Community pharmacy is stuck in a vicious circle. Pharmacies are closing at an alarming rate whilst the government is talking about moving care away from hospitals and into the community to focus on preventative care. While the government talks about this vision for the future, the stark reality is that pharmacies are already struggling to meet the current Pharmacy First thresholds. And now we discover that one of the key components that could create capacity in pharmacy to deliver more clinical services has been put on hold…indefinitely. The recently published Darzi review, which was commissioned after the new government came to power to assess the state of the NHS, praised the value of community pharmacies and preventative services. But it recognised the level of pharmacy closures across the country and warned pharmacy access could be 'at risk'. Community pharmacy is ideally placed to deliver the government's vision of preventative care in the community. But let's all be clear, moving preventative services into community pharmacies has, so far, been nowhere near as successful as it could have been due to the current climate pharmacies find themselves in. The Pharmacy First scheme is a testament to this. Just this week it was announced that consultation thresholds for the Pharmacy First scheme have been reduced from 30 consultations to 20. It's the second time in three months that the thresholds have been reduced and it follows an intervention from Community Pharmacy England who wanted to "save many pharmacies from missing out on a vital payment this month." In June alone, 3,269 participating pharmacies failed to meet the threshold - that's almost a third of all community pharmacies in England.

Well Pharmacy has acquired three new pharmacies taking the total number of pharmacies owned by the chain across the UK to 746. The UK's largest independent and third largest pharmacy chain has bought Pharmacy Express in Wakefield, West Yorkshire, and two Frosts Pharmacy's in Banbury, North Oxfordshire, and Marston, near Oxford. All three pharmacies will be rebranded. # Well did not disclose the amount it paid for the new acquisitions. Operations director of Well Pharmacy Louis Purchase said that the group was "excited" to expand. "We believe passionately in community pharmacy, and we want our offering to be the best experience of this in the UK. We also hope our new colleagues can keep progressing and develop their clinical skills even further by being part of Well Pharmacy." Frosts Pharmacy teams have joined Well from their pharmacies based in Ferriston, Banbury, North Oxfordshire and Marston, near Oxford. They both serve local people from the heart of their communities and offer a successful travel vaccination clinic in Marston.

In just three days, three well-established pharmacies were sold in Essex -Dips Chemist and Riverview Pharmacy in Grays, and Longfield Pharmacy in Maldon. According to specialist business property adviser Christie and Co, Dips Chemist and Riverview Pharmacy were included in a wider portfolio sale following their entry into administration in 2023. Both pharmacies, which collectively dispense an average of circa 14,000 items per month, have been acquired by Primary Health Solutions Ltd, an independent pharmacy operator led by Husain Master. "These two pharmacies are the perfect addition to my existing portfolio," said Master, who plans to increase the services offered as well as invest in the pharmacies to create a more clinical and welcoming environment. Mark Page, director - Pharmacy at Christie and Co, highlighted that there's an appetite for a full range of pharmacies in Essex and the Northern Home Counties, with many buyers looking to take on well-performing pharmacies or purchase a quieter pharmacy that can be improved.

EMIS Pinnacle, the leading Pharmacy First IT supplier, has begun implementing the GP Connect Update Record feature to all community pharmacies in England that use PharmOutcomes. Starting June 28, this new feature was made available for use with the Hypertension case-finding service and the Pharmacy contraception service, as announced by the Community Pharmacy England (CPE). The GP Connect Update Record will enable community pharmacies to securely share "structured" information with GP practices regarding consultations conducted under the Pharmacy First programme, which includes minor illness and clinical pathways, along with Hypertension Case Finding and Pharmacy Contraception services Live testing is underway with EMIS Pinnacle for Pharmacy First minor illness and clinical pathways. There are plans to expand the system in the future to include updates for urgent supply of medicines under the Pharmacy First service. EMIS Pinnacle is the third CPCF IT system supplier to offer this functionality, following Cegedim's Pharmacy Services system and Positive Solutions' HXConsult system.

National pharmacy bodies have expressed their disappointment on the launch date for Tier 1 of the Pharmacy Contraception Service which has been announced by NHS England (NHSE) as '24th April 2023′. Pharmaceutical Services Negotiating Committee (PSNC) said that this start date for the service has not been agreed with PSNC and is in direct contradiction of our warning to Ministers that no new or expanded services can be rolled out in 2023/24 unless extra funding is put into community pharmacies. Responding to this announcement, PSNC Chief Executive Janet Morrison said: "This is despite our warning last month that without additional funding, the roll-out of Year 5 additional services and the Pharmacy Quality Scheme is neither feasible nor affordable. Community pharmacies are having to work harder and harder for less money and many are at breaking point. And just this week the results of our 2023 Pharmacy Pressures Survey have confirmed the worsening situation. Clearly our view is not because contractors don't see the benefit of the service. This is a much-anticipated service that could deliver real benefits to patients and community pharmacies are always eager to support public health initiatives. But capacity in the sector is now so stretched that more money is needed to safely resource additional work. We have repeated our concerns to the Department in recent days and reminded them of the potential for a properly funded community pharmacy sector to play a greater role in providing clinical solutions and relieving pressures elsewhere in primary care."

Gurinder Singh, a lecturer of Pharmacy at the University of Reading, is set to transition into the role of an independent community pharmacist, along with his wife, Rajdip Purewal, a Clinical Pharmacist in General Practice and clinical lecturer at University of Hertfordshire. In an exclusive with Pharmacy Business, Singh shared his excitement about his acquisition of a Boots pharmacy located in picturesque Malmesbury, Wiltshire. His acquisition reflects broader trends in the pharmacy sector where experienced professionals are stepping in to fill gaps left by corporate closures. According to Singh, driven by a passion for healthcare and a commitment to their community, the new venture promises to bring an innovative and personalised touch to pharmacy services offered in the area. Discussing his motivation for revamping the pharmacy, now Malmesbury Pharmacy, Singh said: "My initial motivation was to ensure the local community didn't have to put up with another closure of a pharmacy after the recent closure of the Lloyds Pharmacy in Malmesbury."

Shadow Health Secretary Wes Streeting outlined his plans for NHS reform on Friday (21 April), where he also mentioned a desire to see pharmacies take on more workload and pharmacists' clinical skills to be better utilised. In his speech, he also expressed concern at warnings from the pharmacy sector and pharmacy closures. The Company Chemists' Association (CCA) appreciated Shadow Health Secretary's desire to enhance pharmacies' role in primary care reform but also stated that the sector is in urgently needs a funding injection. Dr. Nick Thayer, Head of Policy at the Company Chemists' Association (CCA) comment: "We are pleased to see the Shadow Health Secretary recognise the enhanced role that pharmacies should play in primary care reform. Mr Streeting wants to see care brought closer to people's homes and more front doors into the NHS. With 89.2% of the population located within a 20-minute walk of a pharmacy, pharmacies do just that. They are essential to realising his vision of a 'neighbourhood health service'. Pharmacies could free up over 42m GP appointments annually, including 10m vaccination appointments. Urgent same-day appointments for minor conditions in pharmacies will immediately benefit patients by increasing GP access. The first step is for the Government to immediately commission a fully-funded Pharmacy First service in England.

Rowlands Pharmacy has joined hands with healthcare software company HasHealth for an enhanced digital proposition to facilitate seamless delivery of a raft of clinical services including flu and travel vaccinations. The new system is now available across 400 Rowlands pharmacies across the UK. The partnership comes as Rowlands Pharmacy strives to provide patients with a seamless booking experience whilst minimising time-consuming administration for pharmacy staff, with its waiting list, medical pre-screen questionnaires and resource management capabilities, the company said in a statement. HasHealth offers a virtual healthcare software solution that allows community pharmacies to schedule, manage, and communicate with patients easily online. It combines its cutting-edge video technology with booking and management software, which can be adapted to any clinical service. Nigel Swift, managing director at Rowlands Pharmacy, commented: "We pride ourselves on listening to our staff, based on feedback we knew we could do better with our digital proposition for clinical services and flu vaccinations in particular.

The 8th Pharmacy Business Conference, organised by Pharmacy Business, unfolded a dynamic narrative around the theme of 'Pharmacy of Tomorrow', highlighting the trajectory of innovation, adaptation, and the evolving landscape of pharmaceutical services. Attended by over 200 pharmacy owners, industry leaders, and stakeholders, the conference served as a medium for robust discussions and the exchange of valuable insights regarding the future of community pharmacy. Amidst the persistent challenges posed by an underfunded reimbursement system and negotiations with governmental bodies and the NHS for the new community pharmacy contractual framework 2024/25, the conference pivoted towards investing in staff, adapting to change by investing in new technology, and optimising commissioning as pivotal strategies. "Pharmacy professionals are playing increasingly important clinical roles in both primary and secondary care," shared David Webb, Chief Pharmaceutical Officer (CPO), NHS in a video message. He highlighted the NHS's commitment to empowering community pharmacy, with plans to expand services and deprescribe to align with the NHS's focus on preventive healthcare.

In response to concerns raised by Community Pharmacy England (CPE), ministers have decided to lower the October threshold for the Pharmacy First service to 20. This decision addresses a major worry for pharmacy owners who have struggled to meet the required number of clinical pathways for monthly Pharmacy First payments, especially as the October increase to 30 pathways approached. CPE has been actively engaged in discussions with the Department of Health and Social Care and NHS England over the past few weeks to address this issue. Commenting on the decision, Janet Morrison, CEO of Community Pharmacy England, said: "It is great that ministers have responded to our concerns about another increase to the Pharmacy First payment threshold: this decision will save many community pharmacies from missing out on a vital payment this month." Morrison added that CPE will continue to monitor service data and, if needed, make further representations to ministers about future months. She also emphasised the need to find a long-term solution, which involves "NHS England marketing Pharmacy First effectively and ensuring that GPs are actively referring people to the service."

Pharmacy professionals to be included as key stakeholders in the implementation, delivery and evaluation of a wide range of genomic services, said the Royal Pharmaceutical Society (RPS). RPS's statement has been developed in collaboration with pharmacy organisations who have co-badged the report, such as the British Oncology Pharmacy Association, the UK Clinical Pharmacy Association, Association of Pharmacy Technicians and the College of Mental Health Pharmacy. It looks at current and future roles for pharmacy professionals in genomic medicine across many aspects of practice such as person-centred care and collaboration, professional practice, education, leadership, management and research. Pharmacists and pharmacy technicians in the UK have already established roles in the application of genomic medicine in some areas of practice, such as antimicrobial stewardship and infectious diseases, and the management of certain genetic conditions, such as cystic fibrosis. The society believes, the current role of pharmacy professionals in genomics can be expanded upon in the future to both lead and support many relevant aspects of genomic implementation. These are described across all healthcare sectors, within the Genome UK strategy produced by the UK Government, and within the implementation plans published in England, Scotland and Wales. Lead for Pharmacogenomics at RPS Sophie Harding said: "Pharmacy professionals are the gatekeepers of medication safety and efficacy across all areas of healthcare. They are skilled at interpreting complex scientific data and use evidence-based medicine to maximise the benefits of treatments for patients, whilst supporting shared decision-making with patients and the multidisciplinary team.

In response to concerns raised by pharmacy owners, the government has decided to adjust the August threshold for the Pharmacy First service from 20 to 15 clinical pathways. The ability of pharmacy owners to achieve the thresholds for the monthly Pharmacy First payments has been a topic of great concern. Community Pharmacy England (CPE) has been actively discussing this issue over the last few weeks with the Department of Health and Social Care and NHS England. Ministers have now decided to adjust the August threshold for the Pharmacy First service to 15, CPE announced today (July 31). Commenting on this decision, Janet Morrison, CEO of Community Pharmacy England said: "We are pleased that Ministers have listened to our concerns and acted quickly to make an adjustment so soon after coming into office."

The Pharmacists' Defence Association (PDA) has republished a three-year-old report which underscores the vital role of community pharmacists and pharmacy t echnicians in a 'modern NHS' amid speculations that the government is set to announce plans to enhance the role of community pharmacy teams. PDA's 2019 report which "took three year to compile" highlighted significant empirical evidence and made recommendations to develop the roles of both community pharmacists and pharmacy technicians, and the skill mix required to meet patients' needs. It examined in detail the role of pharmacy technicians in community pharmacy. The PDA still that pressure on the NHS can be managed much more effectively through the better use of pharmacists and pharmacy technicians. In the community pharmacy setting, it is evident that if the further development of the pharmacist's role is to be facilitated and pharmacists are to become more patient facing in the future, an extended role and greater responsibility for pharmacy technicians is not only desirable, but ultimately it is essential.

In an oral parliamentary debate on community pharmacy held on Monday (20 March), the government was asked for an indication of 'how much it would cost to make the best use of community pharmacies'. A number of Peers at the House of Lords spoke out in support of pharmacies, while others asked when would the government address the current crisis in the sector. Kicking off the session, Baroness Hodgson asked the responding minister, Lord Evans, about recent pharmacy closures as well as underfunding. She asked whether the government would 'enter into discussions with PSNC to look at introducing a fairly funded pharmacy first service as soon as possible which will help relieve the work load on GPs'. The minister responded: "We have already introduced and funded a range of service in community pharmacy that make use of the clinical skills of pharmacy teams… we continue to discuss with PSNC how the government can best support the sector to provide support to patients." The House of Lords oral questions session was opened by Baroness Hodgson on behalf of Baroness Cumberlege as follows: "To ask His Majesty's Government what plans they have for making the best use of community pharmacies".

Prescribing in pharmacies is an emerging opportunity, and I'm passionate about supporting proper governance arrangements for businesses. But does the governance and administrative burden reduce if your business is not doing the prescribing? Recently, I've come across several independent pharmacy businesses across the UK who have been approached by prescribing clinics to dispense medicines for their patients. While these partnerships can present lucrative opportunities, they also come with significant responsibilities and potential risks. Pharmacy owners and superintendent pharmacists must be fully aware of the guidance set by the General Pharmaceutical Council (GPhC) and ensure that all requisite checks are performed to maintain compliance and safeguard patient safety. The growing trend of prescribing clinic collaborations For one reason or another, private prescribing clinics are sprouting up all over the place, offering specialised services such as weight management, hormone therapy, or dermatological treatments. These are quickly becoming a prominent feature in our healthcare landscape. Many of these clinics lack in-house dispensing capabilities, leading them to seek partnerships with pharmacies to satisfy a growing number of consumers. In the current economic climate, these collaborations can offer a new revenue stream and an expanded patient base. However, with these opportunities come responsibilities that must not be overlooked.

The Department of Health (DoH) in Northern Ireland has published a new Strategic Plan for Community Pharmacy, highlighting the vision and key priorities for the sector over the next 10 years. Co-developed by the DoH and Community Pharmacy Northern Ireland (CPNI), the representative body for community pharmacy owners, the plan aims to fully realise the potential of community pharmacy services to support better health outcomes from medicines and prevent illness. The vision outlined in the plan sees community pharmacies offering more clinical services, leveraging the skills of their teams to provide the public with safe, convenient, and faster access to care. According to DOH, the vision will be realised through four major reform programmes focusing on services, legislation, workforce, and digital enablers, alongside six strategic priorities. Together, these initiatives are anticipated to transform the role of community pharmacy within the Health and Social Care (HSC). Health Minister Robin Swann has extended his support to the new Community Pharmacy Strategic Plan, which builds on the previously agreed 3-year community pharmacy commissioning plan.

The Royal Pharmaceutical Society (RPS) has urged people across Scotland to be kind to their pharmacy teams this winter through its campaign called 'Be Kind'. The pharmacies are facing lot of pressure and challenges during winter, RPS has heard from its members that abuse towards pharmacy teams, particularly those in community pharmacy, has 'unfortunately' risen. The campaign has been running throughout December, to promote 'be kind' message to the public at a time when pharmacy services are particularly stretched by winter pressures and other factors, including the recent shortages of antibiotics. "Throughout December, leading figures have provided a series of powerful videos, which all encourage the public to be kind to their pharmacy teams. The campaign has highlighted personal video messages from many figures including Jason Leitch, Scotland's National Clinical Director, Maree Todd, Minister for Public Health, Women's Health and Sport, and Andrew Carruthers, RPS Scottish Pharmacy Board Chair," said RPS.

Community Pharmacy Wales and the Welsh government have entered an agreement on Thursday (December 16), enabling all pharmacies to deliver provision of contraception, treatment for common minor ailments, and access to repeat medicines in an emergency. Expansion of clinical community pharmacy service will be supported with an increased funding of £20 million a year by April 2024 from the current level of £11.4m. The two bodies have been negotiating the terms of agreement, which also includes plans to roll out a Wales-wide pharmacy prescribing service, allowing trained pharmacists to treat an extended range of conditions that currently require GP visits. The agreement will allow patients to access NHS services closer to home which in turn would free up GP and other NHS services for patients with more complex needs. Eluned Morgan MS, minister for Health and Social Services said: "I welcome the positive approach to negotiations taken by Community Pharmacy Wales, embracing our ambition for a reimagined community pharmacy service, which is an integral part of a strong primary care landscape."

The Pharmaceutical Services Negotiating Committee (PSNC) has confirmed that the community pharmacy contractors are 'not required' to undertake a contractor-chosen or an NHS England determined clinical audit in 2023/24. "A clinical audit on anticoagulants is included in the Pharmacy Quality Scheme (PQS) 2023/24. If contractors choose to not participate in PQS, and therefore do not complete the anticoagulant clinical audit, there is still no requirement to complete two clinical audits in 2023/24," said PSNC. "The Department of Health and Social Care and NHS England have also committed to consider, during 2023/24, the removal of the requirement for a contractor-chosen clinical audit permanently."

The NHS England has introduced a training programme for 1000 staff to broaden their community pharmacy roles, thereby improving patient access to services. Among them, 500 pharmacy staff will be trained to supervise pharmacists during prescription training, while the remaining 500 will provide support for various training, including pre and post-registration programmes for pharmacists and pharmacy technicians. According to the NHSE, pharmacist and pharmacy technician roles are rapidly evolving due to healthcare changes and increased demand for clinical services in community pharmacy. "There are 3,000 independent prescribing training spots available for 2023/24. Educational supervision is a crucial part of this to enable the ongoing transformation and skills development of the growing pharmacy workforce." The NHSE-funded training, scheduled to begin next month, will ensure that those involved in educational supervision, including Designated Supervisors (DSs), Pre-registration Trainee Pharmacy Technician education supervisors and Designated Prescribing Practitioners (DPPs), can effectively support learners.
