Among white people, however, those with HDL levels below 40 milligrams per deciliter had a 22% higher risk for coronary heart disease compared with those whose HDL levels were higher. High HDL levels (above 60 mg/dL), which are thought to be protective, were not linked with lower coronary heart disease risks in either race, researchers found.

    The Pharmaceutical Services Negotiating Committee (PSNC) has announced the margin delivery rates will increase from October as the agreed £100m write off is implemented. "The baseline margin allowance will remain £800m per year in 2022/23 and 2023/24," said PSNC. "Margin levels in the last two full years (2020/21 and 2021/22), as measured by the Margin Survey of independent pharmacies, were in excess of the baseline allowance, meaning that an excess was accrued." As new services have been introduced and take up of these has accelerated, the amount of unallocated funding left in the CPCF is now less than it previously was. As such, the value of the Transitional Payments will decrease from its current level. Some of the unallocated funding remaining in 2023/24 will be repurposed into a Flat Payment for all contractors (see details below). Over the course of the second half of 2022/23, the value of the Transitional Payments will be phased down. This will start in October 2022 with a reduction in payment levels to approximately 85% of the current level, as shown in the following table:

"Traditionally, continuous glucose monitors are more expensive than their flash monitor counterparts - which record glucose levels by scanning a sensor - but thanks to the NHS agreeing on a new cost-effective deal with manufacturers DEXCOM, they will now be available for NHS patients on prescription at a similar price," said NHS. Dexcom ONE Real Time-Continuous Glucose Monitoring, uses a sensor no bigger than a bottle cap that attaches to the arm for up to 10 days and measures glucose levels from just under the skin. The wider rollout of the technology will help diabetes patients manage their condition better - reducing hospitalisations and associated diabetic illnesses which will ultimately ease pressure off the NHS.     The Pharmacists' Defence Association (PDA) has raised concerns over "certain elements" of the Pharmaceutical Society of Northern Ireland (PSNI)'s consultation on pharmacy staffing levels. PSNI recently consulted on its draft guidance for pharmacy staffing levels which sets out to explain what pharmacy owners and superintendents must do to ensure that each pharmacy has enough skilled and qualified staff. This includes having an appropriate skill mix, to provide safe and effective pharmacy services which comply with the standards. PDA said that the role of the 'responsible pharmacist' has not been "fully acknowledged" and that more clarification is needed. The association has made a few recommendations to PSNI that includes: the guidance must explicitly recognise the statutory role of the responsible pharmacist and their authority in securing the safe operations within the pharmacy. "The guidance should specify that pharmacy owners and superintendents must ensure that they do not set incentives or targets which may compromise the professional judgement of staff." It also recommended that unregistered staff (including unregistered managers) are accountable firstly to the responsible pharmacist for any activity involving the clinical care of patients (for example unregistered managers must not interfere in diary bookings for vaccinations).

    Leaders from across the pharmacy sector and trade bodies dwelled on the challenges faced by the pharmacists at the Sigma Conference held on Sunday (2 October). "If the cost of carrying out regulation is higher than the benefits, it's time for all the bodies to sit together and talk on what can be adjusted accordingly," said Salim Jetha, Chairman, Avicenna. Jetha highlighted the challenges faced by the independent pharmacy and also raised the concern about the 'level playing field' for pharmacists compared to GPs. He said: "GPs get paid for energy cost, premises cost, the staffing cost. We don't get paid for that. So don't be misled by saying we are being exactly the same. Yes, on paper we are, but in reality we are not. I think also the rules of engagement are different." He called for better collaboration with GPs, and urged PSNC and other trade bodies to ensure a level playing field. Helen Knight, interim director of medicines evaluation at NICE, said: "Icosapent ethyl is the first licensed treatment of its kind for people who are at risk of heart attacks and strokes despite well controlled LDL cholesterol because they have raised blood fats. And although lifestyle changes, including diet and exercise, can help to reduce their risk, these may not work for everyone.
