and in-person modes, including aggressive non-response follow-up and oversamples of race/ethnic-minority and low-socioeconomic-status individuals. Re-visit cohort members for an in-home health exam that includes venous blood collection. Assay biological specimens for biomarkers of disease. Enrich measures in domains that may elucidate mid- and later-life health and disparities therein (e.g., cumulative stress, discrimination, work-life balance, caregiving). Clean, document, disseminate, archive (including storage of biological specimens for future study), and promote the Wave VI data to the scientific community for aging research.

The purpose of this FOA is to solicit applications for the AD/ADRD Health Care Systems Research Collaboratory ("Collaboratory", henceforth) to improve care for persons with dementia (PWD) and their caregivers through health systems. Health and long-term systems for this FOA are defined broadly and include organizations providing care across settings to include primary and specialty outpatient care, acute inpatient care, skilled nursing and other rehabilitation facilities, residential long-term care, and home and community-based services. Organizations may be traditional health care systems (e.g., health maintenance organizations, or HMOs), health insurance companies, managed care plans, home health care providers, memory clinics, nursing homes, assisted living facilities, outpatient clinics, federally-designated health centers, hospitals, and other providers of acute and long-term care.

Public Health Services and Systems Research (PHSSR) is a multidisciplinary field of study that examines the organization, financing, delivery and quality of public health services within communities and the resulting impact on population health. The National Coordinating Center (NCC) for PHSSR and the Robert Wood Johnson Foundation (RWJF) seek to expand the evidence base for effective decision-making in public health practice and policy through research that responds to the questions defined in the National Agenda for PHSSR: www.publichealthsystems.org/research-agenda.aspx.  This solicitation is intended to strengthen the pool of researchers available to conduct PHSSR and to build on successful principles and models previously demonstrated in public health and health services research. The awards support mentored, intensive career development through funding, educational experiences and protected time to conduct independent research.

The Centers of Excellence on Environmental Health Disparities (EHD) research program is a collaborative effort supported by the National Institute on Minority Health and Health Disparities (NIMHD), the National Institute of Environmental Health Sciences (NIEHS), and the U.S. Environmental Protection Agency (EPA) that encourages basic, biological, clinical, epidemiological, behavioral and/or social scientific investigations of disease conditions that are known to be a significant burden in low socioeconomic and health disparate populations. The centers define environmental health disparities as inequities in illnesses that are mediated by disproportionate exposures associated with the social, natural and built environments. The kick-off meeting will feature presentations from each of the five funded centers highlighting their proposed research.

The Michigan Health Fund currently is accepting applications to its Healthy Aging program, which seeks to improve access to and the availability of integrated, comprehensive services for older adults and their caregivers. To that end, grants ranging from between $100,000 and $500,000 will be awarded in support of strategies and service delivery models focused on improving health outcomes for older adults and those who care for them. Grantmaking categories identified by the foundation include: Health Aging - The fund supports strategies designed to improve the health and well-being of older adults, including the development and piloting of new models aimed at better supporting older adults; the replication and expansion of evidence-based or promising models, the expansion of a current program to a new target population, or the replication of an existing program that does not yet exist in a local community and does not have a funding mechanism available; strategies that improve access to current or expanded services; projects that address the structural barriers and inefficiencies preventing various systems (e.g., education, health, social) from delivering care to older adults; and projects that address a significant issue area (e.g., social isolation) or various health disparities impacting older adults.

This funding opportunity is for competitive grants to be awarded to support systematic testing and documentation of innovative and promising practices that enhance the quality, effectiveness, and proven outcomes of nutrition programs and services within the aging services network. Innovative and promising practice projects awarded through previous INNU announcements have focused on at least one of the following areas; Creating partnerships between Universities and/or other research organizations in collaboration with aging network organizations to develop or test innovative evidence based programs or practices for senior nutrition; Modernizing the congregate and/or home delivered meal program infrastructure, delivery mechanisms, and/or marketing and outreach that can be used by the national aging network to ensure that States are able to maximize the return on their investment in nutrition programs and expanding the reach of the OAA target populations; Enhancing partnerships with health care partners (e.g. oral health, behavioral health, alternative health, and etc.) to further demonstrate the network's value in solving local and national problems, and/or increasing business acumen opportunities and; Advancing the use of technology within the aging and nutrition network Successful awardees will be expected to focus on outcomes including, but not limited to, methods to improve collaboration with local health care entities, decrease health care costs for a specific population, and/or decrease the incidence of the need for institutionalization among older adults.

Research projects applicable to this FOA are those that are either applied or basic in nature (including methodological development), have a human behavioral and/or social science focus, and feature systems science methodologies

This FOA invites Research and Development Center (P30) grant applications in demography, economics and health services research relevant to Alzheimer's Disease and Alzheimer's Related Dementias (AD/ADRD). Areas of focus that are especially encouraged are: a) national and international population trends in cognitive aging and AD/ADRD; b) demography of dementia care and caregiving; c) economic burden of AD/ADRD; d) impact of health care systems and long-term supports and services on outcomes for persons with dementia and their care providers; e) impact of health care financing policies on outcomes for persons with dementia and their care providers; f) how regulatory and economic incentives affect access, quality and health outcomes in health and long-term care systems for persons with dementia; g) disparities in quality and access to dementia care; h) effects of population-level health delivery and care interventions on outcomes of persons with dementia; and i) national and international projections of dementia caseload, incidence and prevalence. Center grant applications must include two mandatory Cores and may choose among three optional Cores. Centers are required to work collaboratively with the Coordinating Center to be funded via RFA-AG-20-003.

Approaches that partner technology-based solutions with biomedical and biobehavioral research are supported by multiple agencies of the federal government including the National Science Foundation (NSF) and the National Institutes of Health (NIH). The purpose of this program is to develop next-generation multidisciplinary science that encourages existing and new research communities to focus on breakthrough ideas in a variety of areas of value to health, such as networking, pervasive computing, advanced analytics, sensor integration, privacy and security, modeling of socio-behavioral and cognitive processes and system and process modeling. Effective solutions must satisfy a multitude of constraints arising from clinical/medical needs, barriers to change, heterogeneity of data, semantic mismatch and limitations of current cyberphysical systems and an aging population.Such solutions demand multidisciplinary teams ready to address issues ranging from fundamental science and engineering to medical and public health practice.

The goal of the Hillman Innovations in Care Program is to advance leading edge, nursing-driven models of care that will improve the health and health care of vulnerable populations, including the economically disadvantaged, racial and ethnic minorities, LGBTQ people, the homeless, rural populations, and other groups that encounter barriers to accessing quality health care services. We seek bold, evidence-based, patient- and family-centered approaches that challenge conventional strategies, improve health outcomes, lower costs, and show potential for national replicability. The Rita & Alex Hillman Foundation specifically seeks proposals that address the health care needs of vulnerable populations in the following areas: Maternal and Child Health Care of the Older Adult Chronic Illness Management

The purpose of this funding opportunity announcement (FOA) is to encourage interdisciplinary research aimed at promoting health, preventing and limiting symptoms and disease, and reducing health disparities across the lifespan for those living or spending time in non-traditional settings (i.e. playgrounds and nursing homes). These settings result in exposure to environmental pollutants and toxins that result in health risks, symptoms, and other health conditions/diseases; including lower respiratory disease, chronic obstructive pulmonary disease, cardiovascular disease, and complex environmental exposures that may be exacerbated by non-chemical stressors encountered in community settings, physiological function of organs and systems of the fetus/child/adolescence, and lower respiratory disease. Risk identification and symptom management include prevention and behavior changes and actions to maintain health and prevent disease with an emphasis on the individual, family, and community which will advance nursing science. For purposes of this FOA, non-traditional settings include, but are not limited to, places such as community centers; pre-school and non-traditional school environments (e.g., churches, daycare, home-based schools, dormitories, alternative schools, and playgrounds); child and older adult foster care facilities; older adult day care facilities; half-way homes; and assisted living and long-term care facilities.

The purpose of this Funding Opportunity Announcement (FOA) is to solicit innovative social, behavioral, health services, and policy research that can directly and demonstrably contribute to the elimination of health disparities. Projects may involve primary data collection or secondary analysis of existing datasets. Projects that examine understudied health conditions; examine the effectiveness of interventions, services, or policies for multiple health disparity populations; and/or directly measure the impact of project activities on levels of health disparities are particularly encouraged.

The purpose of this FOA is to encourage behavioral and social science research on the causes and solutions to health and disabilities disparities in the U. S. population. Health disparities between, on the one hand, racial/ethnic populations, lower socioeconomic classes, and rural residents and, on the other hand, the overall U.S. population are major public health concerns. Emphasis is placed on research in and among three broad areas of action: 1) public policy, 2) health care, and 3) disease/disability prevention. NIHCM Foundation is making up to $150,000 available to support investigator-initiated research with high potential to inform improvements to the U.S. health care system by reducing health spending growth, enhancing quality, and/or expanding access to health insurance and health care services. Projects must advance existing knowledge in the areas of health care financing, delivery, management and/or policy.

This initiative seeks applications that propose to stimulate and expand research in the health of minority men. Specifically, this initiative is intended to: 1) enhance our understanding of the numerous factors (e.g., sociodemographic, community, societal, personal) influencing the health promoting behaviors of racial and ethnic minority males across the life cycle, and 2) encourage applications focusing on the development and testing of culturally and linguistically appropriate health-promoting interventions designed to reduce health disparities among racially and ethnically diverse males age 18 and older.
