International day for persons with disabilities on 3rd December - Note on schemes for the welfare of persons with disabilities and concessions available to them. The Government of India is committed for the realization of full participation, protection of rights, and equal opportunities to all including persons with disabilities. India is a country where most of the population lives in the rural areas and accessing the health and rehabilitation services has always been a challenge. In a Backgrounder issued yesterday, a brief summary of the major steps initiated by the Ministry of Social Justice & Empowerment in this direction were given. Following is the Background Note on the legislative and Institutional framework along with the major schemes of the Ministry of Social Justice & Empowerment for the welfare of the People with Disabilities (PwDs). The Note also contains information on facilities and concessions available to PwDs under other central schemes which are being run by other Union Ministries. The Rehabilitation Council of India Act, 1992 regulates and monitors training of rehabilitation professionals and personnel. The National Trust for the welfare of Persons with Autism, Cerebral Palsy, Mental Retardation and Multiple Disabilities Act, 1999, aims at enabling and empowering persons with disabilities to live independently as fully as possible in their community. The Integrated Care Systems (ICSs) provide an opportunity to break the silo mentality in the health and care sector, National Pharmacy Association (NPA) chair Andrew Lane told MPs. While giving evidence to the Health and Social Care Select Committee on 6 December about the effectiveness of ICSs, Lane also poured cold water on newspaper reports that community pharmacists will be drafted in to break the NHS strike, as he reiterated the call for a "properly costed and funded environment." "We've been in different silos historically and ICSs are an opportunity to pull all systems together for the benefit of patients," he told committee members. Lane pointed to dementia-trained pharmacy delivery drivers and the Discharge Medicines Service as instances where the influence of community pharmacy already stretches beyond primary care into hospitals and social care. "We've seen pharmacists prescribe and get UTIs off doctors' desks, so we are starting to release capacity [in the system] and we're on that clinical journey. We welcome that, but it has to be with the right level of funding," he added.

NHS England and NHS Improvement published on May 26 the findings of the Fuller 'stocktake' - setting out how primary care can work with partners across health and care to best meet the needs of their local communities. Dr Claire Fuller's report lays emphasis on the essential role of primary care and the potential of integrated neighbourhood teams in reducing the burden of ill health and tackling health inequities. It commends community pharmacy for keeping "its doors open to the public throughout" the pandemic whilst being "among the most recognisable of a multitude of dedicated staff delivering care around the clock in every neighbourhood in the country". The report highlights "recruitment and retention challenges across the wider primary care workforce" including in community pharmacy. Stressing the importance of community pharmacy teams in urgent care and prevention, including early diagnosis of cancers, the report points out that pharmacists could play "a more active role in signposting eligible people to screening and supporting early diagnosis, building on a number of successful pilots such as those from the Accelerate, Coordinate, Evaluate (ACE) programme".

The Pharmaceutical Services Negotiating Committee (PSNC) has launched a four point plan at its parliamentary drop-in event for MPs and peers hosted on Tuesday (15 November). The event lasted for two hours, during which MPs had the opportunity to drop-in and have their blood pressure measured by pharmacists with experience of working on the front-line and were briefed on the urgent need for action to address the pressures on the sector. Outline of the four-point plan: Resolve the funding squeeze: Community pharmacy needs an immediate funding uplift to prevent large-scale pharmacy closures, as well as emergency business relief to get through this winter. Tackle regulatory and other burdens: Pharmacies must be protected from medicines market shocks, supported to help them weather the workforce crisis, helped to free up capacity and freed from red tape that does not enhance patient care. Help pharmacies to expand their role in primary care: Pharmacies could do more to support the delivery of primary care for example through offering clinical services for long-term conditions - like hypertension, diabetes and respiratory disease, supporting people to adopt healthier lifestyles and prevent the development of long-term conditions, medicines optimisation, and a much wider variety of NHS vaccinations.

At their recent online briefing, DAUK's GP committee outlined a comprehensive 10-point plan designed to relieve pressure on GPs, improve access, reduce mortality rates, lower hospital admissions, and enhance patient satisfaction. DAUK's manifesto advocates reallocating resources, including the Additional Roles Reimbursement Scheme (ARRS), currently directed towards Pharmacy First and acute care hub pilots, amounting to £2 billion. This strategic reallocation aims to enhance primary care capacity and reduce reliance on acute services. Dr Lizzie Toberty, DAUK's GP lead, emphasised the critical need for patients to have timely access to GPs who understand their needs, stating, "We need patients to be able to see a GP who knows them and understands their needs in a timely way wherever possible." She highlighted DAUK's vision to restore general practice as the cornerstone of the NHS through practical, achievable solutions.

A recently published report on medicines optimisation highlights the urgent need for transformative changes across all pharmacies to unlock their full potential in delivering integrated care. The report by Public Policy Projects (PPP) is first of the four in the series that emphasises the significant benefits that medicines optimisation can bring to integrated care systems (ICSs). This includes reducing health inequalities and improving population health. Pointing to the "untapped opportunity" that medicines optimisation presents for ICSs facing financial challenges, the report underscores the vital role of pharmacies in enhancing patient outcomes. The launch of the Pharmacy First services has already demonstrated the essential contribution of pharmacies to the health and care system. With "medicines being the second highest cost to the NHS," the report recommends ways "to achieve better value for money while improving patient outcomes".

The recent announcement that Royal Mail will be partnering with distance selling pharmacy (DSP) giant pharmacy2U highlights how standards of regulatory enforcement are being ignored to accommodate the DSP model. The brunt of these double standards hinges around the levelling down of temperature enforcement standards by the Medicines and Healthcare products Regulatory Agency (MHRA) which demands mapping must be audited from the point of dispensing to the patient. The MHRA has a well recognised duty to ensure medicines reach patients in a safe condition. The current anomaly appears to turn a blind eye to this step in the supply chain at the point the wholesaler releases goods to the pharmacy hub. Equally the training on delivering medicines safely and effectively direct to patients should apply fully to all hubs including DSPs. Why is it that DSPs are being treated differently to bricks and mortar pharmacies? It's essentially the same patients receiving the same medicines from the same wholesalers. A further regulatory disparity exists around how parcels must be "tracked and signed for" to be reasonably certain medicines are delivered into the hand of the intended recipient, as per existing regulations. Clearly an untracked, unsigned package cannot be guaranteed to finish in the hands of the intended recipient. There is a very real possibility that such omission could lead to community pharmacy closures which will, in turn, lead to unemployment and a reduction in the care services. At a time when integrated care systems have just gone live, the removal of vital support services leading to further inequalities is the wrong message for both providers and patients alike.

The NHS is facing a range of long term challenges which needs to be countered with prevention, enhance personalised care and sustainable performance, health and social care secretary Sajid Javid said on Tuesday (March 8). In his speech on healthcare reform at the Royal College of Physicians in London, he said: "We face some long-term challenges: how to keep the NHS focused on delivery while futureproofing it for changing demographics and disease; how to meet rising patient expectations and address the injustices of widespread disparities; and how to deal with an unsustainable financial trajectory while backing the brilliant people who work in health and care." Javid emphasised on three key points for healthcare reform - prevention, personalisation and performance. He said, prevention is not just about building a 'national hospital service' but a true 'National Health Service'.

A standout pledge was the establishment of a Community Pharmacist Prescribing Service, aimed at granting pharmacists independent prescribing rights where clinically suitable. "Let's be honest, pharmacists are an underutilised resource within the healthcare system. We possess extensive training and expertise, yet our full potential hasn't always been acknowledged. "By harnessing pharmacists expanded prescribing rights, Labour is acknowledging our significant role in patient care. This initiative aims to recognise our skills and enhance patient care, ensuring quicker access to necessary treatments," he said. However, he pointed out that the concept requires further exploration, as the rights to prescribe are already granted through the recent change to undergraduate training that incorporates prescribing.

In a recent interview with The Telegraph, health secretary Steve Barclay stated that he has asked his officials within DHSC to look at a "pharmacy first" approach to alleviate pressures on A&E departments in order to avoid the widely predicted NHS winter crisis. On the face of it, this a welcome if long overdue recognition that community pharmacy is an essential part of our national healthcare infrastructure alongside our GP and A&E colleagues. But let's not get carried away - we have had lots of praise from politicians in the past which have not then been backed by firm commitments for a sustainable future for the network. Could this be a turning point? I hope so, but I am not confident it will be. I fear this may turn out to be another emergency stop-gap measure which does nothing to secure the long-term viability of the sector in England. The role of community pharmacy during the recent Covid pandemic demonstrated clearly how important we are to ensure people have easy access to essential healthcare support, advice and services. The NHS winter crisis can only be avoided or at least mitigated if the potential of the community pharmacy network to provide more patient care services is unlocked and that Barclay requires you to end pharmacy funding austerity and start investing. The Treasury will no doubt say there is no more money, but what then the alternative other than a NHS winter crisis? And, of course, treating people in secondary care settings is far more costly than community pharmacy based interventions.
