In a decisive move highlighting the financial strain faced by community pharmacies, the National Pharmacy Association (NPA) submitted a £108 million invoice to the Department of Health and Social Care (DHSC) today (16 May). According to the NPA, this substantial sum represents the amount that pharmacies in England personally covered for the dispensing of NHS medicines last month, as a result of inadequate funding. "The £108m figure is an average monthly figure based on the loss to pharmacy incomes over the past decade," it said. The association believes that mass closures can be prevented only when the government stops expecting pharmacies to subsidise the cost of delivering NHS care. NPA chief executive Paul Rees, said: "The soaring costs of dispensing medicine coupled with declining real terms funding has led to community pharmacies in England having to subsidise the dispensing of drugs to the tune of £108m a month.

The UK government has announced plans to update legislation to enable more services and individuals to provide take-home supplies of naloxone - a life-saving opioid overdose antidote - over the next few weeks. Naloxone can immediately reverse the effects of an opioid overdose by reversing breathing difficulties. The medicine can be administered by anyone in an emergency. However, current legislation allows only drug and alcohol treatment services to supply it without a prescription to individuals for future use. The Department of Health and Social Care (DHSC) confirmed on Tuesday (14 May) that more professionals, including nurses, paramedics, police officers, probation officers and pharmacy professionals, will be able to supply the medicine without a prescription to save lives. Health and Social Care Secretary Victoria Atkins warned that opioid addiction can ruin lives and it accounts for the largest proportion of drug-related deaths across the UK.

The Department of Health and Social Care (DHSC) has finally published its response to the 2022 consultation on hub and spoke dispensing. Considering the consultation evidence and further discussions, the government has expressed its intention to progress the proposals for enabling hub and spoke models across different legal entities as soon as possible. This will be achieved by using the enabling powers outlined in Part 2 of the Medicines and Medical Devices Act 2021 (MMDA) to amend the Medicines Act 1968 and the HMRs. Furthermore, the DHSC has decided to proceed with the implementation of the two models of hub and spoke dispensing that it consulted on. The government response to the consultation reads: "Having considered the responses, the government intend to proceed to implement the necessary changes to Medicines legislation to remove the current restrictions that prevent the hub and spoke dispensing models from operating across different legal entities found in section 10 of the Medicines Act 1968.

NHS England will collaborate with "a number of system suppliers" to increase the number of trusts using electronic prescription service (EPS) over the next few years. Nishali Patel, clinical lead for digital medicines at NHS England, announced it during the Clinical Pharmacy Congress, held at the Excel, London, on Friday, 10 May 2024. According to Nishali, at least half of all NHS secondary care trusts could be using EPS by 2026. This digital service allows prescribers to send prescriptions electronically to a patient-nominated pharmacy. It is widely used in primary care, where over 95 per cent of prescriptions are now electronically generated. NHS recently extended the service to secondary care settings.

A report released by Community Pharmacy England (CPE) has warned that pharmacies across England are grappling with daily medicine supply challenges, posing significant risks to patients' health. The Pharmacy Pressures Survey 2024: Medicines Supply Report, which is based on the views of the owners of over 6,100 pharmacy premises in England and 2,000 pharmacy team members, has exposed some alarming trends concerning medicine supply problems in the country. Almost all pharmacy team members surveyed (97 per cent) reported patients being inconvenienced due to medicine supply issues, and 79 per cent of the participants said that the worsening situation is putting patient health at risk. Nearly all respondents reported patient frustration stemming from medicine supply issues, with 84 per cent of them saying they had experienced aggression from patients. Additionally, 98 per cent indicated an increase in 'owings,' where patients receive only part of their prescription and must return to the pharmacy for the remaining medication(s) at a later time."

The Department of Health (DoH) in Northern Ireland has published a new Strategic Plan for Community Pharmacy, highlighting the vision and key priorities for the sector over the next 10 years. Co-developed by the DoH and Community Pharmacy Northern Ireland (CPNI), the representative body for community pharmacy owners, the plan aims to fully realise the potential of community pharmacy services to support better health outcomes from medicines and prevent illness. The vision outlined in the plan sees community pharmacies offering more clinical services, leveraging the skills of their teams to provide the public with safe, convenient, and faster access to care. According to DOH, the vision will be realised through four major reform programmes focusing on services, legislation, workforce, and digital enablers, alongside six strategic priorities. Together, these initiatives are anticipated to transform the role of community pharmacy within the Health and Social Care (HSC). Health Minister Robin Swann has extended his support to the new Community Pharmacy Strategic Plan, which builds on the previously agreed 3-year community pharmacy commissioning plan.

Hospitals and healthcare providers across the UK have been notified about the shortage of an anti-inflammatory medicine caused by a manufacturing failure. The Department of Health and Social Care (DHSC) on Friday (3 May) issued a National Patient Safety Alert stating that Erelzi 50mg solution for injection in pre-filled pen will be out of stock from early May until mid-July 2024. "The supply issues have been caused by a manufacturing failure resulting in delays in production and the implementation of global allocations," it said. However, it noted that homecare providers (Alcura, Healthnet and Sciensus) have sufficient stock to supply existing patients until mid/late May 2024. Although Erelzi 50mg pre-filled syringes (PFS) are still available, they cannot support a full increase in demand, the DHSC warned.

The Royal Pharmaceutical Society in England (RPS) and Community Pharmacy England (CPE) have raised alarm over the impending increase in NHS prescription charges, slated to take effect from May 1, 2024. The RPS, represented by Chair Tase Oputu, condemned the rise, labeling it a "dark day for patients" who will now face nearly £10 per prescription item. Oputu emphasised the disproportionate impact on individuals with low incomes, citing the "relentless annual increases in prescription charges" as exacerbating the affordability crisis in healthcare. " Every day pharmacists are asked by patients who are unable to afford all the items in their prescription which ones they can 'do without'" she said. She further added that the financial barriers to get vital medicines should not be faced by any one and advocated for "the abolition of prescription charges in England, as has been implemented in other parts of the UK".

The Medicines and Healthcare products Regulatory Agency (MHRA) is doubling down on its efforts to combat online pharmacy frauds, emphasising innovation, eradication of legacy systems, and cybersecurity in its updated technology roadmap. The online sale of Medicines and medical devices is a "growing phenomenon offering convenience, anonymity, and lower prices " for the public. However the lack of robust cyber monitoring also "poses significant risks", as many online pharmacies are "unregulated, illegal, or fraudulent." According to the MHRA's recent report highlighting the impact of artificial intelligence on the regulation of medical products, the strategic themes of "fundamental to the successful deployment of AI" are essential for protecting consumers from fraudulent medical products.

In a recent report released by the Company Chemists' Association (CCA), data from the NHS Pharmacy First service indicates a substantial uptake and positive impact on patient care just two months after its launch. The CCA's findings shed light on the service's efficacy in meeting patient needs and its potential to alleviate pressure on General Practices (GPs) within the NHS. According to the report, over 90,000 Pharmacy First consultations were conducted solely by CCA members within the initial two-month period. Impressively, 88 per cent of these consultations, totaling 81,627, were deemed eligible for NHS-funded care indicating a significant demand for the service among patients. In the first month, the data revealed that more than 90 per cent of eligible patients received necessary care directly through Pharmacy First, bypassing the need for further referral.

In a recent announcement the Medicines and Healthcare products Regulatory Agency (MHRA) unveiled its strategic approach to artificial intelligence (AI), as it sets out to embrace the transformative potential of AI in healthcare regulation. Building upon the Government's white paper 'A pro-innovation approach to AI regulation', released in 2023, the MHRA outlined five key strategic principles guiding its adoption of AI. These principles encompass safety, security, and robustness; appropriate transparency and explainability; fairness, accountability, and governance; and contestability and redress. A science-led organization, the MHRA was tasked with providing an independent view of its approach to AI in medicine and science, aligning with a government white paper. Recognising its role in UK's goal to be a science and tech leader by 2030, MHRA is tasked to evaluate AI's opportunities and risks across regulatory, public service, and evidence-based decision-making area.

A full CPE Committee Meeting was organised in London on April 17 and 18, 2024, to discuss crucial sectoral issues. Key topics discussed during the two-day meeting included intolerable pressures on pharmacy owners, the ongoing Community Pharmacy Contractual Framework (CPCF) negotiations, implementation of the Pharmacy First service, and governance changes. Committee Members reviewed the progress of CPCF negotiations with the Department of Health and Social Care (DHSC) and NHS England. These critical negotiations are being led by the Negotiating Team (NT), which includes independent pharmacy owners and representatives of CCA and non-CCA multiples. The CPE has been advocating for an uplift to the core global sum, margin write-offs, an agreed mechanism for regular funding increases linked to activity and inflation, annual uplifts to service fees, more fundamental reform of the margin delivery framework and an economic review of the medicines supply chain. The Committee also discussed the results of the 2024 Pharmacy Pressures Survey, which was conducted during March and April.

The 14th Community Pharmacy Conference by Sigma Pharmaceuticals has been timely, prime minister Rishi Sunak said, noting that it coincided with the launch of Pharmacy First, a new initiative that enables patients to receive treatments for seven common conditions directly from a pharmacist without the need for a GP appointment or prescription. "We are all grateful for the tremendous contributions that pharmacists make to our NHS. The fantastic work you do is critical to the success of Pharmacy First and many other initiatives," Sunak said in a written message. The conference was organised from 25-29 February at Sun City in Johannesburg, South Africa, under the theme 'community pharmacy in an integrated NHS'. In a recorded video address to the Sigma Conference, shadow health secretary Wes Streeting said community pharmacies are 'critical' to the Labour party's mission to make the NHS 'fit for the future'. "You should be playing a much more significant role. Labour's reform agenda will get pharmacists working to the top of their licence. We want community pharmacists to play a greater role in healthcare, with more focus on their expertise in prescribing and medicines management," he said.

Adding to the growing evidence of global medicine shortages, a new study has revealed that the European generic medicines market is "not in shape" to help Europe meet its public health priorities. In the past decade, the rate of generic medicines withdrawals has risen by 12 per cent, while there has been a three per cent decrease in the launch of generic products, as per Teva Pharmaceuticals' recent analysis of IQVIA data. Within the mental therapeutic area, seven per cent of generic products disappeared between 2013 and 2023, while there was a seven per cent decrease in the availability of generic cancer medicines in just six years (2017-2022). These medicines were listed on the Union List of Critical medicines to help avoid potential shortages, as the European Commission (EC) said this could cause "significant harm to patients and pose important challenges to health systems." While mature generic products constitute the majority of the List, they remain susceptible to withdrawals, despite containing products crucial for safeguarding Europe's public health, the Teva analysis report noted. Since 2013, the number of generic products for the treatment of schizophrenia and bipolar disorder has declined by 25 per cent, with Hungary and Bulgaria experiencing the biggest loss at 83 per cent and 58 per cent respectively.

The UK COVID-19 Inquiry began on 28 June 2022 to examine the UK's preparedness and response to the pandemic, aiming to draw insights for the future. Its investigations are organised into modules, gathering evidence from witnesses, experts and core participants through a series of corresponding hearings throughout each module. A preliminary Module 3 hearing for its investigation into was held at Dorland House, 121 Westbourne Terrace, London, W2 6BU on Wednesday 10 April at 10.30am. NPA calls for funding to boost pharmacy resilience Presenting its evidence before the inquiry, the NPA highlighted the unsung contribution of pharmacies nationwide in combatting COVID-19 and maintaining health services throughout the pandemic while calling for funding to create greater resilience in community pharmacies.

In a recent announcement, the British supermarket chain, Asda has launched its first online prescription service becoming the first self-serving shop to offer prescribed medicine supply at the doorsteps through its latest healthcare accessibility venture. The NHS certified online pharmacy service, Asda Online Pharmacy, is set to offer patients a digital platform designed to simplify the prescription process for patients, offering the convenience of online management and home delivery with their partnership with Pharmacy2U. Effective immediately, new patients can register for an account on the platform via pharmacy.asda.com. Once registered, they gain access to a seamless prescription ordering system that facilitates home delivery, along with the ability to monitor the progress of their orders. Prescriptions submitted through Asda Online Pharmacy will undergo rigorous scrutiny by qualified pharmacists before being dispatched directly to patients' doorsteps, all without any additional charges.

The General Pharmaceutical Council's Investigating Committee took decisive action on March 25, 2024, by issuing a warning to Dilsha Kiran Shah, registration number: 2049787, for her conduct as Superintendent Pharmacist of Jhoots Healthcare Ltd. Miss Shah's failure to uphold the Medicines Act 1968, specifically in ensuring the proper management of medicinal products, triggered this warning. The Act mandates supervision in "keeping, preparing, and dispensing medicinal products other than those on a general sale list." Moreover, pharmacies are inspected on five principles - Governance, Staff, Premises, Services including Medicines management, and Equipment and facilities to meet the right standards. The alarm was initially raised in August 2021 when a Jhoots Healthcare Ltd pharmacy branch under Miss Shah's supervision operated without a responsible pharmacist. Despite explicit instructions, pharmacy staff were allegedly instructed to proceed without proper supervision, raising grave concerns regarding patient safety.
