NHS Online GP Registration: 425k Patients in First Year - 0 views

    The National Health Service said that over 425,000 patients have utilised its new national online service to complete GP registration within its inaugural year. The GP surgery registration service, managed by NHS England, has granted NHS patients efficient and user-friendly access to GP registration since its inception in August 2022. According to the NHS, more than 1,400 GP surgeries, a fifth of the national count, have joined the NHS online registration service as part of a new plan to improve access to primary care. "This aligns with a new strategy to enhance primary care access, with a target of enrolling 2,000 practices in the Register with a GP surgery service by December 2023." Many GP surgeries still use paper forms, requiring patients to visit surgeries to collect paperwork. However, the new online service has been shown to save GP surgery staff up to 15 minutes per registration by helping to reduce paperwork and admin time, the NHS said. The NHS said that this service enables patients to register with a local GP online at any time, eliminating the need for in-person visits and simplifying the process for both patients and GP surgeries.

    There is no doubt that the pandemic has changed the way healthcare professionals work within the primary care setting. The advent of Covid-19 meant that we all had to rapidly modify the way we supported and met the needs of patients, some of whom saw the services they usually took for granted, virtually cease overnight. There are around 15 million people in England living with long-term health conditions including asthma. These people have the greatest healthcare needs of the whole population with 50 per cent of all GP appointments and 70 per cent of all bed days taken by this cohort of patients, and their treatment and care absorbing 70 per cent of acute and primary care budgets in England. This situation isn't going to improve any time soon. In the past, most people had a single condition, today multi-morbidity is becoming the norm. At the start of the pandemic, the Royal College of General Practice and British Medical Association issued guidance to practices on prioritising workload. This included the importance of maintaining long-term condition reviews in asthma, COPD and diabetes, along with appropriate transition of at-risk warfarin patients. These reviews were deemed as essential workstreams for patients considered to be at high risk. Traditionally, the unique skills set of a pharmacist has meant that we have played a major role in supporting these patients. I work with a team of over 90 clinical pharmacists who, in partnership with individual practices, PCNs, CCGs and STPs, help with the long-term management of people with chronic conditions. But the onset of Covid-19 meant that we now had to plug a potential gap in service provision, and quickly.

    Over the past two years, the Covid pandemic has resulted in a backlog of elective care within the NHS, with record numbers of patients on waiting lists across the UK. Despite efforts to catch up after the first wave, subsequent waves have put further strain on an already stretched NHS system. This has had a tremendous impact on many patients waiting for routine dermatology appointments and, while every effort has been made to see patients with suspected serious skin cancers within two weeks, this target is not always achieved. With rising waiting times, pharmacists are increasingly the first port of call for patients suffering with a broad range of skin problems. From acne and rosacea, eczema or psoriasis, to changing skin lesions or simple insect bites, pharmacists can give helpful advice, allowing some customers to find quick relief to their skin complaint in mild cases, while directing others to see their GP without delay. Below is some practical advice that pharmacists can give to their customers with commonly encountered skin complaints. Acne Acne is a common skin condition that not only affects teenagers but adults too. It is estimated that up to 40 per cent of women continue to suffer with acne into their adult years. Acne has been linked to an increased incidence of depression, anxiety and even suicidal ideation. Patients have reported feeling overlooked and often dismissed in their concerns by health care professionals. Unfortunately, these issues have been further compounded by delays in being seen and treated due to the ongoing Covid pandemic. Pharmacists are well placed to help acne patients choose the right skincare. This should consist of lightweight products, such as gel or milky cleansers, with moisturisers in serum, lotion or light cream preparations. Some acne suitable products are further labelled as non-comedogenic. Oil-based or occlusive preparations are best avoided. There are many good quality, non-comedogenic sun blocks suited

Double standards:Dangerous for patients and can't acceptable - 0 views

    The recent announcement that Royal Mail will be partnering with distance selling pharmacy (DSP) giant pharmacy2U highlights how standards of regulatory enforcement are being ignored to accommodate the DSP model. The brunt of these double standards hinges around the levelling down of temperature enforcement standards by the Medicines and Healthcare products Regulatory Agency (MHRA) which demands mapping must be audited from the point of dispensing to the patient. The MHRA has a well recognised duty to ensure medicines reach patients in a safe condition. The current anomaly appears to turn a blind eye to this step in the supply chain at the point the wholesaler releases goods to the pharmacy hub. Equally the training on delivering medicines safely and effectively direct to patients should apply fully to all hubs including DSPs. Why is it that DSPs are being treated differently to bricks and mortar pharmacies? It's essentially the same patients receiving the same medicines from the same wholesalers. A further regulatory disparity exists around how parcels must be "tracked and signed for" to be reasonably certain medicines are delivered into the hand of the intended recipient, as per existing regulations. Clearly an untracked, unsigned package cannot be guaranteed to finish in the hands of the intended recipient. There is a very real possibility that such omission could lead to community pharmacy closures which will, in turn, lead to unemployment and a reduction in the care services. At a time when integrated care systems have just gone live, the removal of vital support services leading to further inequalities is the wrong message for both providers and patients alike.

NPA : Patients go without prescription medicines - 0 views

    Almost nine in ten community pharmacists in England say they have patients who sometimes go without prescription medicines because they cannot afford the prescription charge levied by the government. Sixty-eight per cent of pharmacists in a National Pharmacy Association (NPA) survey, conducted via email in June 2022, said this has become more frequent in the past year - suggesting that the rising cost of living could be leading more people to miss out on vital medicines. While prescription charge does not apply in Scotland, Wales and Northern Ireland, in England an NHS patients needs to pay £9.35 per item. For patients who need multiples medicines the cost could be exponential and virtually unaffordable amid rising inflation and higher cost of living. The survey found that 89 per cent of pharmacies in England have patients who sometimes go without prescription medicines due to cost. For most pharmacists (74 per cent) this happens one to five times a week. Fifteen per cent said they see such patients from six to 20 times a week.

    Two weeks prior to the closing of the hub and spoke consultation, issues around patient safety, costs and competition as well as practicalities such as what to do with uncollected medicines were discussed in a webinar on hub and spoke dispensing on Wednesday (May 26). The webinar was organised by Sigma Pharmaceuticals in association with the National Pharmacy Association (NPA). NPA director of corporate affairs Gareth Jones chaired the event and was joined by Sigma's Hatul Shah and Raj Haria as well as NPA vice chair Nick Kaye and NPA head of advice and support services Jasmine Shah. Kaye said: "There are still many 'unknown unknowns' with hub and spoke and the jury's still out on any potential benefits. I have lots of reservations about cost and it worries me how efficient this is for the business and the long term sustainability for the sector as a whole. Above all we need to think about the patient at the centre of all this and the potential confusion for them." Jasmine Shah felt patient safety was going to be the key in regards to whether the spoke and the hub would hold the ultimate responsibility and "who is going to take the accountability as far as patient care goes". She added: "It is most important that GPhC standards and NHS requirement are both met in identifying all the risks associated with the arrangement with hub and spoke and ensuring that (patient safety) measures are in place. Everything that needs to be looked at is by putting the patient at heart of the arrangement and seeing what is the safest way for them to receive care from both spoke and hub."

Teva UK launches generic version of Apixaban - 0 views

    Recently launched generic version of Apixaban by Teva UK is said to bring savings to the NHS drug bill while making sure patients get the medicine they need. The generic apixaban is available for the prevention of stroke and systemic embolism in adult patients with non-valvular atrial fibrillation (NVAF), with one or more risk factors, such as prior stroke or transient ischaemic attack (TIA) and treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE), and prevention of recurrent DVT and PE in adults. Previously there was only a 'branded' product available, but Castleford-based Teva UK succeeded in invalidating the apixaban patent and SPC (supplementary protection certificate) in the UK High Court and so is now able to launch its own 'generic' version - which will bring savings for the NHS while making sure patients get the medicine they need. "We've always said that we stand up for the patient", said Kim Innes, General Manager of Teva UK and Ireland. "The launch of generic apixaban emphasises Teva's commitment to doing the right thing by putting patients at the heart of everything we do by giving them and the NHS access to affordable treatments." Apixaban is an anticoagulant which directly inhibits factor X (factor Xa), inhibiting thrombin formation and the development of thrombi (blood clots). For at-risk patients, such as those with, or at risk for DVT, or NVAF, the risk of stroke related to blood clots forming in the body and traveling to the brain is a serious concern. Each year, DVT affects around 1 person in every 1,000 in the UK and if left untreated, about 1 in 10 people with a DVT will develop a PE.

Roche AccuChek Insulin pumps: warning over insulin leakage - 0 views

    Following concerns raised about cracked cartridges and insulin leaks, the Medicines and Healthcare products Regulatory Agency (MHRA) has issued a national patient safety alert for the NovoRapid PumpCart prefilled insulin cartridge and the Roche Accu-Chek Insight Insulin pump system. The regulator have asked patients to check the pre-filled glass insulin cartridge for cracks before use. It advised against using the cartridge if it has been dropped even if no cracks are visible and urged to closely follow the updated handling instructions in the pump user manual when changing pre-filled glass insulin cartridges. In some of the reported leakage incidents, the cartridges were found to be cracked and provided an inadequate supply of insulin to patients. Leakages also occurred in cases where no cracks in the cartridge were visible, the regulator said. In some patients there were consequences of not receiving enough insulin from their pump system, including reports of severely high blood sugar and diabetic ketoacidosis, a serious complication of diabetes when the body produces high levels of blood acids called ketones. Healthcare professionals are being advised to contact patients over the next six months using said device to discuss their individual needs and source an alternative pump where appropriate. "Because of the rare risk of insulin leakage from the Roche Accu-Chek Insight Insulin Pump, patients should check the pre-filled glass insulin cartridge for any cracks prior to usage," Dr June Raine, MHRA chief executive, said.

It helps to fast-track learning for the EnACT principles described in the framework, outlining how to involve patients in product innovation and critical issues such as data privacy, intellectual property, inclusivity, reimbursement, useability, and recruitment of patients.

Nearly all respondents reported patient frustration stemming from medicine supply issues, with 84 per cent of them saying they had experienced aggression from patients. Additionally, 98 per cent indicated an increase in 'owings,' where patients receive only part of their prescription and must return to the pharmacy for the remaining medication(s) at a later time."

Pain management: Enhancing pharmacist-patient consultations - 0 views

    Haleon's Community Pharmacy Pain Consultations Programme, piloted in Australia, has shown promising potential for enhancing pharmacies' roles in pain management. Running from January to May 2023, the programme was designed and tested by the Haleon Centre for Human Sciences, recognising the pressing need to optimise pain consultations between patients and pharmacists. The used a series of simple interventions, rooted in behavioural science, aimed at improving patient experiences in pharmacies. These include window posters and floor graphics to disrupt habitual behaviours and reframe patients' perceptions of the pharmacist's role. Results measured by Professor Colin Strong, head of behavioral science at IPSOS, indicated a significant increase in patient preparedness for discussing pain. Specifically, 92 per cent of patients reported feeling more prepared for these conversations after the interventions, up from 71per cent before.

Type 1 diabetes:NHS roll out life-changing glucose monitors - 0 views

    NHS England has secured a deal for 'Dexcom ONE Real Time-Continuous Glucose Monitoring' device which will help type 1 diabetes patients to keep track of their glucose levels at all times without having to scan or take a finger prick test. Once the patients receive their starter pack - which will include information on the product and usage, a sensor and transmitter - from the hospital or GP surgery, they can go to the community pharmacy for their repeat prescription. The wearable arm gadget sends information to a mobile app and allows diabetes patients to keep track of their glucose levels. "Traditionally, continuous glucose monitors are more expensive than their flash monitor counterparts - which record glucose levels by scanning a sensor - but thanks to the NHS agreeing on a new cost-effective deal with manufacturers DEXCOM, they will now be available for NHS patients on prescription at a similar price," said NHS. Dexcom ONE Real Time-Continuous Glucose Monitoring, uses a sensor no bigger than a bottle cap that attaches to the arm for up to 10 days and measures glucose levels from just under the skin. The wider rollout of the technology will help diabetes patients manage their condition better - reducing hospitalisations and associated diabetic illnesses which will ultimately ease pressure off the NHS. GSK's Momelotinib Nears Approval for Myelofibrosis - 0 views

    The Committee for Medicinal Products for Human Use (CHMP) of the European Medicines Agency (EMA) has recommended approving an oral therapy to treat myelofibrosis patients with moderate to severe anaemia. Myelofibrosis is a rare blood cancer that affects the body's normal production of blood cells, and the affected patients are likely to develop anaemia over the course of the disease. British drugmaker GSK, the manufacturer of the oral therapy known as momelotinib, said that it can be used to treat "both newly diagnosed and previously treated myelofibrosis patients." The medicine helps address disease-related splenomegaly (enlarged spleen) or symptoms in adult patients who are Janus kinase (JAK) inhibitor naïve or have been treated with ruxolitinib, as stated by the company. Nina Mojas, Senior Vice President of Oncology Global Product Strategy at GSK, said that receiving the positive CHMP opinion "is a significant step in bringing momelotinib to patients in the EU with this difficult-to-treat blood cancer."

    More than a third of pharmacists who participated in a survey said they have seen an increase in patients declining prescriptions due to the cost in the last 12 months. Responding to the survey, 90 per cent of pharmacists admitted seeing cases where patients decline all the medicines on a prescription due to cost. They have warned of the impact of prescription charges as patients are denying vital medicines, including those for blood pressure and mental health, inhalers, antibiotics, pain relief, and statins, which can have potential consequences for the individual's health.
