The UK government will lay legislation today (13 December 2023) to allow the General Medical Council (GMC) to begin the process of regulating medical associates to expand their roles in the NHS. This will support plans to reduce pressure on doctors and GPs and improve access for patients, the Department of Health and Social Care (DHSC) said. Physician associates (PAs) and anaesthesia associates (AAs) will have the same levels of regulatory oversight and accountability as doctors and other regulated healthcare professionals once the regulations come into force, which is expected at the end of 2024. The GMC will design and deliver detailed regulatory processes for registration, education, standards and fitness to practise for both professions.

Reena Mehta, consultant pharmacist in the Critical Care team at NHS King's College Hospital, has been announced as chair of the Learning Division for the Intensive Care Society. Mehta took up the post with effect from 8th April, and is the first pharmacist to hold the role of Chair of the Learning Division within the Society. "It is a privilege to be part of the Society and support its life-saving work to enhance our understanding of critical illness and deliver better care to patients," "The intensive care community is made up of multiple professions, each playing a vital role within the critical care team, and I am extremely proud to be the first pharmacist appointed to this important role." Over the last few years, we've seen the aftermath of Covid-19, with the demand for consumer health products being unprecedented and unpredictable, placing pressure on supply chains and labour market. In turn, the industry has experienced acute shortages across the healthcare sector, placing huge pressure on pharmacists, doctors, nurses, and other healthcare professionals. Europe has an estimated shortage of around 50,000 public-sector doctors[1], which is set to increase in the coming years. Medicines and Healthcare products Regulatory Agency (MHRA) has reminded healthcare professionals that use of systemic (oral and injectable) Non-steroidal anti-inflammatory drugs (NSAIDs) such as ibuprofen, naproxen, and diclofenac is contraindicated in the last trimester of pregnancy (after 28 weeks of pregnancy). It has advised that patients who are unsure whether pain relief medicine they are taking is an anti-inflammatory (NSAID), to speak to doctor, midwife, or pharmacist. Some non-prescription pain relief medicines may contain more than one active drug, therefore it is important to read the box or the leaflet provided with the medicine to see if it contains an NSAID like ibuprofen. The use of any non-prescription medicine for the management of pain during pregnancy should be for the shortest possible time at the lowest possible dose, suggested MHRA.

The British Generic Manufacturers Association (BGMA) has elected Matthew Salzmann, UK Country Manager at Viatris, as its new Vice-Chair with immediate effect. He will replace the current chair of the BGMA, Diane DiGangi-Trench, who is the UK country head for Sandoz. Salzman, who is originally from Australia, has served approximately 1 billion patients worldwide last year as the UK Country Manager at Viatris, a global pharmaceutical company operating in 165 countries. In his role as BGMA Vice-Chair, Salzmann would focus on working in partnership with key stakeholders - the NHS, healthcare professionals, policy makers, clinical societies, patient associations, academia - to ensure access to affordable and sustainable medicines for patients and the NHS.

The National Health Service (NHS) retention programme has benefitted 23 NHS Trusts since it launched in April 2022, with thousands fewer staff leaving frontline roles. NHS data showed that 14,000 fewer staff left the service in the 12 months up to August 2023 (108,890) compared to 122,970 the year before. In a statement released on Thursday, the NHS England said it is expanding the pilot programme across the country to benefit 42 more NHS trusts. The programme is part of the NHS Long Term Workforce Plan ambition to retain up to 128,000 more staff over the next 15 years in addition to training record numbers of doctors, nurses and other healthcare professionals. Under this new initiative, NHS staff are being offered extra flexibility with working hours, clinical 'support squads' have been introduced to help menopausal women at work, and HR 'stay advocates' are designated to identify ways to keep staff on the brink of leaving.

The Medicines and Healthcare products Regulatory Agency (MHRA) has warned the public to avoid buying pre-filled weight loss pens without a prescription. The warning came after 369 fake Ozempic injections have been seized since January 2023. However, Saxenda pens were obtained by members of the public in the UK through "non-legitimate routes". The health regulator has urged the public to buy the pens after consulting a healthcare professional and with a prescription. Dr Alison Cave, MHRA Chief Safety Officer shared her opinion on the matter, she said: "Buying products such as Ozempic or Saxenda without a prescription, from illegally trading suppliers, significantly increases the risk of receiving something which is either fake or not licensed for use in the UK.

The Royal Pharmaceutical Society (RPS) has said that multi-buy deals on paracetamol violate the existing voluntary sales guidelines. The professional body is urging the government to pass legislation that prohibits retailers from making such offers on medicines for pain relief. They have also sent letters to MP Maria Caulfield who is the Minister for Mental Health and Women's Health. The Medicines and Healthcare products Regulatory Agency (MHRA) has reminded healthcare professionals prescribing aripiprazole to alert patients about its possible side effects, following a rise in the number of reports of gambling disorder associated with the drug. Patients taking aripiprazole, which is used for the treatment of schizophrenia and bipolar disorder, are at risk of developing unusual urges or cravings that they cannot resist, including behaviours such as addictive gambling, excessive eating or spending, or an abnormally high sex drive. The regulator has received 69 reports of gambling or gambling disorder suspected to be caused by aripiprazole in the last 14 years via the Yellow Card Scheme, out of which 32 were received between 1 January and 31 August this year. Alison Cave, MHRA Chief Safety Officer, said: "The number of reports for suspected gambling and other impulsive behaviours associated with aripiprazole are small in comparison to the frequency with which it is prescribed, but the consequences for any patient developing these conditions can be significant."

The Department of Health and Social Care (DHSC) has issued an alert to update healthcare professionals on the shortage of glucagon-like peptide-1 receptor agonists (GLP-1 RAs), which are used to treat type 2 diabetes. It has informed that the supply of the drugs continues to be limited, and is not expected to return to normal until at least the end of 2024. An increase in demand for these products for licensed and off-label indications is cited as the cause of the supply issues. "Rybelsus® (semaglutide) tablets are now available in sufficient quantities to support initiation of GLP1 RA treatment in people with type 2 diabetes (T2DM) in whom new initiation of GLP-1 RA therapy would be clinically appropriate," the DHSC's National Patient Safety Alert reads.

The Medicine and Healthcare products Regulatory Agency (MHRA) has cautioned men on valproate to talk to their healthcare professional about their treatment, if they want to start a family in the next year. The warning came after a new study, commissioned by the European Medicines Agency, suggested that children fathered by men who took the anti-seizure medicine in the three months prior to conception may be at higher risk of developing neurodevelopmental disorders. Around five in 100 children born to fathers treated with valproate around conception were diagnosed with a neurodevelopmental disorder, compared to three in 100 children whose fathers were taking other antiseizure medicines (lamotrigine or levetiracetam). However, the risk is believed to be much smaller than the risk associated with valproate in pregnancy.

Key insights from the survey underscored that individuals from ethnic minority groups often require a higher number of interactions with healthcare professionals before receiving a cancer diagnosis. Notably, one in three people from Mixed, Black, or Asian ethnicities needed three or more visits to a GP practice, compared to the average of one in five across the population. Recent NHS data sheds light on the evolving landscape of fit note certification, indicating a gradual expansion of roles beyond traditional medical practitioners to pharmacists and nurses. This uptake in numbers coincides with the Department of Work and Pensions's (DWP) call for evidence in relation to fit note reform which will remain open till 8 July 2024. According to figures published by NHS Digital, during the nine-month period from April 2023 to December 2023, pharmacists certified a significant number of fit notes. Approximately 1.4 per cent of fit notes of the 8 million were issued by pharmacists while the rest were issued by the GP surgeries reflecting the impact following legislative amendments in July 2022, which extended certification authority to pharmacists, nurses, occupational therapists, and physiotherapists. In June 2022, the Pharmacists' Defence Association (PDA) advised pharmacists not to sign fit notes where the required therapeutic, diagnostic, or examination skills lie outside their scope of competence. The association remarked that healthcare professionals needed mandatory training under the Health Education England before they are fit to issue fitness notes to the public.

A recent UNISON survey has unearthed disturbing findings regarding the prevalence of sexual harassment among NHS staff across the UK. With data collected from over 12,000 healthcare workers, the survey sheds light on the challenges faced by frontline workers in their workplace environments. Approximately 10 per cent of healthcare professionals reported instances of sexual harassment during their tenure, with nearly 29 per cent of those experiencing harassment also reporting incidents of sexual assault. Shockingly, 50 per cent of respondents reported being leered at or subjected to suggestive gestures, while a quarter faced unwelcome sexual advances or demands for favors. The most common complaint, voiced by 61 per cent of those affected, was the experience of unwanted crude banter or jokes in the workplace.

In a bid to address the ongoing deficit in staff in the UK's healthcare system, the National Health Service (NHS) has entered into a new workforce agreement with India. According to media reports, the healthcare service plans to hire 2,000 doctors from India on a fast-track basis to alleviate this pressing issue. It is learned that the NHS has set up training centres across many Indian cities such as Mumbai, Bangalore, Nagpur, Delhi, Gurugram, Indore, Mysore, Chennai, and Calicut to provide postgraduate training for the first batch of doctors. Post their training period (which ranges from six to 12 months), these doctors will be deployed at hospitals across Britain. They will also be exempted from the Professional and Linguistic Assessments Board (PLAB) examination that is otherwise mandatory for practising in the country.
