There has been extensive national media coverage today (August 11) on how medicine shortages have forced patients to visit multiple pharmacies to get their prescriptions filled or return to their GP to be prescribed alternative drugs. These media reports have been based on a survey involving more than 1,500 pharmacists in the UK, in which over a half of those polled said that their patients' health had been put at risk in the last six months. The UK Department of Health and Social Care (DHSC) has signed a new US$10 million (7.8 million pounds) grant agreement with global non-profit organisation, FIND. This extends the UK government's support to FIND through the Global AMR Innovation Fund (GAMRIF) for another four years. The new Hormone Replacement Therapy (HRT) prescription pre-payment certificate (PPC) has witnessed more than 37,000 purchases both via online website and in-person in pharmacies since its launch on Saturday (1 April). As of 12pm today (4 April), 37,240 HRT PPCs have been bought online and 501 in-pharmacy, totalling 37,741 certificates. Department of Health and Social Care (DHSC) estimated women have saved over £1.13 million in prescription charges for the year, with the average person saving £30 for a year's supply. The new prescription pre-payment certificate is also predicted to enable around 400,000 women to have cheaper access to menopause support. After a month of intense talks, the British government and unions representing consultant doctors in England have reached an agreement, potentially ending six months of disruptive strike action. The Department of Health and Social Care (DHSC) has put forward an offer to reform the pay structure for senior doctors from January 2024, reducing the number of pay points and the time it takes to reach the top. As part of this offer, consultants will also be entitled to enhanced shared parental leave, bringing them in line with other NHS staff. New arrangements will ensure a clearer link between pay progression and evidence of skills, competencies and experience, the DHSC said. The British Medical Association (BMA) and Hospital Consultants and Specialists Association (HCSA) agreed to put the offer to their members for a vote in the coming weeks, with no further strike action to be called during that time.

The Department of Health and Social Care aims to amend the Human Medicines Regulations 2012, thereby enabling pharmacy technicians to provide and administer specific medicines through a patient group direction. In line with this initiative, the Department introduced a six-week consultation to assess whether registered pharmacy technicians should be authorised to dispense medicines using a PGD. The proposals, backed by all four Home Nations, apply exclusively to registered pharmacy technicians. Meanwhile, this coverage will extend to include pharmacy technicians in Northern Ireland upon their achievement of registered professional status. "The proposed statutory instrument, presently applicable in England, Scotland, and Wales would offer 'clear benefits,' especially in areas such as vaccination and the provision of oral contraception and EHC," the DHSC said. A further 18 new products have been entered the list of 'Drugs for which Discount is Not Deducted' (DND) in Part II of the Drug Tariff on 1 November, following applications made by the PSNC. This takes the number of products added to the DND list by the Department of Health and Social Care (DHSC) and the NHS Business Services Authority (NHSBSA) in the last 18 months to over 400. The latest list of products include Aciclovir 200mg/5ml oral suspension sugar free, Acitretin 25mg capsules and Strattera 4mg/1ml oral solution. The Pharmaceutical Services Negotiating Committee (PSNC) has refused to accept the Department of Health and Social Care (DHSC)'s decision to get rid of 'transitional payments' from February 2023. The pharmacy negotiator said that 'any reductions in payments at this point will be impossible for community pharmacy contractors to manage financially.' "We are also continuing to be clear with officials and ministers that CPCF funding needs an urgent uplift to help businesses to cope with soaring costs being driven by inflation and the workforce crisis. We put a comprehensive business case to the government for this uplift in the last CPCF negotiations." The latest 'transitional payments decision' by the Department follows the announcement last year that the value of the these payments would be phased down over the second half of 2022/23 and will be based on the latest monitoring and analysis of funding delivery. PSNC says it submitted a fully-costed bid for a 'Pharmacy First' service in its last round of negotiations alongside the case for an uplift to core CPCF funding. Both of these were refused.

The Serious Shortage Protocol (SSP), for Estradot 50mcg patches will expire at 23.59pm on Friday 24 February 2023. Department of Health and Social Care (DHSC) has confirmed that sufficient stock of Estradot 50mcg patches are now available to meet normal demand. "After 24th February, any prescriptions for Estradot 50mcg patches must be dispensed in accordance with the prescription, and SSP048 will no longer be valid for use," said DHSC. NHSBSA advise that contractors must follow the specific endorsement guidance issued with each SSP and endorsements should be clear and unambiguous - NHSBSA processing staff must be able to determine what has been supplied. NHSBSA have published information on common SSP endorsing errors they see when processing claims.

The government's formal consultation on pharmacy supervision, which closes today (29 February), leaves important questions unanswered, the National Pharmacy Association (NPA) has warned. Pharmaceutical Services Negotiating Committee (PSNC) has expressed its support for alterations in the Human Medicines Regulations 2012 to enable original pack dispensing (OPD) and supply of medicines containing sodium valproate by pharmacists. PSNC's support for changes was in response to the consultation on OPD, initiated by the Department of Health and Social Care (DHSC). The consultation had proposed regulatory changes to allow pharmacists to dispense (up to 10 per cent) more or less than the prescribed quantity of a given medicine in the manufacturer's original packs. Key aims of the proposal are to "support increased patient safety by ensuring patients receive the necessary information that is included in the original manufacturer's packaging about the safe and effective use of a product," DHSC said.

The Department of Health and Social Care (DHSC) has issued an alert to update healthcare professionals on the shortage of glucagon-like peptide-1 receptor agonists (GLP-1 RAs), which are used to treat type 2 diabetes. It has informed that the supply of the drugs continues to be limited, and is not expected to return to normal until at least the end of 2024. An increase in demand for these products for licensed and off-label indications is cited as the cause of the supply issues. Therefore, the Serious Shortage Protocol, SSP032 Lipitor 10mg chewable tablets expired at 23.59pm on Friday (9 September). "After this date, any prescriptions for Lipitor 10mg chewable tablets must be dispensed in accordance with the prescription, and the SSP032 will no longer be valid for use," said DHSC.

Unlicensed supplies of clomifene 50mg tablets may be sourced, lead times vary. Alternative medicines for use in infertility remain available," it added. The specialist importers- Orifam and Target Healthcare have confirmed to DHSC they can source unlicensed clomifene 50mg tablets.
