The 1000th Building the Community-Pharmacy Partnership (BCPP) programme in Centred Soul, Newry received funding from the Department of Health in Northern Ireland to improve the health and wellbeing of women in the local community. BCPP programme is led by Community Development and Health Network (CDHN) in Northern Ireland and is funded by the Department of Health. It supports communities and community pharmacists to address locally defined needs so that people make connections, listen to and understand each other better and work together to address the social determinants of health and health inequalities Department of Health's Chief Pharmaceutical Officer, Cathy Harrison said: "I am delighted to mark the 1000th project to receive funding to improve health inequalities in the Newry area. The Welsh government has reached an agreement with the NHS and Community Pharmacy Wales (CPW)-the statutory representative body for all community pharmacies-on a six per cent funding increase as part of the 2024/25 Community Pharmacy Contractual Framework (CPCF). The £9.9 million increase will bring the total CPCF funding to £175 million per year, representing a 24 per cent rise in funding since 2017. In a statement published on 20 December 2024, Jeremy Miles, cabinet secretary for health and social care, said that the additional investment will be used to address "both immediate pressures facing community pharmacies and to maintain momentum on our longer-term ambitions for reform." Miles also said that the government would continue to invest in integrating pharmacies in primary care clusters, developing the skills and scope of practice of the wider community pharmacy workforce including pharmacy technicians, and the Choose Pharmacy IT system. In contrast, community pharmacies in England are still awaiting progress on CPCF negotiations for 2024/25. Last month, Community Pharmacy England (CPE) warned of the sector's worsening financial situation, urging ministers to provide immediate funding relief.

A policy brief, developed by researchers from the University of Bath and University of Strathclyde with funding from Sigma Pharmaceuticals, has recommended the Department of Health and Social Care (DHSC) and NHS to increase community pharmacy funding to avoid damaging closures and diminution of quality. The report launched on Wednesday (12 June) at an event in the House of Commons, attended by Members of Parliament, senior policymakers and the pharmacy industry, analysed community pharmacy policies and spoke to stakeholders to explore their opinions of the future of community pharmacy. It was found that patients value their community pharmacies, but staff feel demotivated, insecure and undervalued. Stakeholders and policies suggested that in the future, medicines should be supplied by automated 'hub and spoke' dispensing, enabling community pharmacy staff to provide services that relieve pressure on GP surgeries, such as long-term conditions management, urgent care and public health.

Labour MP Barry Gardiner has raised concerns that the £645 million cash injection announced by the government for community pharmacy is, in effect, 'frozen funding' for the next two years. Speaking on Tuesday (May 9) on the BBC's Politics Live, Gardiner said funding for community pharmacies "was cut before 2015, then it was frozen and now the latest announcement says that it's going to be frozen for a further two years." He said with a current national contract, which already represented a 25 per cent funding cut in real terms, it was not appropriate to ask for community pharmacy "to take on more work" without adequate support. "I heard the prime minister earlier this morning (May 9) on TV saying 'we are investing more in pharmacies'. "No you are not. You just concluded a contract that says for the next two years it will be absolutely flatlined." He added that "there will be no more money" for community pharmacy and that steeply rising costs due to inflation were "eating into that contract's health". Gardiner said community pharmacists "do a phenomenal job" but regretted that from a total of just 11,000, some 700 pharmacies have been lost due to funding cuts.

Achieving the community pharmacy vision necessitates legal adjustments and additional funding beyond recent boosts, the pharmacy vision document published by Nuffield Trust and The King's Fund has revealed. It says community pharmacies in England must adapt to changing population needs amidst unprecedented health and care challenges. Commissioned by Community Pharmacy England, the report titled 'A Vision for Community Pharmacy', published on Sept. 19, is a result of almost a year of research and extensive consultation with the sector and wider stakeholders, advocating for community pharmacy. The think tanks detail a shift in the community pharmacy sector, highlighting how pharmacies can contribute to crucial policy objectives regarding population health, prevention, and the increasing demand in primary care. "The health and care needs of the population are evolving, and pharmacies in England, like all components of the health and care system, must adjust to address these changes," said Helen Buckingham, Director of Strategy at Nuffield Trust.

The Parliamentarians have called the government to urgently consider the short-term funding to stabilise the community pharmacy sector. 39 MPs and Peers from across the political spectrum have signed letters to the Health Secretary, Steve Barclay MP and raised their growing concern about the pressures facing England's community pharmacies and the risks this poses to patient care and safety. "Steve Barclay MP received correspondence from 23 Opposition MPs, with six Peers also signing a letter. Separately, 9 Conservative MPs also wrote to the Health Secretary and up to 10 others have signalled their intention to write to the relevant Minister," said the Pharmaceutical Services Negotiating Committee (PSNC). The Parliamentarians acknowledged that pharmacies have the potential to do more to support the NHS and they called for the government to urgently consider a commitment to working with the sector to create a fair and fit for purpose future funding model that ensures sustainability and properly funds pharmacies for what they do. All signatories also called for the introduction of a fairly-funded Pharmacy First service.

Calling it "a severe blow to an already financially strained sector", McQuillan emphasised that the additional burden has the potential to threaten the viability of many pharmacies. The current funding and contractual framework for community pharmacy is not fit for purpose, the Health and Social Care Committee report has concluded, recommending a complete overhaul to reduce its complexity. Published today, the report highlighted the necessity for a new framework to ensure adequate funding and to prevent damaging cross-subsidy between clinical services and prescription dispensing, as is presently observed. The report revealed that community pharmacy funding has fallen by over 30 per cent in real terms since 2015, resulting in an annual shortfall of at least £67,000 per pharmacy for individual owners. More than 1,100 community pharmacies have closed since 2015, of which were serving the most deprived areas, it said. MPs on the committee have urged the government to address medicine shortages, warning that they could undermine flagship initiatives such as 'Pharmacy First' by eroding public confidence in pharmacists. "People living with type 2 diabetes, ADHD, epilepsy and cystic fibrosis, and those experiencing the menopause, have faced challenges accessing the vital medication they need. Many others are facing similar experiences.

Health secretary Wes Streeting has pledged to stabilise the community pharmacy system before enabling it to take on a broader role in healthcare, as he addressed the Health and Social Care Select Committee in Parliament on Wednesday (18 December). "Now that we have got the budget set, we will be consulting with Community Pharmacy England shortly in the New Year," Streeting stated. "I do want to reassure pharmacists across the country that we are taking into account the enormous pressures they are under when thinking about allocations so that we can stabilise the system and create the foundations from which to recover." The health secretary emphasised that his top priority is to "stabilise the [community pharmacy] system" before expanding initiatives such as Pharmacy First and independent prescribing. He also expressed a vision for community pharmacies to play "a bigger role… as part of the shift from hospital to community," but stressed that immediate steps are required to address the sector's current financial strain. The announcement that Community Pharmacy Contractual Framework (CPCF) negotiations will commence early in the New Year was reaffirmed by Streeting, echoing a statement made earlier in the week by minister for care Stephen Kinnock during a Westminster Hall debate.

The Independent Pharmacies Association (IPA), in collaboration with the Association of Optometrists (AOP), the British Dental Association (BDA) and the NHS Confederation has written to health secretary Wes Streeting, urging him to ensure sustainable investment in primary care to strengthen the NHS and better serve patients. In their letter, the group congratulated Streeting on his recent appointment as Secretary of State for Health and Social Care and requested a meeting to provide their joint constructive input towards improving health outcomes. They expressed concern that recent administrations have failed to follow through on promises to properly integrate and fund primary care, leading to deteriorating health outcomes. "Our collective experience is that our National Health Service (NHS), and health outcomes generally, have suffered at the hands of recent administrations that have talked much about the importance of properly integrating and funding primary care but not followed through with action," they wrote. The associations emphasised the need to transition the NHS to a Neighbourhood Health Service, where more care is delivered within local communities to identify problems earlier.

The University of Oxford's Centre for Research Equity (CfRE) has entered into a historic partnership with the Health and Social Care Research and Development (HSC R&D) Division in Northern Ireland to advance health equity across the region and ensure everyone benefits from healthcare advancements. Unveiled during an event at Parliament Buildings in Stormont Estate on Wednesday, this groundbreaking partnership makes Northern Ireland the first UK region to partner with CfRE. Supported by the Department of Health Northern Ireland and key health and social care agencies, the partnership aims to improve health outcomes and reduce disparities through collaborative research. Northern Ireland's Health Minister, Mike Nesbitt, highlighted that this new partnership represents "a significant step forward in efforts to address health inequalities" in the region. "By bringing together leading experts and resources from across sectors, we can work towards conducting research that truly benefits all of our communities," he said.

The government has responded to the Health and Social Care Select Committee's report on community pharmacy, acknowledging 17 of the 19 recommendations. In its report published on 29 May 2024, the Committee recommended, among other things, an overhaul of the Community Pharmacy Contractual Framework (CPCF), closing the funding gap, tackling medicines shortages by introducing generic substitution, and establishing an integrated and fully funded workforce plan for pharmacy. In its response, released today, the government stated that NHS England is currently conducting an economic analysis of the sector, which will inform future proposals for funding and contractual arrangements. On medicine shortages, it mentioned that the Department of Health and Social Care, working closely with NHS England, is taking forward a range of actions to improve their ability to mitigate and manage shortages and strengthen resilience. Additionally, the government has stated that it will "publish a refreshed Long Term Workforce Plan" this summer and is "committed to growing the pharmacy workforce."

In a significant move to address health disparities and enhance clinical research, community pharmacies across several regions are set to become central players in the Genes and Health study. Led by Queen Mary University of London and funded by the Wellcome Trust, this national initiative aims to improve health outcomes within British-Bangladeshi and British-Pakistani communities. Professor Mahendra Patel, Director of the Centre for Research Equity at the University of Oxford, highlighted the importance of this approach. "By engaging the broader pharmacy workforce, including technicians, we aim to establish a research-ready infrastructure within community pharmacies," Patel told Pharmacy Business. He shared that the initiative marks a transformative step in integrating pharmacies into the clinical research landscape. The Thames Valley and South Midlands Clinical Research Network (CRN) is managing a pioneering pilot program for this study, which will initially focus on pharmacies in Banbury, Oxford City, Reading, Slough, High Wycombe, and Aylesbury.

The Department of Health in Northern Ireland has advised pharmacies to continue providing rota services on Easter after learning that many pharmacies are unwilling to provide the services on Easter holidays. "It is extremely regrettable that community pharmacy representatives have advised that access to rota pharmacies in some parts of Northern Ireland will be reduced over the Easter holidays," department said. They have also stated that despite a commitment of recurrent investment, support for new patients assessed by Trusts as requiring blister packs may be restricted. These actions have the potential to impact on patients and other parts of the Health and Social Care system. The department has also called the CPNI's demand to increase the funding up to 50 per cent in the sector "unrealistic" in the current financial climate. It said: "Financial pressures across the entire NI public sector are severe with an extremely challenging health budget anticipated for 2023/24." "The Department is therefore facing a significant funding gap just to maintain existing services and the whole Health and Social Care system is an extremely unpredictable and fragile position. The core funding envelope available to community pharmacies in NI has increased by 16% over the past three years."

As we transition in England through yet another NHS organisational change, I ask myself what does this mean for community pharmacy? I would like to think that this change will bring about opportunity and a chance for community pharmacy to showcase and continue the excellent work that was carried out during the height of the pandemic and is still ongoing today. I hope that it allows community pharmacy to be regarded as part of the NHS rather than sitting on the side lines. This change has to lead to better funding for community pharmacy, without sufficient funding we will see more pharmacies close. We are hearing a lot about winter pressures but this year it feels like all year round pressure. What I have seen, whilst the NHS is under such pressure, is North East London (NEL) CCG transitioning to an integrated care board (ICB) almost seamlessly. I have seen people transitioning into new roles, whilst working hard to ensure that all plates are still spinning, which at the moment is no mean feat. I spent a day out recently visiting pharmacies with the chief medical officer of NHS NEL, Dr Paul Gilluley. The visits were positive, we felt listened to and understood. The feedback was great, it was recognised that community pharmacy is often the informal front door to the NHS and that we have so much to offer in terms of ill health prevention. Community pharmacy can offer a total solution as long as we have the tools to do so, which can save so much time and money. An example is the GP CPCS service, which has launched well across NEL.

The National Pharmacy Association (NPA) has called on the NHS and the Department of Health to immediately publish the government-commissioned analysis of pharmacy underfunding and reveal the true scale of the crisis. Commissioned by NHS England, the long-awaited independent review is expected to "lay bare the perilous financial state" of community pharmacies, which is a vital part of the nation's health infrastructure. However, the NPA is concerned that the government may delay its release until after current funding consultations are concluded. The NPA argued that the public needs to understand the fragile nature of the pharmacy network before any new deal can be agreed. It has also warned health officials not to hide "the true scale of funding needed to reverse a decade of swingeing cuts", which has forced record numbers of pharmacy closures. NPA chair Nick Kaye said: "It would be a scandal to keep that evidence buried secret and leave MPs, pharmacies and the public in the dark." He acknowledged that the new government has inherited a crisis in pharmacy funding and emphasised that it will need to take "strong action" to maintain access to medicines.

A combination of factors, including the doctors' strike, medicine shortages, and funding scarcity, indicates that this upcoming winter could be the most challenging period for both community pharmacies and the NHS, Community Pharmacy England has said. In a September 4 blog post, CPE Chief Executive Janet Morrison emphasised that the relentless pressures show no signs of easing. "Pharmacy businesses, like numerous other sectors, are grappling with the challenges of staying afloat in the current economic climate, compounded by the detrimental effects of historical funding cuts." "The doctors' strike, despite ongoing assurances from the Health Secretary that their pay deal is final, will inevitably exacerbate the demand for both primary and secondary care, leading people to turn to their local pharmacies for assistance," she added. "This, in turn, will heighten the pressure on community pharmacies." "Whatever the coming winter brings I am sure of two things: that community pharmacies are going to be more critical to the health of the population than ever before, and that your representatives here will be fighting on all fronts to get you the help that you need."

Politicians, researchers and pharmacy leaders called on the government to make community pharmacy a fundamental part of clinical services at an event at the House of Commons on Wednesday (13). At the launch of a research publication carried out by the University of Bath in conjunction with Sigma Pharmaceuticals, community pharmacy was described as an "essential pillar of health care provision" in the UK. The aim of this research was to provide evidence to support a strategy for the future developments of community pharmacy to ensure the sector continues to meet the needs of the public in the NHS. Researchers analysed 25 health and community pharmacy related policies published by the department of health, the NHS, the pharmacy profession, and spoke to stakeholders from a wide range of backgrounds, including patients, pharmacists, NHS commissioners and GPS.
