The IOM's 1999 landmark report, "To Err is Human," dropped the first bombshell, reporting that between 44,000 and 98,000 Americans die in hospitals each year from medical mistakes, costing an estimated $17 billion to $29 billion annually. HHS' new finding that medical mistakes kill 15,000 Medicare patients a month equates to 180,000 Medicare deaths per year - more than the IOM's estimate, which attempted to cover all patients in the United States. That means that the annual death toll in this country caused by mistakes in hospitals is well over 250,000 deaths a year! But perhaps the most startling finding by HHS is that a significant number of patients suffered injuries or died needlessly, as 44 percent of the medical errors were preventable.

WASHINGTON -- These days, when we think of George W. Bush, we think mostly of what a horrible mess he made of the economy. But his even more tragic legacy is the loss of our moral authority, and the transformation of the United States of America from global champion of human rights into an outlaw nation.

On May 20, the Senate passed its bill to reregulate Wall Street by a vote of 59-39, complete with a (watery) version of the Volcker Rule. The story of the legislation's passage can be told in a number of ways: a tale of conflict or compromise, triumph or capitulation. But on any reading, that story is only the climactic chapter in a larger narrative: how the masters of the money game fell out of love with-and into a state of bitter, seething, hysterical fury toward-Obama. The speed and severity of the swing from enchantment to enmity would be difficult to overstate. When Obama was sworn into office, Democrats on Wall Street rejoiced at the ascension of a president in whom they saw many qualities to admire: brains, composure, bi-partisan instincts, an aversion to class-based combat. And many Wall Street Republicans-after witnessing the horror show that constituted John McCain's response to the financial crisis-quietly admitted relief that the other guy had prevailed.

It's one thing that the United States will soon be taking orders from China (or already is). But what about when we're becoming less forward-thinking than England? That's the only possible reading of the fact that there, the former top drug official Bob Ainsworth has addressed the House of Commons and argued for the legalization of all drugs. Not just pot-all of them. His reasoning is simple, and has nothing to do with the ideology of Timothy Leary: "We need to take effective measures to rob the dealers of their markets and the only way that we can do that is by supplying addicts through the medical profession, through prescription. We cannot afford to be shy about being prepared to do that." He continues: "We spend billions of pounds without preventing the wide availability of drugs. It is time to replace our failed war on drugs with a strict system of legal regulation, to make the world a safer, healthier place, especially for our children. We must take the trade away from organised criminals and hand it to the control of doctors and pharmacists."

There may yet be justice for the victims of the post-9/11 US torture program. Just not in the United States.

The truth is that the United States has been behind the curve not only in Tunisia and Egypt for the last few weeks, but in the entire Middle East for decades. We supported corrupt autocrats as long as they kept oil flowing and weren't too aggressive toward Israel. Even in the last month, we sometimes seemed as out of touch with the region's youth as a Ben Ali or a Mubarak. Recognizing that crafting foreign policy is 1,000 times harder than it looks, let me suggest four lessons to draw from our mistakes:

Today, hundreds of thousands of people comprising a Main Street Movement - a coalition of students, the retired, union workers, public employees, and other middle class Americans - are in the streets, demonstrating against brutal cuts to public services and crackdowns on organized labor being pushed by conservative politicians. These lawmakers that are attacking collective bargaining and cutting necessary services like college tuition aid and health benefits for public workers claim that they have no choice but than to take these actions because both state and federal governments are in debt.

On Friday, the United States stood alone and vetoed an Arab resolution at the United Nations Security Council. The resolution strongly condemned Israeli illegal settlements in the Palestinian occupied territories of the West Bank and East Jerusalem as a "major obstacle to peace". All 14 other members of the UN Security Council voted in favor of the resolution.

This document presents details on the wealth and income distributions in the United States, and explains how we use these two distributions as power indicators. Some of the information may come as a surprise to many people. In fact, I know it will be a surprise and then some, because of a recent study (Norton & Ariely, 2010) showing that most Americans (high income or low income, female or male, young or old, Republican or Democrat) have no idea just how concentrated the wealth distribution actually is. The past three years have been a disaster for most Western economies. The United States has mass long-term unemployment for the first time since the 1930s. Meanwhile, Europe's single currency is coming apart at the seams. How did it all go so wrong?

The United States is the only major nation in the industrialised world that does not guarantee healthcare as a right to its people. Meanwhile, we spend about twice as much per capita on healthcare and, in a wide number of instances, our outcomes are not as good as others that spend far less.

Speaking at the 25th anniversary celebration of the national media watch group Fairness and Accuracy in Reporting, world-renowned political dissident and linguist Noam Chomsky analyzes the U.S. response to the popular uprisings sweeping the Middle East and North Africa. "Across the [Middle East], an overwhelming majority of the population regards the United States as the main threat to their interests," Chomsky says. "The reason is very simple... Plainly, the U.S. and its allies are not going to want governments which are responsive to the will of the people. Of all the topics on which I've focused, I've likely written most about America's two-tiered justice system -- the way in which political and financial elites now enjoy virtually full-scale legal immunity for even the most egregious lawbreaking, while ordinary Americans, especially the poor and racial and ethnic minorities, are subjected to exactly the opposite treatment: the world's largest prison state and most merciless justice system. That full-scale destruction of the rule of law is also the topic of my forthcoming book. But The New York Times this morning has a long article so perfectly illustrating what I mean by "two-tiered justice system" -- and the way in which it obliterates the core covenant of the American Founding: equality before the law -- that it's impossible for me not to highlight it.

We no longer have freedom; there is only the appearance of freedom. We are consumed by an endless and vague war on terror in which the perfidiousness of our enemy, whose number, location and nature are never clearly defined, justifies the shredding of constitutional rights, torture, kidnapping, detentions without charges or trials and an occult-like battle against an absolute evil. And if you think the state intends to limit itself to the persecution of Muslims, especially once there is an increase in domestic unrest and instability, you know little about human history.

The NAACP has just passed a historic resolution demanding an end to the War on Drugs.  The resolution comes as young Black male unemployment hovers near 50 percent and the wealth gap's become a veritable gulf. So why is the forty-year-old "War on Drugs" public enemy number one for the nation's oldest civil rights organization? Well here's why:  it's not extraneous - it's central: the war on drugs is the engine of 21st century discrimination - an engine that has brought Jim Crow into the age of Barack Obama.     Author Michelle Alexander lays out the statistics -- and the stories --  of 21st Century Jim Crow in her ought-to-blow-your-socks off book: "The New Jim Crow: Mass Incarceration in an Age of Colorblindness." I had a chance to sit down with Alexander earlier this summer. We'll be posting the full interview in two parts.     "We have managed decades after the civil rights movement to create something like a caste system in the United States," says Alexander in part one here  "In major urban areas, the majority of African American men are either behind bars, under correctional control or saddled with criminal record and once branded as criminal or a felon, they're trapped for life in 2nd class status."     It's not just about people having a hard time getting ahead and climbing the ladder of success. It's about a rigged system. Sound familiar?  Like the Pew Research Center report on household wealth and the Great Recession -- the NAACP resolution story was a one-day news-blip - despite the fact that it pierces the by-your-bootstraps myth that is at the heart of - you pick it - the deficit, the stimulus, the tax code - every contemporary US economic debate.     White America just maybe ought to pay attention. With more and more Americans falling out of jobs and into debt, criminal records are a whole lot easier to come by than life-sustaining employment.  Contrary to the conventional media version, the "Drug War" story is not a people with problems

By executive order on Aug. 4, President Barack Obama refused entry to the United States of war criminals and human-rights violators (jurist.org, Aug. 4). He ignored, as he often does, the deeply documented factual evidence of war crimes committed by the Bush-Cheney administration along with grim proof that the Obama administration also violates our anti-torture laws and the U.N.
