Approximately 25 million GP appointments and 5 million A&E visits are utilised each year for self-treatable illnesses. If these individuals chose to self-treat using over-the-counter (OTC) medications, the potential savings for NHS could amount to at least £1.7 billion annually, according to a research commissioned by Proprietary Association of Great Britain. The study, conducted by analysts at Frontier Economics, revealed that embracing self-care could bring an additional economic benefit of £350 million annually, as employees would avoid unnecessary time off from work for medical appointments. Apart from the savings resulting from the increased OTC medicine use, the report emphasises the potential benefits of reclassifying more prescription-only medications (POMs). A mere 5 per cent reduction in NHS prescribing levels and spending could lead to an annual cost-saving of £1.4 billion, encompassing both prescription costs and GP appointments, it said. "This substantial amount could greatly alleviate financial pressures on the NHS and offer essential resources for recruiting and training much-needed healthcare professionals."

Proprietary Association of Great Britain, PAGB - which represents manufacturers of branded over-the-counter (OTC) medicines, self-care medical devices and food supplements - on Tuesday (12 December) celebrated 40 years of reclassification of medicines. Expanding self-care for common ailments and minor injuries will not only help ease pressure on primary care services, but also provides significant potential savings for the NHS, the consumer healthcare association highlighted during the event. Nurofen (ibuprofen) and Imodium (loperamide) were the first medicines to go through Medicines and Healthcare products Regulatory Agency (MHRA)'s switch process in 1983 to make them available over the counter - OTC. Other reclassified medicines which were once available on prescription only include Voltarol (diclofenac dimethylammonium), Canesten (clotrimazole), Nexium (esomeprazole), Nicorette, Nicotinell and Niquitin (nicotine replacement therapies), Regaine (minoxidil), Viagra Connect (sildenafil), Cialis Together (tadalafil) for erectile dysfunction and Gina (estradiol hemihydrate) for the treatment of postmenopausal symptoms such as vaginal dryness.

The Department of Health and Social Care has clarified that community pharmacists will not be among those allowed to issue sick notes, calling the service unsuitable to be provided over the counter. In response to a question from Pharmacy Business, a spokesperson for the department said that the policy change was "aimed at pharmacists working within general practices or hospital settings", adding: "The intention is that 'where' the fit note is issued will remain the same but 'who' issues it will change. The DHSC went on to explain that fit notes, known technically as 'Med3', were "not part of the NHS commissioned services" and therefore private providers, such as high street pharmacies, were unable to provide it under government provision. It added that since "fit notes should only be issued following a full assessment of a patient's fitness for work", it "would be an unsuitable service to be provided 'over the counter'.

A community pharmacist who supplied a drug addict with "under the counter drugs" has been jailed for 18 months. Dushyant Patel, 67, a London pharmacist with more than 40 years' experience, had supplied class C drugs to a drug user in Norwich for months in 2020. Police identified Patel as a suspect four months after the death of drug user, Alisha Siddiqi, whose body was found at a property in Colossus Way, Costessey, in August 2020. An initial post-mortem examination was inconclusive, but toxicology results later showed she died from an overdose of prescription medication. An analysis of her phone revealed that she had frequent communication with Patel between January and August 2020. Class C drugs sold without prescription There was also communication regarding transactions relating to the sale of prescription drugs including class C drugs, without a prescription, namely Zolpidem and Zopiclone. Patel was identified as a suspect and later charged with drugs offences. He was jailed at Norwich Crown Court in December after a trial in August when he was found guilty of two counts of being concerned in supplying a controlled drug between March and August 2020.

The Medicines and Healthcare products Regulatory Agency (MHRA) has launched a public consultation on plans to reclassify a locally-applied hormone replacement therapy product to treat the cause and relieve the symptoms of vaginal atrophy. The MHRA is proposing that Gina 10 microgram vaginal tablets, containing oestradiol, be made available over the counter as a "self care" product, under the supervision of a pharmacist. The Commission on Human Medicines too has advised that the application by Novo Nordisk, which is used to treat vaginal dryness, caused by oestrogen deficiency in postmenopausal women, can be made available as a pharmacy (P) medicine. The MHRA has been asking GPs, pharmacists and members of the public for their opinions on whether the tablets can be made available to women aged 50 and over who have not had a period for at least a year. If the reclassification goes ahead, pharmacists will be given training materials and a checklist to help them supply the medicine safely.

Dry eye disease, also known as dry eye syndrome, occurs when the eye stops producing enough tears, the tears that are made are of poor quality or the tears evaporate faster than they should. Over the last few years, we've seen the aftermath of Covid-19, with the demand for consumer health products being unprecedented and unpredictable, placing pressure on supply chains and labour market. In turn, the industry has experienced acute shortages across the healthcare sector, placing huge pressure on pharmacists, doctors, nurses, and other healthcare professionals. Brits are increasing relying on over-the-counter (OTC) medicines to prevent or treat common ailments, as the 'care at home trend' has become prominent amongst UK households following the COVID-19 pandemic. Additionally, the research, commissioned by Lanes Health, revealed that public awareness about preventative treatment has increased, with 70 per cent of Brits claiming that they do their best to prevent the spread of common illnesses since the pandemic.

The UK based Pharma giant Futura Medical Plc's lead product, Eroxon, a topical Stim-gel is now available on NHS prescription in England and Wales for the treatment of Erectile Dysfunction (ED). The Muti-Drug Resistant (MDR) approved alternative to PDE5i, Eroxon is available as an over-the counter off shelf at retailers like Boots and Superdrug; and is available on NHS prescription in England and Wales as a CE-marked medical device. Erectile dysfunction (ED), also known as impotence, is the inability to get and maintain an erection, typically concerning men over the age of 40 due to both, psychological and physical causes such as anxiety, depression, diabetes, and obesity. According to UK Conformity Assessed (UKCA), the approval of the medication is based on evidence from 2 Phase three clinical studies, which showed that 60 per cent of patients taking Eroxon achieved Minimal Clinically Important Differences (MCID) at 12 weeks.

The Medicines and Healthcare products Regulatory Agency (MHRA) has launched a consultation on the reclassification of codeine linctus to a prescription-only medicine after considering multiple Yellow Card reports for its opioid effects, rather than for its intended use as a cough suppressant. This medication is currently licensed as a pharmacy medicine, which means that it is available to purchase over the counter in pharmacies. If reclassified as a prescription-only medicine, all strengths of codeine linctus will only be available upon presentation of a prescription. Dr Alison Cave, MHRA Chief Safety Officer, said: "Codeine linctus is an effective medicine, but as it is an opioid, its misuse and abuse can have major health consequences. Every response received will help us to develop a broader view on whether codeine linctus should be restricted to prescription-only status. We want to hear from members of the public, health professionals and others who would be affected by this potential change so we can make a properly considered decision for the benefit of patients, carers, and healthcare professionals across the UK.
